Colorado Department of Public Health and Environment Hydrostatic Testing Operations Certification Number COG604116 issued on 2/10/12.
Hydrostatic
Best Management Practices Management Plan dated December 2011.
All water for hydrotesting will be drawn from the Colorado River and the White River in the State of Utah. This hydrotest
water can legally be used by Mid-America Pipeline Company, LLC as noted in the following attached documents:
2/29/12- Filing for Water in the State of Utah- Surface Water Appropriation
Application- Colorado River
2/29/12- Filing for Water in the State of Utah- Surface Water Appropriation Application- White River
All of the hydrostatic testing will be conducted as
per the attached plan and as per the conditions of the CDPHE permit.
