Typed Name 1 D NSA' APPLICATION FOR SITE APPROVAL FOR CONSTRUCTION OF: A NEW DOMESTIC WASTEWATER TREATMENT PLANT ATTACHMENT TO SITE APPLICATION In accordance with C.R.S. 1981, 25-8-702 (2)(a), (b), and (c), and the "Regulations for Site Applications for Domestic Wastewater Treatment Works", the Water Quality Control Division must determine that each site location is consistent with the long range, comprehensive planning for the area in which it is to be located, that the plant on the proposed site will be managed to minimize the potential adverse impacts on water quality, and must encourage the consolidation of wastewater treatment works whenever feasible. In making this determination, the Division requires each applicant for a site approval for a domestic wastewater treatment works to supply an engineering report describing the project and showing the applicant's capabilities to manage and operate the facility over the life of the project to determine the potential adverse impacts on water quality. The report shall be considered the culmination of the planning process and as a minimum shall address the following: Service area definition including existing and projected population, site location, staging or phasing, flow/loading projections, and relationship to other water and wastewater treatment plants in the area. Proposed site location, evaluation of altemative sites, and evaluation of treatment alternatives. Proposed effluent limitations as developed in coordination with the Division. Analysis of existing facilities within the service area(s). Analysis of opportunities for consolidation of treatment works in accordance with the provisions of 22.3(1)(c), including those recommended in the water quality management plan, unless the approved water quality management plan recommends no consolidation. Evidence that the proposed site and facility operations will not be adversely effected by floodplain or other natural hazards. Where such hazards are identified at the selected site, the report shall describe means of mitigating the hazard. Evidence shall be presented in the form of a report, containing soils testing results and design recommendations and prepared by a Professional Geologist and a Geotechnical Engineer, or by a professional meeting the qualifications of both Professional Geologist and Geotechnical Engineer, with an appropriate level of experience investigating geologic hazards, stating that the site will support the proposed facility. Detailed description of selected alternatives including legal description of the site, treatment system description, design capacities, and operational staffmg needs. Legal arrangements showing control of site for the project life or showing the ability of the entity to acquire the site and use it for the project life. Approval by the Division of an application for site approval shall not be deemed to be a determination that the proposed treatment works is or is not necessary, that the proposed site is or is not the best or only site upon which to locate such a treatment works, or that location of a treatment works on the site is or is not a reasonable public use justifying condemnation of the site. Approval by the Division shall only be deemed to be a determination that the site application meets the requirement of this regulation 22 (5 CCR 1002-22). Institutional arrangements such as contract and/ or covenant terms which will be finalized to pay for acceptable waste treatment. Management capabilities for controlling the wastewater loadings within the capacity limitations of the proposed treatment works, i.e., user contracts, operating agreements, pretreatment requirements and or the management capabilities to expand the facilities as needed (subject to the appropriate, future review and approval procedures). Financial system, which has been developed to provide for necessary capital and continued operation, maintenance, and replacement through the life of the project. This would include, for example, anticipated fee structure. Implementation plan and schedule including estimated construction time and estimated start-up date. (CHURCH) has prepared this Addendum to the subject report. The Engineering Report for Site Approval, Proposed Wastewater Treatment Facility at the CDOT Bair Ranch Rest Area (Rest Area) was prepared under CHURCH Job No. 12090, dated December 15, 2002. This Addendum was prepared to provide additional information requested by CDPHE. Existing and Proposed Wastewater Treatment: Additional commentary regarding the justification for the proposed wastewater treatment facility (WTF) installation was requested. The existing waste handling devices at the Rest Area consist of composting toilets for the solid wastes, and a septic system to handle wastewater generated from sinks and the composting toilets. The composting toilets are located in the basement of the Rest Area restroom building. Due to the operating and maintenance demands of the composting toilets and anticipated increase in use of the Rest Area in the future, replacement of these toilets with standard flush toilets is preferred by CDOT. The flush toilets will be connected to the proposed WTF as outlined in the Site Application report. The composting toilets will be dismantled and removed from the Rest Area and the existing septic system absorption bed will be abandoned. A similar conversion from composting toilets to flush toilets and a similar WTF has already been completed at the Grizzly Creek Rest Area. Preliminary Effluent Limits (PELs): CHURCH received PELs prepared by Ms. Karen Young, Permits Unit, Water Quality Protection Section, WQCD at CDPHE, a letter dated January 29, 2002. A copy of the letter is provided with the Site Application Report. CHURCH was informed by Ms. Young, that the PELs for the proposed WTF presented in the letter are still applicable. Other items requested from CDPHE (Grand Junction): 1. Attached is a Revised Figure 6 that presents revised locations of the proposed WTF components and the minimum setback distance (100 feet) from occupied buildings at the Rest Area for the proposed WTF. The revised locations of the WTF components vary from those locations presented in the DENVER 4501 Wadsworth Boulevard Wheat Ridge, CO 80033 303.463.9317 Fax: 303.463.9321 original Site Application report. The new WTF component locations are in the vicinity of those locations originally proposed. The changes are a result of the design process. 2. Attached is a photograph of the public notice sign that is currently installed at the Rest Area regarding the proposed WTF. 3. This site approval will expire one year from the date of this letter if the construction of the project has not commenced by that date. If expiration occurs, you must apply for a new site approval. Construction is defined as entering into a contract for the erection or physical placement of materials, equipment, piping, earthwork, or building that are to be a part of a domestic wastewater treatment works. 3. The design (construction plans and specifications) for the treatment works must be approved by the Division prior to commencement of construction and all construction change orders initiating variances from the approved plans and specifications must be approved by the Division. 4. The applicant's registered engineer must furnish a statement prior to the commencement of operation stating that the facilities were constructed in conformance with approved plans, specifications, and change orders. 5. The applicant and applicanfs design engineer have submitted a variance request for a specific flow rate into the packed bed filter that has not been supported by the data submitted. The variance request is being considered and the appropriate flow rate will be determined at plans and specifications review. The design (construction plans and specifications) for the treatment works must be approved by the Division prior to commencement of construction and all construction change orders initiating variances from the approved plans and specifications must be approved by the Division. 4. The applicant's registered engineer must furnish a statement prior to the commencement of operation stating that the facilities were constructed in conformance with approved plans, specifications, and change orders. 5. The applicant and applicant's design engineer have submitted a variance request for a specific flow rate into the packed bed filter that has not been supported by the data submitted. The variance request is being considered and the appropriate flow rate will be determined at plans and specifications review. This site application is conditional on resolution of this issue. In accordance with Colorado Water Quality Control Commission regulations, this approval is subject to appeal as stated under Section 2.2.5 (7) of "Regulations for Site Applications for Domestic Wastewater Treatment Works". This approval does not relieve the owner from compliance with all county regulations prior to construction nor from responsibility for proper engineering, construction, and operation of the facility.
