DESCRIPTION OF INSPECTION WORK PERFORMED: Special Inspection performed for field welding of multi -pass,
flare -bevel and full -penetration moment connections as required per the special inspection requirements
adopted from the 2009 / 2012 IBC, structural engineer and Garfield County building department as part of the
approved plan.
LOCATION iN STRUCTURE Special inspection provided for work as detailed and approved by engineer for roof
level, grldlines K/9 & K/17.
REMARKS FOR STRUCTURAL STEEL: Witness field welding for all structural steel moment frame connections at HSS
18 x 6 x 1/2" header beam to TS columns as designed by Oddo Engineering, detail 7/S4.1 (structural engineer /
design professional of record). As per engineer, design needed to be modified due to Inability to make up
required A325 bolted connection; engineer required weld at shear plate to be performed as a multi -pass, flare -
bevel groove weld In lieu of bolted connection. Vertical multl-pass flare -bevel welds at TS header to column
connections (each side/ each / end - 4 total) per emailed directive 10-23-13.
All work performed in compliance by certified operators employed by Myers and Company for Structural
Associates as required by the 2009 / 2012 IBC, structural engineer, owner and local building Jurisdiction
(Garfield County). All work prepared in compliance with AWS D1.14 all welding designed as Full Penetration (CP)
or partial -penetration (flare -bevel groove) multi -pass welds were properly prepared and welded with approved
filler material. All discontinuities we repaired and re -welded in inspector's presence - No problems noted. Mill
certifications for steel elements reviewed and kept on file by fabricator.
ALL WORK REFERENCED ABOVE WITNESSED BY INDUSTRIAL INSPECTION SERVICES PERSONNEL, PERFORMED
ON OCTOBER TWENTY THIRD, TWO THOUSAND AND THIRTEEN, APPEARS TO HAVE BEEN SATISFACTORILY
COMPLETED AND CONFORMS TO THE APPROVED PLANS, THE 2009 / 2012 INTERNATIONAL BUILDING CODES
(IBC), THE ADOPTED PROVISIONS OF THE AWS D1.1 AND THE AISC MANU OF STEEL CONSTRUCTION (9TH
EDITION). THIS STATEMENT APPLIED DI; LY TO THOSE POR .HE STRUCTURE WHICH SPECIAL
