ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system
(OWTS) on November 29, 2012 for the subject property. This system will serve an agricultural
building with a half bath, a barn with a utility sink, and a 3-bedroom residence.
The installation included a 1000-gallon, two-compartment concrete septic tank with an effluent filter
on the outlet. Effluent gravity flows to a distribution box, and then to four gravelless chamber beds,
each with two rows of 8 Quick 4 chambers in each bed for a total of 64 chambers.
The sewer line was encased within 25-feet of the well and the distance from the absorption area was
confirmed to be greater than l 00-feet.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWTS.
LIMITS:
The observations were based on information submitted. If conditions are different from conditions
described in this letter, ALL SERVICE septic, LLC should be notified. Any additional OWTS
construction must be according to the county regulations. The installer should have documented and
demonstrated knowledge of the requirements and regulations of the county in which they are
working.
