Dear Jankovsky: The referenced design submitted by Tom Zancanella, P.E., has been reviewed in this office in accordance with the Design Criteria Considered In The Review Of Wastewater Treatment Facilities and Site Application #3926 which was approved September 14, 1989 and has been found in conformance. The design is hereby approved and subject to the following condition(s): The facility owner's registered professional engineer must certify at the completion of construction that the treatment works was constructed according to plans, specifications, and significant amendments approved by the Division. This certification must be submitted to this office. "As Recorded" drawings clearly showing any major alterations to the approved plans shall be prepared and furnished to the owner and the Division at the completion of the work. This review does not relieve the owner from compliance with all local regulations prior to construction nor from the responsibility for proper engineering, construction, and operation of the facility. Compliance with the discharge permit requirements is the responsibility of the permittee/owner. The Water Quality Control Division's review and approval of the design does not relieve the permittee/owner from compliance responsibilities. Please retain this letter for your permanent records. Attached to this letter you will find a Customer Satisfaction Survey. We would greatly appreciate it if you would take a few moments to complete this survey and return it to us. The property is located at Fourmile Creek within Sections 32 and 33, Township 7 South, Range 89 West, Garfield County, Colorado. Based on a site inspection by Mr. Randy Snyder of this office on September 21, 2001, we have determined that your wetland boundary delineation is accurate. The plan referenced below is an accurate depiction of the limits of Federal jurisdiction under Section 404 of the Clean Water Act. This is a request for a proposed Wastewater Treatment Facility located at Sunlight Mountain Resort. This Wastewater Treatment Facility is being proposed for the need for Sunlight Mountain Resort to continue treatment of wastewater, as its existing lagoon plant has several deficiencies that could potentially cause the plant to fall out of compliance with its discharge permit requirements. Working with CDPHE, Sunlight Mountain Resort is submitting this site application as a beginning of its process to construct and operate a modern, mechanical plant that is capable of meeting discharge permit requirements, while continuing to service its existing users. Per your request, you will find enclosed seven (7) copies for your reference. I would also like for you to notify me regarding dates and times of any meetings that I should attend. It is part of the Site Application process that the Planning Commission and BOCC review and sign their approval on the appropriate page of the Site Application. Please have the BOCC sign on the line item #3, entitled "County." We thank you in advance for your immediate attention to this application so that we can submit it to CDPHE as soon as possible.
