Owned By Chevron Inc. USA Located In Section 3, Township 5S, Range
97W, Garfield County."
Williams Production RMT was approved for a Temporary Employee Housing
facility on land owned by Chevron Inc. USA located 55.5 miles by road
(approximately 15 miles directly) North West of the City of Rifle and within the
Resource Lands (RL) — Plateau Zone District. This Special Use Permit (SUP) was
issued on December 4th, 2006 under Resolution 2006-115. Per Condition (11) of
Resolution 2006-115, "The maximum allowable time length of the Special Use
Permit for Temporary Employee Housing is one (1) year. For good cause shown, the
permit may be renewed annually in a public meeting with notice by agenda only.
Annual renewal review shall be based on the standards herein as well as all
conditions of the permit. A permit may be revoked anytime through a public hearing
called up by staff or the Board of County Commissioners." The one year renewal is
the purpose of the meeting with Williams Production RMT and the Board of
County Commissioners (BOCC) on November 19th, 2007.
Based on a review of this application, the following points are worth noting:
1. The Application was approved on December 4th, 2006 while the SUP was
signed by the Chairman of the BOCC on January 15th, 2007.
2. Individual Sewage Disposal Permit (ISDS) Number 4233 was issued on
October 10, 2006.

3. Staff understands that this facility has been in operation since the issuance
of the SUP on January 15th, 2007.
4. Following discussions with Williams Production RMT representatives and
a review of County Building Permit records, it is evident that a Building;
Permit for this facility has not been issued. ,
5. On November 2nd, 2007, Andy Schwaller, Garfield County Chief Building
Official issued a notice of violation to Williams Production RMT for
operating the above identified facility in violation of Section 105 of the
2003 IBC (International Building Code) enabled by Colorado Revised
Statutes 30-28-124 & 124.5.
Since the Applicant has not obtained the appropriate Building Permit for the above
identified facility and has not submitted verification that the site was installed in
accordance with all applicable regulations, permits and conditions, the Applicant is
currently out of compliance with the following Conditions of Approval as identified
within Resolution 2006-115:
Condition 2i- That the operation of the facility be done in accordance with all
applicable federal, state, and local regulations governing the operation of this
type of facility.
Condition #7 - All Special Use Permits for Temporary Employee Housing is
subject to ell applicable 'building code, state and federal permit requirements,
fire protection district requirements and fire code requirements.
Condition #17 - If a Special Use Permit for Temporary Employee Housing is
granted, ,the applicant shall notify the County when site development begins.
The applicant shall verify in writing, by site plan and through photo
documentation that the site, water system, and sewage disposal system were
designed, installed and inspected in accordance with the said special use
permit and comply with all applicable regulations, permits, and conditions.
All written documentation and site plans verifying compliance must be
stamped by a certified Colorado Engineer. The County also reserves the right
to inspect a site, without notice, to assess compliance with the Special Use
Permit for Temporary Employee Housing. A determination of noncompliance
with any Special Use Permit for Temporary Employee Housing, or condition
approval thereof, is grounds for revocation or suspension of said permit, in
accordance with Section §9.01.06.
To this end, as "annual renewal review,shall be based on the standards herein as well
as all -conditions of the permit", Staff has found that the Applicant is not in

conformance with the conditions of the SUP and is hence not eligible for renewal of
Resolution 2006-115.
Thus, Staff recommends that Resolution 2006-115 not be granted renewal and that
use of the facility be halted immediately.

WHEREAS, the Board of County Commissioners of Garfield County, Colorado ("Board"),
has received an application from Chevron USA, Inc. for a Special Use Permit ("SUP") to allow
Temporary Employee Housing on their property within the RL - Plateau (Resource Lands) zone
district; and
WHEREAS, the Board held a public hearing on the 4th day of December, 2006, upon the
question of whether the above described SUP should be granted or denied, at which hearing the
public and interested persons were given the opportunity to express their opinions concerning the
approval of said special use permit; and
WHEREAS, the Board on the basis of substantial competent evidence produced at the
aforementioned hearing, has made the following determination of fact as listed below:
1. Proper posting and public notice was provided as required for the meeting before the
Board of County Commissioners.

2. The meeting before the Board of County Commissioners was extensive and complete,
that all pertinent facts, matters and issues were submitted and that all interested parties
were heard at that meeting.
3. The above stated and other reasons, the proposed special use permit bas been determined
to be in the best interest of the health, safety, morals, convenience, order, prosperity and
welfare of the citizens of Garfield County.
4. The application has met the requirements of Special Use (Sections 5:03, 5.02.21 and
9:03) the Garfield County Zoning Resolution of 1978, as amended.
NOW THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Garfield
County, Colorado, that Special Use Permit for Temporary Employee Housing for a property owned
by Chevron USA Inc. is hereby approved subject to compliance with all of the following specific
conditions:
1. That all representations of the Applicant, either within the application or stated at the
hearing before the Board of County Commissioners, shall be considered conditions of
approval unless explicitly altered by the Board.
2. That the operation of the facility be done in accordance with all applicable federal, state, and
local regulations governing the operation of this type of facility.
3. Emissions of smoke and particulate matter: every use shall be operated so as to comply
with all Federal, State and County air quality laws, regulations and standards.
4. Emission of heat, glare, radiation and fumes: every use shall be so operated that it does not
emit heat, glare, radiation or fumes which substantially interfere with the existing use of
adjoining property or which constitutes a public nuisance or hazard. Flaring of gases,
aircraft warning signals, reflective painting of storage tanks, or other such operations
which may be required by law as safety or air pollution control measures shall be exempted
from this provision.
5. Water pollution: in a case in which potential hazards exist, it shall be necessary to install
safeguards designed to comply with the Regulations of the Environmental Protection
Agency before operation of the facilities may begin. All percolation tests or ground water
resource tests as may be required by local or State Health Officers must be met before
operation of the facilities may begin.
6. If applicable, the Applicant shall provide a copy of the appropriate bond supplied to the
Colorado Oil and Gas Conservation Commission (COGCC) intended to guarantee
reclamation of the Temporary Employee Housing site prior to the issuance of a Special Use

Permit. If the COGCC does not require reclamation and revegetation bonding, the applicant
shall work with County Staff to establish a bond appropriate to guarantee reclamation and
revegetation of the Temporary Employee Housing facility.
7. All Special Use Permits for Temporary Employee Housing is subject to all applicable
building code, state and federal permit requirements, fire protection district requirements
and fire code requirements.
8. Water and wastewater systems proposed to service temporary employee housing must
comply with all applicable state and local laws and regulations.
9. Applicants must keep appropriate records, to be provided to the County upon request to
demonstrate that water supplied to a site is from an approved source and that wastewater
is disposed at an approved facility. For facilities serving twenty-four (24) people or less,
the operator must conduct monthly tests (or quarterly if an on-site disinfection system is
installed) and maintain records of stored potable water samples specific for coli form.
Any tests indicating coli form contamination must be disclosed to the Garfield County
Board of Health or designee.
10. In no case shall unsafe water be used for drinking nor shall raw sewage or used water be
discharged on the ground surface.
11. The maximum allowable time length of the Special Use Permit for Temporary Employee
Housing is one (1) year. For good cause shown, the permit may be renewed annually in a
public meeting with notice by agenda only. Annual renewal review chall be based on the
standards herein as well as all conditions of the permit. A permit may be revoked anytime
through a public hearing called up by staff or the Board of County Commissioners.
12. Inhabitants of the temporary housing shall be applicant's employees and/or
subcontractors, working on the related construction or mineral extraction operation, and
not dependents of employees, guests or other family members.
13. Temporary employee housing sites shall be maintained in a clean, safe and sanitary
condition, free of weeds and refuse. Any hazardous or noxious materials that must be
stored on site for operational or security reasons must be managed in accordance with all
applicable federal, state and local laws and regulations.
14. Fire Protection General Requirements: Provisions shall be made for giving alarm in case
of fire. It shall be the responsibility of the duly authorized attendant or caretaker to
inform all employees about means for summoning fire apparatus, sheriff's office and
resident employees. All fires are subject to §307 of the 2003 International Fire Code
(IFC) including but not limited to permits, attendance, open fires, coal grills, fire. bans and
bon fires. One (1) or more approved extinguishers of atype suitable for flammable liquid

or electrical fires (Class A, Class B and Class C), carbon dioxide or dry chemical, shall be
located in an open station so that it will not be necessary to travel more than one hundred
(100) feet to reach the nearest extinguisher.
15. Outdoor food storage is prohibited unless facilities that prevent the attraction of animals
to the temporary employee housing site are provided.
16. The applicant shall provide a detailed map and GPS coordinates to the Garfield County
Sheriff's Office and the relevant Fire Protection District which is sufficient for emergency
response purposes, including location of the temporary employee housing site; private
and public roadways accessing the site, marked as open, gated and/or locked; and detailed
directions to the site from a major public right-of-way. The map is subject to approval by
the Garfield County Sheriff's Office and relevant Fire Protection District.
17. If a Special Use Permit for Temporary Employee Housing is granted, the applicant shall
notify the County when site development begins. The applicant shall verify in writing, by

I. DESCRIPTION OF THE PROPOSAL
The Building and Planning Department received a Special Use Permit (SUP) application for
"Temporary Employee Housing" on property owned by Chevron USA, Inc located on an 17,013 acre
property approximately 15 miles North West of the Town of Parachute "as the crow flies" (See
location maps). By vehicle, this property is 55.5 miles North West of the City of Rifle (See proposed
emergency services map route, P.9).
More specifically, the Applicant, represented by PVCMI Land Planning Division, requests approval
for one Temporary Employee Housing site to house personnel for the purpose of winter natural gas
drilling operations. The field in which this facility is to be located is known as "Trail Ridge" and has
been in "operation" by Williams for 5 years. Williams plans on constructing 1,500 wells in this
region over the next 20 years.
This site is very remote as it is a 2' /2 hour car drive from the nearest housing and is intended to be
contained (except for the ISDS) on an existing COGCC approved well pad along with other drilling
facilities. The Temporary Employee Housing will be in use from November to May and will be
managed by PTI/Camp Crown t

Housing Structures
The Applicant is proposing a 6 -unit
side-by-side (total of 72' X 54') which
will include a kitchen, dining room
and recreation room. The plans
anticipate a total of 13 rooms that can
sleep up to two people each per the
2003 IBC. The facility will have three,
4000 gallon potable water storage
tanks; three, 70 kW generators; and an
ISDS system for sewage.

Water: The Applicant is proposing to haul water to the site using a water hauling contractor. The
potable water is to be supplied by GTM, Ltd (does not have a potable water haulers license number)
who will get potable water from the Meeker City Water Department. Thus, all water will be treated
prior to delivery at the site.
The site will be equipped with three, 4000 gallon potable water storage tanks. The facility is
designed for 80 gallons of potable water use per day which includes dining, food preparation,
laundry, sewage, sanitation and a contingency. At this rate, the facility is anticipated to use
approximately 1,920 gallons of water per day. This will provide a 6.25 day supply of potable water.
Sewage: The site is to be served by one ISDS system which is to be located just off the approved
COGCC well pad. The ISDS has been engineered by Cronk Construction Incorporated and
signed/sealed by Thomas A. Cronk P.E. The proposed system is designed to handle the estimated
1,800 gal of sewage and wastewater/day.
Refuse: The facility is to be served with a 6 cubic yard, bear -proof dumpster.
Density: Since this facility is not entirely contained within the COGCC approved well pad, per
5.02.21 (2)(D), this site must maintain at Least a one mile spacing from all other Temporary
Employee Housing SUP sites. As this is the first Temporary Employee Housing site to be reviewed
under the recently adopted 5.02.21 regulations, there are no other approved housing sites within one
mile of the proposed site.
Proximity to Permanent Available Housing: As this site is over 55 miles from the City of Rifle, the
nearest municipality by car, staff has concluded that this is a very remote site which is not proximate
to any permanent housing.
Renewal: The Applicant is proposing to renew this annual permit twice (2007 and 2008) to allow for
two full winter seasons at the site.
Reclamation and Revegetation: All housing structures and associated equipment will be located on
the COGCC approved well pad except for the ISDS. Since the ISDS is not located on the well pad, it
is not covered by the COGCC reclamation standards and is required to meet the standards set for in
Section 5.02.21 (11). A "Site Rehabilitation Plan" has been submitted (See Exhibit J). Since the area
disturbed is less than 1/2 of one acre, staff is not recommending that security be collected for this
revegetation plan.

REVIEW CRITERIA FOR SPECIAL USE PERMITS (SECTION 5:03)
Pursuant to Section 5.03, as listed under the Zone District Regulations, special uses shall conform to
all requirements listed there under and elsewhere in the Zoning Resolution, as well as the following
standards:
1. Utilities adequate to provide water and sanitation service based on accepted engineering
standards and approved by the Board of Counry Commissioners shall either be in place or shall be
constructed in conjunction with the proposed use.
Response
Staff feels that provisions made for water hauling and sewage disposal as described above and in
combination with the staff recommended conditions, this standard is satisfied.
2. Street improvements adequate to accommodate traffic volume generated by the proposed
use and to provide safe, convenient access to the use shall either be in place or shall be
constructed in conjunction with the proposed use.
Response
Overall, the approval of this Special Use Permit for Temporary Employee Housing will reduce traffic
on the access roads to the site. For this reason, staff feels that additional road improvements are not
necessary.
3. Design of the proposed use is organized to minimize impact on and from adjacent uses of
land through installation of screen fences or landscape materials on the periphery of the lot and
by location of intensively utilized areas, access points, lighting and signs in such a manner as to
protect established neighborhood character.
Response
As this site is very remote, located primarily on an already approved COGCC drill pad and will
ultimately reduce the amount of traffic to and from the site, it is staff's opinion that this Temporary
Employee Housing facility will not adversely effect any nearby neighbors as proposed.

Referral to Planning Commission
As an oversight from the recent staff turnover in the department, this application was inadvertently
scheduled for a public hearing before the BOCC without first getting the Board's direction on
referring the application to the Planning Commission. At the Board's discretion, this application may
still be referred to the Planning Commission for their recommendation. However, staff feels that
given the recent text amendment to Section 5.02.21 "Temporary Employee Housing", the Planning
Commission has spent significant time reviewing the standards and is familiar with the issues. In
addition, staff feels this housing application is in the overall best interest of the County. To this end,
staff recommends that the Board move forward with a motion on this item during the scheduled
public hearing.
III. SITE DESCRIPTION
The proposed site is to be located on a 17,013 acre parcel owned by Chevron USA Inc. The primary
use of this land is currently grazing and natural gas extraction operations. The site is very remote as it
is 2 '/2 hours (55.5 miles) by car from Rifle and approximately 15 miles "as the crow flies" North
West from Parachute. There are no roads which connect this site to the Town of Parachute, which
makes the City of Rifle the nearest municipality.
IV. ZONING & ADJACENT USES
The subject property is zoned Resource Lands - Plateau (RL). The type of use requested falls under
the definition of"Temporary Employee Housing" which are contemplated as special uses in the RL
zone district. As this site is very remote and the parcel is large (17,013 acres), the only adjacent use
is grazing at this time. (See above Zoning Map on Page 5)

V. AUTHORITY &
APPLICABILITY
Pursuant to Section 9.03.04 of the Zoning
Resolution, an application for a Special Use

Section 9.03.05 [Periodic Review of SUP!
Pursuant to section 9.03.05 of the Zoning Resolution:
Any Special Use Permits may be made subject to a periodic review not less than every six (6)
months if required by the County Commissioners. The purpose of such review shall be to
determine compliance or noncompliance with any performance requirements associated with the
granting of the Special Use Permit The County Commissioners shall indicate that such a review
is required and shall establish the time periods at the time of issuance of a Special Use Permit
Such review shall be conducted in such manner and by such persons as the County
Commissioners deem appropriate to make the review effective and meaningful. Upon the
completion of each review, the Commissioners may determine that the permit operations are in
compliance and continue the permit, or determine the operations are not in compliance and either
suspend the permit or require the permittee to bring the operation into compliance by a certain
specified date. Such periodic review shall be limited to those performance requirements and
conditions imposed at the time of the original issuance of the Special Use Permit.

VII. RECOMMENDED FINDINGS
1. Proper posting and public notice was provided as required for the meeting before the Board of
County Commissioners.
2. The meeting before the Board of County Commissioners was extensive and complete, that all
pertinent facts, matters and issues were submitted and that all interested parties were heard at that
meeting.
3. The above stated and other reasons, the proposed special use permit has been determined to be in
the best interest of the health, safety, morals, convenience, order, prosperity and welfare of the
citizens of Garfield County.
4. The application has met the requirements of Special Use (Sections 5:03, 5:03:07, 5:03.08 and
9:03) the Garfield County Zoning Resolution of 1978, as amended.
VIII. STAFF RECOMMENDATION
Due to 1) the limited nature of potential impacts to surrounding properties, 2) by proceeding with
this proposal there will be an overall decrease in traffic on unimproved roadways and county roads
which will promote safety and wellbeing, 3) the fact that the site itself will be situated on an existing
well pad with an active gas operation, 4) and that the Temporary Employee Housing facility meets
the requirements set forth in Section 5.02.21, Staff recommends the Board approve the request for a
Special Use Permit for Temporary Employee Housing on in the Resource Lands —Plateau (RL) zone
district with the following conditions:
1. That all representations of the Applicant, either within the application or stated at the
hearing before the Board of County Commissioners, shall be considered conditions of
approval unless explicitly altered by the Board.
2. That the operation of the facility be done in accordance with all applicable federal, state, and
local regulations governing the operation of this type of facility.
3. Emissions of smoke and particulate matter: every use shall be operated so as to comply
with all Federal, State and County air quality laws, regulations and standards.
4. Emission of heat, glare, radiation and fumes: every use shall be so operated that it does not
emit heat, glare, radiation or fumes which substantially interfere with the existing use of
adjoining property or which constitutes a public nuisance or hazard. Flaring of gases,
aircraft warning signals, reflective painting of storage tanks, or other such operations

which may be required by law as safety or air pollution control measures shall be exempted
from this provision.
5. Water pollution: in a case in which potential hazards exist, it shall be necessary to install
safeguards designed to comply with the Regulations of the Environmental Protection
Agency before operation of the facilities may begin. All percolation tests or ground water
resource tests as may be required by local or State Health Officers must be met before
operation of the facilities may begin.
6. If applicable, the Applicant shall provide a copy of the appropriate bond supplied to the
Colorado Oil and Gas Conservation Commission (COGCC) intended to guarantee
reclamation of the Temporary Employee Housing site prior to the issuance of a Special Use
Permit. If the COGCC does not require reclamation and revegetation bonding, the applicant
shall work with County Staff to establish a bond appropriate to guarantee reclamation and
revegetation of the Temporary Employee Housing facility.
7. All Special Use Permits for Temporary Employee Housing is subject to all applicable
building code, state and federal permit requirements, fire protection district requirements
and fire code requirements.
8. Water and wastewater systems proposed to service temporary employee housing must
comply with all applicable state and local laws and regulations.
9. Applicants must keep appropriate records, to be provided to the County upon request to
demonstrate that water supplied to a site is from an approved source and that wastewater
is disposed at an approved facility. For facilities serving twenty-four (24) people or less,
the operator must conduct monthly tests (or quarterly if an on-site disinfection system is
installed) and maintain records of stored potable water samples specific for coli form.
Any tests indicating coli form contamination must be disclosed to the Garfield County
Board of Health or designee.
10. In no case shall unsafe water be used for drinking nor shall raw sewage or used water be
discharged on the ground surface.
11. The maximum allowable time length of the Special Use Permit for Temporary Employee
Housing is one (1) year. For good cause shown, the permit may be renewed annually in a
public meeting with notice by agenda only. Annual renewal review shall be based on the
standards herein as well as all conditions of the permit. A permit may be revoked anytime
through a public hearing called up by staff or the Board of County Commissioners.

12. Inhabitants of the temporary housing shall be applicant's employees and/or
subcontractors, working on the related construction or mineral extraction operation, and
not dependents of employees, guests or other family members.
13. Temporary employee housing sites shall be maintained in a clean, safe and sanitary
condition, free of weeds and refuse. Any hazardous or noxious materials that must be
stored on site for operational or security reasons must be managed in accordance with all
applicable federal, state and local laws and regulations.
14. Fire Protection General Requirements: Provisions shall be made for giving alarm in case
of fire. It shall be the responsibility of the duly authorized attendant or caretaker to
inform all employees about means for summoning fire apparatus, sheriffs office and
resident employees. All fires are subject to §307 of the 2003 International Fire Code
(IFC) including but not limited to permits, attendance, open fires, coal grills, fire bans and
bon fires. One (1) or more approved extinguishers of a type suitable for flammable liquid
or electrical fires (Class A, Class B and Class C), carbon dioxide or dry chemical, shall be
located in an open station so that it will not be necessary to travel more than one hundred
(100) feet to reach the nearest extinguisher.
15. Outdoor food storage is prohibited unless facilities that prevent the attraction of animals
to the temporary employee housing site are provided.
16. The applicant shall provide a detailed map and GPS coordinates to the Garfield County
Sheriffs Office and the relevant Fire Protection District which is sufficient for emergency
response purposes, including location of the temporary employee housing site; private
and public roadways accessing the site, marked as open, gated and/or locked; and detailed
directions to the site from a major public right-of-way. The map is subject to approval by
the Garfield County Sheriffs Office and relevant Fire Protection District.
17. If a Special Use Permit for Temporary Employee Housing is granted, the applicant shall
notify the County when site development begins. The applicant shall verify in writing, by
site plan and through photo documentation that the site, water system, and sewage
disposal system were designed, installed and inspected in accordance with the said special
use permit and comply with all applicable regulations, permits, and conditions. All
written documentation and site plans verifying compliance must be stamped by a certified
Colorado Engineer. The County also reserves the right to inspect a site, without notice, to
assess compliance with the Special Use Permit for Temporary Employee Housing. A
determination of noncompliance with any Special Use Permit for Temporary Employee
Housing, or condition approval thereof, is grounds for revocation or suspension of said
permit, in accordance with Section §9.01.06.

Exhibit A
5.02.21 Special Use Permits for Temporary Employee Housing:
(1) At times of severe housing shortage, extremely remote locations or other
emergency conditions, special use permits for temporary employee housing in
the nature of manufactured homes [as defined under C.R.S. 42-1-102 (106) (b)]
and/or recreational vehicles [as defined under C.R.S. 42-1-102 (61), with the
addition that such truck, truck tractor, motor home or camper trailer is being used
for temporary living quarters and not recreational purposes] may be granted for
projects within Garfield County related to commercial, industrial, mineral
extraction or highway operations of substantial size in any zone district by the
County Commissioners through the special use permit process. Such housing
shall be of a temporary nature. At the expiration of the permit, the lands shall be
restored and all housing structures and associated infrastructure shall be
removed. Review of the permit shall be subject to §9.03 and §5.03 of the Garfield
County Zoning Regulations of 1978 as Amended. All Special Use Permits for
Temporary Employee Housing is subject to all applicable building code, state and
federal permit requirements, fire protection district requirements and fire code
requirements.
(2) Special Use Permits for Temporary Housing: The applicant shall submit an
adequate site plan, consistent with §9.01.01 and including proposed water
supply, proposed method of sewage treatment and names and addresses of
adjacent property owners.
(A) Water and wastewater systems proposed to service temporary employee
housing must comply with all applicable state and local laws and regulations.
In addition, all sewage must be disposed of on-site using an Individual
Sewage Disposal System (ISDS) unless the applicant can prove:
1. That, at the discretion of the Garfield County Board of Health, an
ISDS system is not feasible due to environmental, topographic or
engineering conditions where the temporary housing is to be located;
or
2. That, at the discretion of the Garfield County Board of Health, year-
round access is available and maintained for safe and regular access
for sewage hauling vehicles. In addition, the following conditions must
be met:
a. The applicant must demonstrate and guarantee an
arrangement for hauling sewage; and
b. The applicant must maintain all records including but not
limited to trip logs/reports and landfill receipts; and
c. All sewage disposal records must be maintained as public
records to be available to the County and/or any other
interested third party upon request; and
d. The temporary housing must not exceed a cumulative of one
(1) year at an approved location.

(B) For sites where potable water is hauled to and wastewater is hauled out,
applicants must keep appropriate records, to be provided to the County upon
request to demonstrate that water supplied to a site is from an approved
source and that wastewater is disposed at an approved facility. For facilities
serving twenty-four (24) people or less, the operator must conduct monthly
tests (or quarterly if an on-site disinfection system is installed) and maintain
records of stored potable water samples specific for coli form. Any tests
indicating coli form contamination must be disclosed to the Garfield County
Board of Health or designee. Water systems serving twenty-five (25) people
or more must demonstrate conformance to state regulations by obtaining all
necessary state permits prior to the scheduling of a Special Use Permit for
Temporary Employee Housing public hearing.
(C) In no case shall unsafe water be used for drinking nor shall raw sewage or
used water be discharged on the ground surface.
(D) Special Use Permits for Temporary Employee Housing sites must be related
to one or more commercial, industrial, mineral extraction or highway
operation locations and shall be limited to a spacing of at least one (1) mile
between temporary employee housing sites, regardless of land ownership or
operator. Special Use Permits for Temporary Employee Housing for oil and
gas extraction purposes in the Resource Lands (RL) zoning district may be
exempted by the BOCC from the one mile spacing if the operator can prove
that the housing structures and all supporting infrastructure will be contained
within the Colorado Oil and Gas Commission (COGCC) approved well pad
and there will be no new additional land disturbance outside of the COGCC
approved well pad area. If the applicant is applying for a Special Use Permit
for Temporary Employee Housing on an approved COGCC well pad, the
applicant must provide the relevant approved APD permit indicating housing
location(s) along with the application for a Special Use Permit for Temporary
Employee Housing.
(3) The maximum allowable time length of the Special Use Permit for Temporary
Employee Housing is one (1) year. For good cause shown, the permit may
renewed annually in a public meeting with notice by agenda only. Annual renewal
review shall be based on the standards herein as well as all conditions of the
permit. A permit may be revoked anytime through a public hearing called up by
staff or the Board of County Commissioners. By way of example and not
limitation, continued non-availability of a permanent housing inventory or the
nature of the construction or extraction project may constitute "good cause" for
renewal. The applicant must provide an estimated total cumulative length of time
the temporary employee housing will be at the proposed location along with a
statement of intentions to request renewal past the one year expiration date as
part of the Special Use Permit for Temporary Employee Housing application.
Failure to provide a statement of intention for renewal will prohibit the Special
Use Permit for Temporary Employee Housing permit from future renewal
consideration.
(4) Temporary housing shall be located at a site authorized by the Board of County
Commissioners and identified on the relevant site plan submitted with the Special
Use Permit for Temporary Employee Housing. Inhabitants of the temporary

(A) Fire Protection General Requirements: Provisions shall be made for giving

water storage tank may be required if County and local fire protection officials
deem it necessary.
(B) Bear -proof refuse containers must be provided for trash. At least one thirty
(30) gallon (4 cubic feet) container shall be provided for each unit or the
equivalent in a central trash collection facility. Said container(s) must be
durable, washable, non-absorbent metal or plastic with tight -fitting lids.
Refuse shall be disposed of not less than once weekly.
(C) Outdoor food storage is prohibited unless facilities that prevent the attraction
of animals to the temporary employee housing site are provided.
(D) The applicant shall provide a detailed map and GPS coordinates to the
Garfield County Sheriff's Office and the relevant Fire Protection District which
is sufficient for emergency response purposes, including location of the
temporary employee housing site; private and public roadways accessing the
site, marked as open, gated and/or locked; and detailed directions to the site
from a major public right-of-way. The map is subject to approval by the
Garfield County Sheriffs Office and relevant Fire Protection District.
(6) If structures, requiring Building Permits under the Garfield County Building Code,
are constructed for the commercial, industrial, highway project or mineral
extraction operation related to the Special Use Permit for Temporary Employee
Housing, upon expiration or revocation of the permit Certificates of Occupancy
for such structures shall be withheld until the temporary living quarters are
removed and the site is restored to the satisfaction of the County Building and
Planning Director.
(7) If a Special Use Permit for Temporary Employee Housing is granted, the
applicant shall notify the county when site development begins. The applicant
shall verify in writing, by site plan and through photo documentation that the site,

water system, and sewage disposal system were designed, installed and
inspected in accordance with the said special use permit and comply with all
applicable regulations, permits, and conditions. All written documentation and
site plans verifying compliance must be stamped by a certified Colorado
Engineer. The county also reserves the right to inspect a site, without notice, to
assess compliance with the Special Use Permit for Temporary Employee
Housing. A determination of noncompliance with any Special Use Permit for
Temporary Employee Housing, or condition approval thereof, is grounds for
revocation or suspension of said permit, in accordance with Section §9.01.06.
(8) If there is suitable permanent housing inventory available in an area near the
commercial, industrial, highway project or mineral extraction operation, as
determined at the discretion of the BOCC, the Special Use Permit for Temporary
Employee Housing shall not be granted.
(9) No animals shall be allowed at temporary employee housing sites.
(10) In evaluating a request fora Special Use Permit for Temporary Employee
Housing, the County Commissioners may require compliance with additional
conditions of approval as may be needed to ensure the health, safety and
welfare of the public. (A.81-263)
(11)The applicant shall submit as part of the Special Use Permit for Temporary
Employee Housing, a reclamation and revegetation plan for each specific site
addressing all points in section eleven (11) within this §5.02.21.
a. Debris and waste materials, including, but not limited to structures, concrete,
footings, sewage disposal systems and related infrastructure, water storage
and related distribution infrastructure, roads, and other sand, plastic, gravel,
pipe and cable shall be removed. All pits, cellars, and other holes will be
backfilled as soon as possible after all equipment is removed to conform to
surrounding terrain. All access roads to the site and associated facilities shall
be closed, graded and recontoured. Culverts and any other obstructions that
were part of the access road(s) shall be removed. Upon closure of a camp
facility, wastewater tanks shall be completely pumped out and either crushed
in place, punctured and filled with inert material or removed. Any waste
material pumped from a wastewater tank or waste debris from tank removal
must be disposed of at an approved facility that is permitted by CDPHE and/or
Garfield County to receive said wastes. Materials may not be bumed or buried
(other than ISDS) on the premises. All disturbed areas affected by temporary
employee housing or subsequent operations shall be reclaimed as early and
as nearly as practicable to their original condition and shall be maintained to
control dust, weeds and minimize erosion. As to crop lands, if subsidence
occurs in such areas additional topsoil shall be added to the depression and
the land shall be re -leveled as close to its original contour as practicable.
Reclamation shall occur no later than three (3) months after the Special Use
Permit for Temporary Employee Housing expires or is revoked unless the
Director or designee extends the time period because of conditions outside
the control of the applicant.
b. All areas compacted by temporary employee housing and subsequent

operations shall be cross -ripped. On crop land, such compaction alleviation
operations shall be undertaken when the soil moisture at the time of ripping is
below thirty-five percent (35%) of field capacity. Ripping shall be undertaken to
a depth of eighteen (18) inches unless and to the extent bed rock is
encountered at a shallower depth.
c. When a temporary employee housing site is removed, all disturbed areas will
be restored and revegetated as soon as practicable. For disturbed areas not
regulated by the Colorado Oil and Gas Conservation Commission, the following
regulations will apply:
(1) Revegetation of crop lands. All segregated soil horizons removed from
crop lands shall be replaced to their original relative positions and
contour, and shall be tilled adequately to re-establish a proper seedbed.
The area shall be treated if necessary and practicable to prevent invasion
of undesirable species and noxious weeds, and to control erosion. Any
perennial forage crops that were present before disturbance shall be
reestablished.
(2) Revegetation of non -crop lands. All segregated soil horizons removed
from non -crop lands shall be replaced to their original relative positions
and contour as near as practicable to achieve erosion control and long-
term stability, and shall be tilled adequately in order to establish a proper
seedbed. The disturbed area then shall be reseeded in the first favorable
season. Reseeding with species consistent with the adjacent plant
community is encouraged. In the absence of an agreement between the
applicant and the affected surface owner as to what seed mix should be
used, the applicant shall consult with a representative of the local soil
conservation district to determine the proper seed mix to use in
revegetating the disturbed area.
d. During occupation and reclamation operations, all disturbed areas shall be
kept free of Garfield County and State of Colorado List A and B noxious
weeds.
e. Successful reclamation of the site and access road will be considered
completed when:
(1) On crop land, reclamation has been performed as per 11(c)(1) of this
section, and observation by the Director or designee over two growing
seasons has indicated no significant unrestored subsidence.
(2) On non -crop land, reclamation has been performed as per 11(c)(2) of this
section, and the total cover of live perennial vegetation, excluding noxious
weeds, provides sufficient soils erosion control as determined by the
Director through a visual appraisal. The Director or designee shall
consider the total cover of live perennial vegetation of adjacent or nearby
undisturbed land, not including overstory or tree canopy cover, having
similar soils, slope and aspect of the reclaimed area.
(3) A final reclamation inspection has been completed by the Director or
Garfield County, Board of County Commissioners

designee, there are no outstanding compliance issues relating to Garfield
County rules, regulations, orders or permit conditions, and the Director or
designee has notified the applicant that final reclamation has been
approved.
f. Specifically as to revegetation, the applicant shall provide security for
revegetation of disturbed areas in amount and in accordance with a plan
approved by the Garfield County Vegetation Management Department. The
security shall be held by Garfield County until vegetation has been successfully
reestablished according to the standards in the Garfield County Vegetation
Management Plan adopted by resolution No. 2002-94, as amended.

SITE REHABILITATION PLAN
Williams Production RMT Co.- Temporary Living and Storage Facility
Section 3, Township 5S, Range 97W, 6th P.M.
The Williams Production RMT Co.- Temporary Living and Storage Facility will be located on rangeland owned by Chevron
USA Inc. The temporary living and storage facility will be placed entirely upon a Colorado Oil and Gas Conservation
Commission regulated natural gas well pad.
The exception to COGCC pre-eminence would be the installation of an absorption field for the individual sewage disposal
system. This system is approximately 100'x100' in size and is not located on the natural gas well pad and thus would be
subject to the rehabilitation regulations set forth by Garfield County. Please see tab 3b Wastewater for a full engineered
design and Garfield County ISDS Permit for this site.
The subject site appears to lie in the Parachute-Rhone-Irigul soil series. The series consists of shallow, well -drained soils.
The topsoil should be stripped to a depth of approximately twelve (12) inches and stockpiled at the ISDS margins. This
topsoil material will then be mounded 5% over the absorption field to promote surface runoff.
RECLAMATION
The objectives of ISDS absorption field area reclamation will be as follows:
1. Stabilization of the disturbed areas will be conducted by providing wind and water erosion control to reduce soil
loss and to promote evapotranspiration over the absorption field .
2. Utilize the prescribed seed mixtures and additional vegetation practices as described below to establish a self-
sustaining vegetative rangeland cover for cattle pasture use.
ISDS ABANDONMENT
The Individual Sewage Disposal system will be abandoned as per the following protocol detailed in the Garfield County
Zoning Resolution of 1978 Section 5.02.21 (11 a):
1. Waste materials will be pumped from the septic tank and will be disposed of at an approved facility that is
permitted by the CDPHE and/or Garfield County.
2. The septic tank will be filled with inert material.
3. Underground sewer piping and the absorption field will remain in place, undisturbed.

SEEDBED PREPARATION/SOIL TILLAGE
Seedbed preparation and soil tillage will be completed after the application of subsoil, topsoil, and any soil amendments.
Soil tillage will be to a minimum depth of 4" utilizing a disk, chisel plow, or harrow. Seedbed preparation will also include
removal of coarse fragments (rock material) that exceed 35% to 40% of the soil surface as well as rocks 8" in diameter
that occupy more than 10% of the soil surface.
SEEDING METHODS AND TIMES
The area disturbed by construction of the ISDS absorption field will be seeded. If seeding is performed in the spring, it will
be accomplished by May 15; if fall seeding is performed it will be completed after August 30 and before the soil freezes.
Seeding efforts will consist of drill seeding with a rangeland drill to a planting depth of to 12" on slopes 3:1 or flatter.
Broadcast seeding followed by harrowing or hand raking to lightly cover the seed with soil will be used on slopes steeper
than 3:1, or on areas inaccessible for drill seeding equipment.
All areas to be reclaimed will be mulched no later than 24 hours after seeding with a certified weed -free straw or grass
hay material. Grass hay mulch will be applied at 1 Y2 tons per acre, or straw mulch will be applied at 2 tons per acre.
Mulch material will be crimped into the soil surface with a commercial mulch crimper, a straight disc, or bulldozer tracks if
too steep to otherwise crimp mulch in place.

Please see Table 1 for the seed mixtures.
WEED CONTROL PLAN
A qualified person will inspect the ISDS absorption area and will inventory and map the area for any listed Garfield County
noxious weeds. Based on this inventory and mapping, methods, materials, and timing of weed control measures will be
specified.

Hi Dave
I looked at the attachments re: revegetation for the off site ISDS. Generally we don't ask for a reveg security if
the disturbed area is less than half of an acre. In this case it looks like Phil estimates it at 100 ft square -which is
10,000 sq. ft or a little less than a quarter of an acre. So my recommendation would be to accept the submitted
revegetation plan and not require a reveg security.

1.0 Site History
The site consists of approximately 18.51 acres of uncultivated native sod. Drainage is approximately 7'S„
to the northwest. A percolation testrsods evaluation was conducted on the property of reference on
09.19'06 by 1 om A. Cronk, registered professional engineer (R.P.E.). The perr excavation trench
(excavation A) was located approximately 410' southwest of the first well head located on the parcel and
approximately 25' northwest of the access road Additional pelt holes were located 45' northwest of the
access road (excavation B) and 70' northwest of the access road (excavation C)
The 09/19/06 investigation was intended to develop subsurface design parameters for an Inch n tdual
sewage disposal system (1SDS) to serve a proposed commercial man camp at the site. Because of the
commercial nature of the project. an engineered ISOS design has been prepared. The 0919/06 pert
test sods evaluation is attached for reference as Appendix A. A discussion of developed subsurface
design parameters and the engineered ISRS design follows.
2.0 De‘clopment of Design Parameters
A per( excavation trench (excavation A) was extended to a depth of96" below ground surface (BGS).
there was no evidence of ground water or high seasonal water table in the open excavation to a depth of
96" BGS . he soils evaluation indicates three distinct soil horizons underlie the sue. A lithological
description follows.

INFILTRATOR absorption trenches arc proposed for discharge of septic effluent at the site A lift station
discharging into pressurized effluent distribution laterals will be employed to discharge septic effluent
into the absorption field. Construction of the system will consist of excavating level trenches in the area
comprising the absorption field. As shown in the attached septic layout plan, the trenches will be
oriented perpendicular (i.e., long axis northeast to southwest) to the natural slope (northwest) to
minimize the depth of excavation along the trench length. Each trench excavation shall he continued to a
maximum depth of 48" BGS at the uphill side of the trench (see absorption field cross-section). After
removal of surface soils. the open excavations will he disked or scarified to promote subsurface
absorption.
Following completion of the initial push -outs. INFILTRATORS will be used to construct a septic effluent
distribution system in the open excavations. The INFILTRATOR effluent distribution system will be
installed in accordance with the "Infiltrator Technical Manual". available from Infiltrator Systems Inc.,
123 Elm Street, Smte 12, Old Saybrook, Connecticut 06475. Adjacent INFILTRATORS shall be fastened
with a minimum of four (4) 3/4" x sf8 self -tapping sheet metal or bright brass deck screws to prevent
movement and separation during backfilling. As shown in the attached Absorption Field Cross Section.
the bottom of the INFILTRATOR seepage trenches will be located a maximum of 48" below ground level.
The installation will provide in excess of48" of usable infiltration soil between the bottom of each
INFILTRATOR trench and the adjoining subsoils.
After the INFILTRATORS are installed, the absorption Geld will he covered with a soil cap. 'The soil cap
will consist of approximately 36" of native soil. A minimum of 5% slope will he maintained to promote
surface run off away from the absorption field.
Septic effluent will be Introduced Into the INFILTRATOR absorption trenches through pressurized
distribution laterals installed in each row of INFILTRATORS as shown in the attached graphics. The
injection laterals will consist of 1" Schd 40 PVC pipe with 3/16" (0.1875") holes equally spaced 72" o.c.
along each row of INFILTRATORS. Holes in the injection laterals will be drilled at 12:00 o'clock to
direct efllueni up for dispersal against the top of the chamber. Injection laterals shall be suspended from
the top of the chamber with all weather plastic pipe strapping (mm. 120 lbs tensile strength) at every
chamherjoint connection. Septic effluent will be delivered to the distribution laterals through two 2"
(Schd 40 PVC) pressure manifold. Each pressure manifold will be centrally located in the absorption
field and serve ii of the INFILTRATORS (see attached absorption Geld plan view),
A lift station will be utilized to deliver septic effluent to the absorption field and promote equal
distribution of septic effluent. The lift station will employ a duple\ OSI high head effluent pump (OS1
model no P500512) and screened pumping vault (OSi model no. PVU57-2419-L) and shall he
constructed in accordance with the attached graphics.
Two 2" Schd 40 PVC pressure transport lines will be used to transfer septic effluent from the duplex
pumping system to the absorption field (one line to each pump). The pressure mains will be self draining
or be buried a minimum of 36" below ground surface to prevent freezing. All pressure piping and
absorption bed construction shall be completed in accordance with the attached graphics.
The installer must also confirm the setbacks from property lines, building envelopes, and existing
easements shown in the attached graphics are maintained. Additionally. any unknown utility lines.

easements, or other adverse conditions disclosed during construction must maintain the required setbacks
listed on page 6 of this document. A minimum of 12" (18" recommended) of soil cover is required for all
gravity flow effluent piping, the septic tank lift station, and the absorption field (excepting maximum 8"
soil cover over septic access ports). All non -draining pressure effluent piping must be covered with a
minimum of 36" of cover to prevent freezing. Additionally, all effluent lines under vehicle traffic areas
shall be minimum Schedule 40 and shall be covered with flow fill and high density 1-1/2" blue board tf
less than 12" of cover Finally. 4" clean outs shall be provided at maximum 100' intervals in all effluent
lines exceeding 100' in length.
The ISDS will be designed to accept sewage discharge from a twenty four (24) person man camp. The
man camp will provide sleeping, eating and laundry facilities for the occupants. Sewage loading
estimates follow:

A minimum 3,375 gallon septic tank is required to provide 30 hours of retention capacity for sewage
generated at the above estimated rate. To obtain the required retention capacity, a 2,000 gallon single
compartment septic tank (i.e.. interior baffle removed) will be placed in series and ahead of a 1.500
gallon single compartment septic tank. Finally, a 1,500 gallon standard two compartment septic iank wdl
accept sewage from rhe office trailers and crew changing facility (see attached septic layout plan).
The second 1.500 gallon septic tank placed in series behind the 2.000 gallon tank will he used as a
pumping vault for the OSI pump and screened pumping vault. The effluent pumping system will consist
of a P500512 OSI duplex pumping system with an associated Biotube pumping vault (OSI Model
PVU57-2419-L) as shown in the attached effluent pump system detail. All construction, dosing chamber
capacity, location of ISDS elements. and setbacks from property lines, building envelopes, and existing
easements shall be in accordance with the attached graphics.
As discussed above, an INFILTRATOR system is proposed to discharge septic effluent to the underlying
sub -soils. As shown in the attached graphics. the absorption field will consist of 216 QUICK4
INFILTRATOR units and shall be configured as shown in the attached graphics. The absorption field
will encompass a gross area of2,592 square feet with an effective area (9.87 sq. ft/INFILTRATOR plus
50% reduction factor) of 4,263.84 sq. ft.

4.0 Site Specific Installation/Operation Requirements
The owner and system contractor shall be aware and comply with the following installation and system
operation requirements.
4.1 Installation • Setbacks Notifications. and inspections
• All installation activities shall he conducted in accordance with current Colorado
Department of Health ISDS Recu/unions as well as any relevant Garfield County
regulations and requirements. [fat any time during construction subsurface site
conditions are encountered which differ from the designparameters developed in Section
2.0. construction activities will stop and the design engineer will be contacted to address
any necessary design modifications.
• Installation procedures including grade, location, setbacks, septic tank size. and
absorption field size shall conform with the attached graphic details. Construction
activities and system components will not encroach upon existing easements or utility
corridors. A minimum of 6 ft of undisturbed soil shall be maintained between individual
absorption elements and the septic tank andior adjacent absorption elements. Minimum
site specific setbacks for system components are:

• All gravity flow sewer lines and effluent distribution piping shall be 4 inches in diameter
and have glued )omts. INFILTRATORS shall be placed level in the absorption field. All
lines discharging sewage from the residence to the septic tank shall maintain fall of
between 1//8 rn. and 114 rn. per foot and shall employ sweep 90's or 2-45's at all turns.
Sewer lines from the dwelling lo the septic tank and at (east 6 ft from the septic tank
outlet must meet minimum standard ASTM -3034. Sewer lines under driveways shall
always meet minimum Schedule 40 PVC standards and shall be encased with corrugated
metal pipe or flow fill and minimum 1-1i2" high density blue board when covered with
less than 12" of sod.
• A minimum of 12" of soil cover (18" recommended) shall be maintained over all prat ity
draining ISDS components to prevent freezing of septic effluent (excepting maximum 8"
cover over septic access ports). Four in. sanitary clean -outs shall be installed at
maximum 100' intervals in all non -pressurized effluent lines exceeding 100' in length.

• The installer shall not place fill to the open excavation until inspected and approved by
the design engineer. Additionally. the final cover shall not be placed on sewer lines.
septic tank. or the absorption area until the system has been inspected and approved by
the design engineer. The installer shall provide 48 hour notice to the engineer for all
required inspections.
• The effluent distribution system shall be constructed in accordance with the "Infiltrator
Technical Manual" available from Infiltrator Systems Inc., 123 Elm Street. Suite 12. Old
Saybrook. Connecticut 06475.
• Pressure piping shall consist of two each 2 inpressure transport Tines, two each 2 in.
pressure manifolds. and 1 in. pressure laterals. All pressure piping shall be min. Schd 40
PVC and have glued joints.
• The septic effluent pumping system shall consist of OSI components (OSI effluent
duplex pump model no. P500512 and associated OSI model PVLI57-2419-L pump vault)
as detailed in the attached effluent pumping system detail. The effluent pump control
panel shall be Orenco model no. SIETM. Electrical wiring shall he continuous cable
with all connections made in a weather proof box exterior to the dosing chamber. Limit
switching shall consist of: 1) low level/pump off. 2) high level/pump on. and 3) high
level alarm'system failure The high level alarm shall be easily detectable by occupants
Emergency notification information (e.g., telephone numbers of owner and sen ice
personnel) shall be posted near the high level alarm.
• The effluent pump shall he configured with a minimum dose volume of 200 gal. Storage
capacity in excess of the high-level alarm shall he 6" above the pump -on float level.
• All access manholes on septic tanks and dosing chambers will be child proof and contain
appropriate warning labels if accessible to the public. Confined space entry precautions
should be observed by maintenance personnel.
• The surface cap shall be mounded 5% over the absorption field to promote surthce
runoff,
• The system contractor shall be aware of the potential for construction activities to reduce
soil permeabilities at the sne through compaction, smearing, and shearing The
following precautions and construction procedures should he employed during
installation to minimize disturbance to native soils:
Excavation should proceed only when the moisture content of the soil is below
the plastic limit. If a sample of sol forms a rope instead of crumbling when
rolled between the hands it is too wet and should be allowed to dry before
excavation continues.
it. Excavation and backfill equipment should work from the surface where at all
practical to avoid compaction of the soils at depth.
n. The bottom and stdewalls of the excavation should be left with a rough. open
surface. The appearance should be that of broken or ripped soil as opposed to a
sheared, smeared, or troweled surface. Any smoothed or smeared surfaces

should be removed with a toothed rake or shallow ripper taking care to ieniore
loose residues from the bottom of the excavation by hand i r necessary.
Care should be taken in placing fill materials in the excavation to avoid
damaging the exposed soil surfaces.
4.2 Operation - Maintenance and Inspections
• The owner shall install a structural barrier if necessary and take precautions to prevent
vehicular traffic, excessive surface watering. accidental flooding. or other activities to
the vicinity of the absorption field which may compact. saturate, or otherwise alter the
subsurface sod parameters used in designing the septic system.
• The owner will plant and maintain grass or other shallow rooted cover crop to pre\ cnt
erosion and promote evapotranspiration over the absorption field.
• The owner will inspect and maintain the required mounding and drainage away from the
absorption field to prevent saturation from precipitation and surface floc+s.
• To mitigate the generation of preferential flow channels which may compromise the
operation of the system, the owner will inspect and prevent intrusion of burrowing
animals and deep rooted plants into the absorption field.
• The effluent pump filter shall be inspected and cleaned as necessary every six (6)
months.
• The owner will conduct periodic maintenance of the septic system by removing
accumulated sludge from the septic rank every 3-4 years to prevent clogging of the
absorption field.

The site consists of approximately 18.51 acres of uncultivated native soil. Drainage is approximately 7"i
to the northwest A percolation test/soils evaluation was conducted on the property of reference on
09/19/06 by Toni A. Cronk, registered professional engineer (R.P.E.). The pert excavation trench
(excavation A) was located approximately 410' southwest of the first well head located on the parcel and
approximately 25' northwest of the access road. Additional pert holes were located 45' northwest of the
access road (e.xcanation B) and 70' northwestof the access road (excavation C).
A pert excavation trench (excavation A) was extended to a depth of 96" below ground surface (BGS).
There was no evidence of ground water or high seasonal water table in the open excavation to a depth of
96" BGS. The soils evaluation indicates three distinct soil horizons underlie the site. A htholouical

Pere holes were constructed in excavation A at the surface and al depths of approximately 40 in and 80
In. Additional pere holes were constructed in excavation Bat the surface and at depths of 36 In and 72
In.. and in excavation C al the surface and al depths of 32 in. and 66 in. The holes appeared to he well
saturated at the time of the test. Results of the percolation test are shown in Table I.

2.0 Conclusions and Recommendations
Soils al the site appear acceptable for implementation of an individual sewage disposal system (1SDS).
The following site-specific considerations should be observed during implementation of the ISDS:
• The bottom of the ISDS absorption field should be located no deeper than 48" DOS to
Maintain 4' of usable infiltration soil above the extent of'the soils evaluation excavation
located at 96" BGS. A design per( rate of 60 mmhn is recommended for overall system
sizing.
• Care should be taken during construction to avoid compaction and smearing of the
exposed Infiltrative surfaces and preserve the natural permeability of the native soils

disposal system on the properly of reference. The design parameters developed in this document are
representative of the site conditions disclosed at the specific time of the site investigation. Site
conditions are subject lo change from external events both manmade (irrigation or pond construction)
and naturally occurring (flooding or excessive precipitation). Cronk Construction Incorporated is not
responsible and accepts no liability for any variation in assumed design parameters caused by external
events

Dear Commission:
Williams Production RMT Co. respectfully requests the above referenced Special Use Permit be
extended for the period of one year on the basis of continued use of the facility during the
remainder of 2007 and 2008.
In light of current drillings plans proposed for the Trail Ridge project area, one drill rig will be
drilling 10 wells throughout the winter months (November 15, 2007 — March 31, 2008) from a
COGCC approved drill pad in Section 16: T5N-R97W. The intent is to leave the existing
approved Temporary Living & Storage Facility where it is currently located in Section 3: T5S-
R97W and transport workers/employees to and from the existing facility. The one year extension
will accommodate both drilling and completion operations for wells referenced above.
Leaving the facilities in their current location will allow Williams Production RMT Co. to minimize
overall surface disturbance, utilize existing transportation routes, utilize existing infrastructure,
and minimize winter traffic to and within remote areas.
All facilities including 24 person camp, catering, ISDS, potable water storage, and bear proof
trash containment are consistent with the above approved Special Use Permit.
All existing camp facilities meet or exceed CDH, CDPHE, Colorado Division of Fire Safety, and
2003 IBC including but not limited to snow load requirements and fire suppression systems.

In the review of the request to extend this Special Use Permit, it has become evident that
a building permit for this structure was never obtained. This is a violation of Sec 105 of
the 2003 IBC (International Building Code) enable by Colorado Revised Statutes 30-28-
124 & 124.5.
You are hereby given notice, pursuant to the above statutes and regulations, that you must
completely correct the violations or establish an acceptable compliance schedule with this
department, within thirty (30) days of the date this notice is received. You are
encouraged to communicate with this office at your earliest convenience to begin the
compliance process. Failure to initiate compliance within the allowed time or to request
an appeal will result, per Garfield County Code Enforcement Protocol, in this matter
being referred to the legal department. Building code violations are referred to the Board
of Review. Zoning code violations are referred to the Board of Adjustment, should you
request an appeal. Please be advised that violations of the above statutes is a
misdemeanor crime and can, upon conviction, be subject to a fine or not more that
$100.00 or imprisonment for not more than ten days, or both, and that each day that the
violation continues is deemed to be a separate offense.

December 4th, 2006: "A Resolution Concerned With The Approval Of A
Special Use Permit For Temporary Employee Housing For A Property
Owned By Chevron Inc. USA Located In Section 3, Township 5S, Range
97W, Garfield County."
Williams Production RMT was approved for a Temporary Employee Housing
facility on land owned by Chevron Inc. USA located 55.5 miles by road
(approximately 15 miles directly) North West of the City of Rifle and within the
Resource Lands (RL) — Plateau Zone District. This Special Use Permit (SUP) was
issued on December 4th, 2006 under Resolution 2006-115. Per Condition (11) of
Resolution 2006-115, "The maximum allowable time length of the Special Use
Permit for Temporary Employee Housing is one (1) year. For good cause shown, the
permit may be renewed annually in a public meeting with notice by agenda only.
Annual renewal review shall be based on the standards herein as well as all
conditions of the permit. A permit may be revoked anytime through a public hearing
called up by staff or the Board of County Commissioners." The one year renewal is
the purpose of the meeting with Williams Production RMT and the Board of
County Commissioners (BOCC) on November 19th, 2007.
Based on a review of this application, the following points are worth noting:
1. The Application was approved on December 4th, 2006 while the SUP was
signed by the Chairman of the BOCC on January 15th, 2007.
2. Individual Sewage Disposal Permit (ISDS) Number 4233 was issued on
October 10, 2006.

3. Staff understands that this facility has been in operation since the issuance
of the SUP on January 15`h, 2007.
4. Following discussions with Williams Production RMT representatives and
a review of County Building Permit records, it is evident that a Building
Permit for this facility has not been issued.
5. On November 2"d, 2007, Andy Schwaller, Garfield County Chief Building
Official issued a notice of violation to Williams Production RMT for
operating the above identified facility in violation of Section 105 of the
2003 IBC (Intemational Building Code) enabled by Colorado Revised
Statutes 30-28-124 & 124.5.
Since the Applicant has not obtained the appropriate Building Permit for the above
identified facility and has not submitted verification that the site was installed in
accordance with all applicable regulations, permits and conditions, the Applicant is
currently out of compliance with the following Conditions of Approval as identified
within Resolution 2006-115:
Condition #2 - That the operation of the facility be done in accordance with all
applicable federal, state, and local regulations governing the operation of this
type of facility.
Condition #7 - All Special Use Permits for Temporary Employee Housing is
subject to all applicable building code, state and federal permit requirements,
fire protection district requirements and fire code requirements.
Condition #17 - If a Special Use Permit for Temporary Employee Housing is
granted, the applicant shall notify the County when site development begins.
The applicant shall verify in writing, by site plan and through photo
documentation that the site, water system, and sewage disposal system were
designed, installed and inspected in accordance with the said special use
permit and comply with all applicable regulations, permits, and conditions.
All written documentation and site plans verifying compliance must be
stamped by a certified Colorado Engineer. The County also reserves the right
to inspect a site, without notice, to assess compliance with the Special Use
Permit for Temporary Employee Housing. A determination of noncompliance
with any Special Use Permit for Temporary Employee Housing, or condition
approval thereof, is grounds for revocation or suspension of said permit, in
accordance with Section §9.01.06.
To this end, as "annual renewal review shall be based on the standards herein as well
as all conditions of the permit", Staff has found that the Applicant is not in

conformance with the conditions of the SUP and is hence not eligible for renewal of
Resolution 2006-115.
Thus, Staff recommends that Resolution 2006-115 not be granted renewal and that
use of the facility be halted immediately.

(Chevron USA Inc.) as approved under Resolution 2006-115. In conformance with the conditions of
approval as set forth in Resolution 2006-115, this Special Use Permit is now set to expire on
January 15, 2010. If the Applicant wishes to extend this Special Use Permit beyond January 15,
2010, the Applicant must request a public meeting with the Board of County Commissioners on or
before this date to allow for the Board of County Commissioners to make that determination.
