Annual Report of Operations
Dear Sir/Madam:
Garfield County Special Use Permit No. 2008-86 ("Permit') was granted in June 2008 to Black Diamond
Minerals, LLC ("BDM") to operate a shale borrow pit within the area known as Tepee Park Ranch
("TPR") located in Garfield County, CO. A report of the operations is noted below.
Extraction of shale rock at the TPR Borrow Pit is also permitted through the Colorado Department of
Mining, Reclamation and Safety (DRMS). A bond was posted with that agency as per their conditions of
approval. BDM has complied with the performance standards as outlined in the Garfield County SUP
and the conditions placed by the DRMS permit.
The Annual Activity Plan, as required under the City of Rifle Watershed Permit, is provided below, per
the SUP requirements.
All construction work done at this site was conducted under a Stormwater Management Permit
("SWMP") issued by the CDPHE. Best Management Practices were implemented as defined in the
SWMP. The required inspections were conducted and documented by Kleinfelder of Littleton, CO.
There were no incidents of non-compliance with the SWMP.
Annual Activity Plan (2013):
BDM has been intermittently mining the shale rock from the borrow pit since August 1, 2013, and
expects to continue until the end of the year. The rock has been used for road maintenance and
maintenance of well pad locations to support drilling activities.

Approximately 100 cubic yards of shale rock was taken from the borrow pit in November 2011 for French
drain construction at Well Pad 25A.
2012 Activities:
No extraction of shale rock is planned for the remainder of 2012. The project to drill natural gas wells is
deferred due to poor economic conditions. Work may be performed on the infrastructure to support the
development of natural gas, however no extraction of material from the borrow pit is anticipated in 2012.

Dear Sir/Madam:
Black Diamond Minerals (BDM) has plans to perform work under its City of Rifle
Watershed Permit No. 1-08, granted on June 30, 2008. In accordance with the Permit
Conditions, a description of those activities follows:
2011 Planned Activities (within the City of Rifle Watershed Boundary):
Within the City of Rifle Watershed boundary, BDM plans to install several pipelines in a
single trench as noted on the attached map. The pipelines will consist of 1-12" nominal
welded steel pipe and 14" nominal "Flexsteel" continuous reel pipe. The trench will be
approximately 4' to 6' deep and 36" wide.
2011 Planned Activities (outside the City of Rifle Watershed Boundary):
In areas falling outside but near the City of Rifle Watershed boundary, BDM plans
additional work, including:
• Installation of several pipelines in a common trench from Well Pad 25A to the
Central Processing Facilities. The pipelines will include: 2-12" nominal welded
steel pipe, 2-4" nominal "Flexsteel" continuous reel pipe and 1-6" nominal
welded steel pipe. The trench will be approximately 4' to 6' deep and 42" wide.
• Installation of a production header and associated piping to gather production
from future wells drilled on Well Pad 25A. There are currently no wells fully
drilled on this pad.

• Installation of a steel, lined tank berm at the Central Processing Facility to be used
as secondary containment for 2 current tanks. The berm can handle an additional
4 tanks if needed.
• Setting of production vessels and installation of associated piping.
Best Management Practices, as developed in our Stormwater Management Plan/Permit,
will be employed during this process. Although there are no wells capable of production
on this property, either inside or outside the City of Rifle Watershed, an Emergency
Response Plan is also in place. All of this work will be done on private property owned
by BDM. It is anticipated that work within the Watershed boundary will begin the week
of November 7 and be completed by late November, 2011. We do not anticipate that
snowplowing will be required to complete the work planned.

2010 Activity Summary
2011 Activity Plan
Dear Sir/Madam:
The City of Rifle Watershed Permit No 1-08 was granted on June 30, 2008 to Black
Diamond Minerals, LLC to conduct activities to develop natural gas resources within the area
known as Tepee Park Ranch (TPR). This report summarizes the work done in 2010 and the
work planned for 2011.
2010 Activities:
No wells were drilled in 2010 on either of the two well pads in TPR. Both of these well pads
lie outside the Watershed boundary.
No construction work was done in TPR in 2010. Some oilfield equipment was trucked into
TPR for storage until it is needed.
Salvage timber was hauled from TPR from June through October, 2010

22ti0'Potenital Activities:
There are no planned activities for 2011 for TPR. However, should the market allow
development to begin, the following activities could occur. The following activities would lie
within the Watershed:
Installation of a temporary gas sales pipeline, and associated facilities, from the
Central Production Facility (CPF), located outside of the Rifle Watershed on TPR, to a
tie-in point with ETC Gas Gathering which is located on the northern portion of the
City of Rifle March 14, 2010 Page 1
former Honea property. The permitting for this pipeline has been reviewed by the
USFS and a Decision Memo to authorize the Permit was granted on March 19, 2009.
All other activities that may occur (depending upon market conditions) in 2011 will lie outside
the City of Rifle Watershed boundary. They are listed here for your information.
Drilling and completing seven to ten (7-10) wells on Well Pad 25A.
If the wells are drilled, installation of production equipment, in -field gathering pipelines
and other associated equipment necessary to support the operation of these wells
would occur.
