Julie, This office is in receipt of the Special Use Permit Amendment Application to amend Resolution 2009 -41 to replace fourteen (14) 500 bbl existing frac tanks with five (5) 1,000
bbl closed top upright tanks with vapor recovery and SCADA including a provision to install three (3) additional 1,000 bbl closed top upright tanks in the future on property owned by
Chevron USA Inc. The application also indicates that all 24 existing frac tanks and secondary containment berm will be removed with the development of the proposed tanks and secondary
containment. The County has conducted a review of the application and determined it to be technically incomplete. Please provide the following information so that we may continue to
process the application: • Narrative addressing the Non-Substantial Change under Article XIV, Definitions of the ULUR; and, • Statement that Chevron is the mineral owner of the subject
site. Other Comments: • I believe the proposed application is on Parcels 2137-321-00-008 and 2139c163-00-014, please verify. • Please address the comments from Chris Hale's letter dated
May 12, 2011 and revise Site Plan accordingly. See attached letter. • Is a CDPHE air permit required for the new tanks? Once you have adequately addressed the items above, the Director
shall make a determination as to whether the proposed change constitutes a substantial modification to the condition of approval contained in the Resolution of Approval. Do not hesitate

CONSULTING AND DESIGN RE: SUAA-6800, SUP Amendment to Produced Water Management Tank Facility: Chevron U.S.A. Inc. Dear Molly: This office has performed a review of the documents provided
for the amendment to the Special Use Permit for the Produced Water Management System of Chevron U.S.A. Inc. The submittal was found to be thorough and well organized. The review generated
the following questions, concerns, or comments: I. Swale #1 and swale #2 drop into the diversion ditch from the top of the 3: I side slopes. Erosion should be expected and riprap or
other erosion control should be employed down 3: I side slopes. 2. The Applicant should discuss how the tanks are interconnected. If the tanks are interconnected and act as a single
storage system then the contailmlent berm should be for 150% of the total storage volume plus allowances for storm events and freeboard. 3. The secondary contain~ent berm does not appear
to be larger enough to anticipate the three future tanks. The benn should be enlarged for as necessary. Feel free to call ifany of the above needs clarification or if you have any questions
