(For New Rome Construction)
INSTRUCTIONS:
This Application & Checklist is to be used for all project submittals to the Battlement Mesa Service Associati
Architectural Committee (BMSAIAC). This form is intended to assist in ensuring that the application submit
- is complete. This will help provide.a quicker, more thorough review process.
It is recommended that each Applicant thoroughly review the regt,ents of the current Axclutectuw
Stap4ards, theemdcd and Restated Declaration of Covenantsjinciudiva R soli hors) and the mord
Final Plat for their respective neighborhood prior to preparing a submittal.
Applicants preparing a submittal should compare the information they are about to submit with the requiremei
of this form and "check off' each required item on the form. If any item is not checked, the Application submit
is not complete and should not be submitted. Incomplete Applications will not be processed and will be return
to the Appiicaut.
In addition to the checklist, Applicants should fill in all of the places on the Application Form where i forrnail
is requested. The Architectural Committee will use this infoz ionevaluating your Application. Iflufonnati
is requested but is not applicable to your specific project submittal, write "N/A" in the space provided. If sol
required information is yet to be determined (such as exterior paint colors you may not have yet selected) wr

T have been retained by Grace Homes to provide engineering services . r the
above referenced project. T have supervised inspection of the foundations and alts prior
to concrete placement to ensure conformance with the drawings that I prepareand were
submitted for permit.
The foundations are structurally adequate to satisfy the building codes •urrently
adopted by Garfield County.
