Pe r our discussion yesterday, I am providing this letter to accomplish two tasks . The first is to transm it
to you an Onsite Wastewater Treatment System Permit Appl ication for the 7 person or less Temporary
Housing vau lt and haul permit and the second is t o provide informatio n about the sma ll temporary
employee housing facilities pursuant to Section 7-707 of the Garfield County Land Use Code.
Cynosure Energy, LLC has contracted w i th Frontier Drilling on behalf of the pad owner, Coachman
Energy, to drill a well on Pad 21A and subsequently 8 wells on Pad 21B. Both pads are in close proximity
of each other on Jolley M esa in Section 21, T6S, R91W. The temporary hou sing for the well drilling is
being located on Pad 218. The land owner for the location of the sma ll temporary housing is the
Colo rado Department of Wild l ife. The fol lowing vicinity map lists the location of the small temporary
housing location.

The zone district by which the drilling and small temporary hous i ng resides is that of the PL (Publ ic
Lands) zone district. Cynosure/Coachman have entered into a surface use agreement with the Colorado
Parks and Wildlife for the activities on Pad 218 as well as the appl icable COGCC perm itting and BLM
permitting (as in the case of Pad 21A).
It is anticipated that the use will remain for the drilling of the total of the 9 wells to be drilled and the
anticipated time to drill will be approximately 100 days. The drill rig with t he ancillary facilities is in the
process of moving onto the site with the drilling operations anticipated to ensue within the next weeks'
timeframe. As noted previously, the timeframe anticipated to dri ll is in the 100 day time period which
will provide a removal by the end of the year.
The landowner contact name for the Colorado Park s and Wildlife is JT Romatzki of the CPW located i n
the Grand Junction, Colorado office at (970) 255-1600. A local contact would be Levi Atwater at (970)
985-5882 .
For Cynosure, the local contact for compliance and for contact relative to 24-hour emergencies is Kenny
Kuhn at (970) 274-0546.
We have observed and documented the installation of both the water and wastewater system and have
found the installation for the water system to be in compliance with the Garfield County Land Use Code
(GCLUC) Section 7-7058 and the wastewater system to be in compliance with the GCLUC Section 7-705C.
Both follow compliance with the 2011 IPC.

the vault and haul operations:

The driller, in addition to on site labor, has contracted with a roustabout crew respons i b le for
maintaining the temporary housing associated w ith the dri lling operations in a clean, safe and sanita ry
condition that is free of weeds and refuse . We do realize that any haza rdous or noxio us materia ls must
be stored in accordance with the applicable Federa l, State and Local laws and regulations. Wildlife-
proof refuse containers are being employed and no outdoor food storage is allowed . As well, no
domestic animals ar e allowed on site for the facil ities .
Frontier Drilling has employed policies and provisions for fire protection i n acco r dance to the standards
for the Grand River Fi re Protection District. As well, since all of the habitable units are
mobile/manufactured units, the single-station carbon monoxide alarms are in place as a standard fixtu re
with each un it.
