to your June 15,2011 request for a Department of the Anny penn it for the CDOW Glenwood Springs Regional Service Center project. This project involves activities, including discharges
of dredged or fill material, in waters of the United States to construct a regional customer service center. The project is located about five miles east of New Castle on the southwest
comer of exist 109 and 1-70 within the SE~NE ~ of Section 35 and SW ~NW~ of Section 36, Township 5 South, Range 90 West, Sixth Principal Meridian, Latitude 39.5728°, Longitude -107.4523°,
Garfield County, Colorado. Based on the infonnation you provided, the proposed activity will impact approximately 0.12 acre of an unnamed drainage and its associated wetlands. The proposed
activity is authorized by Nationwide General pennit number (NWP) 39 Commercial and Institutional Developments. Your work must comply with the general tenns and conditions listed on the
enclosed NWP infonnation sheets and the following special conditions: 1. To mitigate for the loss of 0.12 acre of waters of the United States, you shall purchase 0.14 acre of credits
from the Rocky Mountain Institute Mitigation Bank. Evidence of this purchase shall be provided to this office prior to proceeding with any activity otherwise authorized by this permit.
2. You must sign the enclosed Compliance Certification and return it to this office within 30 days after completion of the authorized work. This verification is valid until March 18,
2012, when the existing NWPs are scheduled to be modified, reissued, or revoked. It is incumbent upon you to remain infonned of changes to the NWPs. We will issue a public notice when
the NWPs are reissued. Furthennore, if you commence or are under contract to commence this activity before the date that the relevant NWP is modified or revoked, you will have twelve
(12) months from the date of the modification or revocation of the NWP to complete the activity under the present terms and conditions of this NWP. Failure to comply with the General
Conditions of this NWP, or the project-specific Special Conditions of this authorization, may result in the suspension or revocation of your authorization.
