1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine — 6
months in jail or both).

SPECIFICATIONS:
- Traffic compact the trench gravel & imported silty sand to at least 8596 of Maximum Standard Proctor, ASTM D-698
or 'water settle' the sands, prior to placing the screened grovel and distribution piping.
- Bottom of trenches to be graded to 0.04%, away from the distribution pipe.
- Soil type & conditions must be the same across the trenches and is to be confirmed by the Design Engineer.
- Do not compact or smear the native soils in the trenches during the excavation and installation process.
- Outfall lines from the house and the septic tank to be 4" dia. PVC.
- Perforated pipes to be 4" dia. and placed with the perforations down (5 & 7 o'clock positions).
- Sand Filter to be a Clean, Coarse Sand, Effective Size between 0.25 & 0.6 mm & Cu = 4.0 or less.
- Screened/Washed Gravel to be placed around the Piping, Minimum 2" cover on Sides and Base.
- Cover the Sand Bed & Gravel with Filter Fabric, Nonwoven, min. Grab Strength of 120 lbs.
Use Amoco 4547, Contech C-50NW, Mirafi 140N, or equal.
- System to be inspected by Design Engineer at:
1) bottom excavation, 2) bottom sand, gravel & pipe in-place, 3) silty sand in-place & 4) final.

Gentlemen:
Pcrsannel of Grand Junction Lincoln DcVore observed the mostly constructed but not backfilled Individual
Sewage Disposal System at the above -referenced site on October 16, 2006. The system was observed
utilizing the sewage field design prepared by Grand Junction Lioaeoln DeVore, Job #92503-Gi, dated August
8, 2006. The excavation for the piping and tranches encountered silty sands with gravels. and some sandy
clays, similar to those described in tht Report of Subsurface Soils Exploration and the soils that were tested
for a soil percolation rate. The soils in lite bottom of the trenches were somewhat sandier, containing less
clay, than those described in the soil profile log for soil percolation teeing at this depth. The soils were
observed to be slightly loosened by thee xeavation process but were not "smeared" or excessively compacted,
neither on the base nor on the trench sides.
The sou Is observed during our observation on October 16,2006 appeared to fit the design parameters and our
findings during the ensile percolation testing, Basal upon aur observations, no modifications to the sewage
Geld design performed by Grand Junction Lincoln DeVore are required as of the date of our observations.
