CONTRACTOR: Mayan Piering Kurtz and Associates, Inc . is the structural engineer of record for the foundation and structural repairs for the noted single family residence . As such , of office provided structural construction documents specifying repairs to the existing foundation dated April 02, 2014. The documents specified and detailed the installation of eleven (l l ) hydraulically driven helical piers to stabilize and re-level the foundation of the attached garage and the area immediately adjacent to the garage . We also perfonned three (3) site inspections during the course of the foundation repairs and the subsequent structural framing repairs . FOUNDATION INSPECTION NO. 1 (05/14/29014): The excavations adjacent to the existing foundation walls were complete at this time . The existing perimeter , cast-in-place concrete foundation walls are in reasonably good condition despite the differential foundation movement that the residence has suffered . The existing concrete walls are 8" thick by 4'-0" high . One of the eleven piers to be installed was in conflict with the existing utility lines which serve the residence. During the course of my site visit I recommended and approved the re-location of that single pier: the pier was moved, in plan, approximately 2'-0". The attached partial foundation plan documents that revision. FOUNDATION INSPECTION NO. 2 (05/22/2014): I observed the foundation after the all eleven (11) helical piers had been installed and the foundation had been re-leveled . I also observed the boring logs for the hel ical piers . The installed pier depths were consistent with the anticipated depths. The pier brackets were properly attached to the existing concrete foundations . FRAMING INSPECTION (05/29/2014): I performed a framing inspectiQIJ to verify that the existing framing was properly plumbed and nailed . Framing is exposed in the attic above the garage. All of the attic bracing which had been displaced and separated from the attic trusses was re-positioned and re-nailed at the time of this inspection. The hurricane ties which attach the individual attic trusses to the top plate of the garage walls were in good condition : they were properly positioned and nailed . It is my professional opinion that the structural foundation and framing repairs have, at this time , been successfully and fully completed in accordance with the structural documents .
