SOIL MANAGEMENT ANd REVEGETATION PLAN
Due to the relatively small disturbance at the Mesa Communication Tower
O Facility location, ConocoPhillips Company will institute the following procedures
a to manage the soil and revegetate the site located in the Southwest One-quarter,v Section 33, Township 4 South, Range 96 West, 6th P.M., Garfield County.O cotoraoo.o
O Prepare Site
- . 1 . An area 140-feet by 160-feet will be cleared and grubbed of vegetation.!t 2. All cleared vegetation will be collected and shipped offsite.O 3. Earthmoving 6quipment will level the main tower site (approximately 50-
a feet by S0-feet) , as required.
O 4. No soil stockpiles will remain.
O Construct Tower Base and Guy Supports
1. Tower contractor will excavate holes for tower foundation and three-(3)
guy support points.
2. The reinforced concrete tower foundation and guy support dead-man shall
be formed, rebar placed and poured with concrete.
3. Once the foundations have cured, removed soil shall be properly
backfilled and compacted to match existing grade.
4. Any spoil materials shall be evenly distributed around the tower area.
O Tower Facilitv Construction
1. Erect tower structure with guy supports.
2. Place gravel within the SO-feet by SO-feet tower area.
3. Install perimeter fencing and gate.
4. Form and pour required concrete pads (slab-on-grade).
5. Install tower support facilities including: support building, solar equipment,
electric generator and propane tank.
Sequence of Sfeps 2 through 4 may vary per contractor's actual approach.
O Post Construction (Reveqetation)
1. All remaining disturbed areas will be revegetated with a Garfield County
and/or surface owner approved seed mix.
2. Monitor site and treat for noxious weeds (reference Weed Management
Plan).

RECLAMATION PLAN
ConocoPhillips Company will utilize the following procedures to reclaim the Mesa
Communication Tower Facility site located in the Southwest One-quarter, Section
33, Township 4 South, Range 96 West, 6h P.M., Garfield County, Colorado,
upon final decommissiong.
Communication Tower / Facilities Removal and Surface Reclamation
1. Remove all communication tower transmission and receiving equipment
attached to the tower.
2. Safely dismantle tower structure and lateral guy supports. Tower structure
and communication equipment to be salvaged (if still useful) and shipped
offsite.
3. Tower foundation structures (tower and guy points) will be over-excavated
and concrete foundations will be entirely removed. The void space will be
properly backfilled and compacted to match existing grade.,
4. Tower support facilities including support building, solar equipment,
propane tank, and electric generator will be shipped offsite.
5. The perimeter fencing including support posts, post foundation
will be removed and shipped offsite for disposal.

6. All concrete pads (i.e., slab-on-grade) will be removed and shipped offsite
for disposal.
7. Surface gravel located within the S0-feet by SO-feet tower area will be
picked up and shipped offsite for disposal.
8. All disturbed areas (including access roads at surface owner's request)
will be re-contoured as required to match natural topography and top soil
shall be placed in support of revegetation efforts.
9. Disturbed areas shall be reseeded with a Garfield County and/or surface
owner approved seed mix.
10. Cover disturbed areas with either weed free mulch or a seed blanket.
11. Monitor disturbed areas for noxious weeds and treat as needed until the
disturbed areas are properly revegetated.
12.Notify Garfield County when revegetation has occurred and schedule an
acceptance i nspection.

WEED MANAGEMENT PLAN
All heavy equipment brought into Garfield County for site development will be
washed prior to entering the County. Any equipment to be moved to the location
from an area known to contain noxious weed species will be cleaned prior to moving
out of the infested area.
Topsoil will be segregated and seeded in accordance with applicable Natural
Resource Conservation Service (NRCS) and/or County standards and/or surface
owner requirements.
When construction is complete, the disturbed areas covered by a concrete slab-on-
grade or gravel will be kept free of all vegetation, including weeds. However, if any
disturbed but unoccupied areas of native soil remain, they will be seeded with native
species based on appropriate NRCS range site and a Garfield County and/or
surface owner approved seed mix. Any noxious weeds found throughout the
operational life of the communication tower will be removed by mechanical/manual
methods, or treated with approved chemicals in compliance with manufacturer
recommendations.
The Mesa Communication Tower Facility site will be inspected at least monthly for
weeds and any weeds will be properly eradicated per the methods outlined above.

Description
Cummins Power Generation commercial generator sets
are fully integrated power generation systems providing
optimum performance, reliability, and versatility for
stationary standby power applications.
A primary feature is strong motor-starting capability and
fast recovery from transient load changes. The torque-
matched system includes a heavy-duty GM 4-cycle
liquid-cooled spark-ignited engine, an AC alternator with
high motor-starting kVA capacity, and an electronic
voltage regulator for precise regulation under steady-
state or transient loads. The GenSet accepts 100% of
the nameplate standby rating in one step, in compliance
with NFPA110 Level 1 requirements.
LP vapor fuel system is standard with several options for
natural gas and LP liquid.
The GenSet offers user-frigndly operation. The
standard PowerCommanO* digital electronic control is
an integrated system that combines engine and
altemator controls for high reliability and optimum
GenSet performance. Optional display provides NFPA
110 Level 1 compliance.
A wide range of options, accessories, and services are
available, allowing configuration to your specific power
generation needs.
Every production unit is factory tested at rated load and
power factor. This testing includes demonstration of
rated power and single-step rated load pickup.

Singlebearing alternators couple directly to the engine flywheelwith flexible discs for drivetrain reliability and
OuriUility. No gear reducers oi speed changers are used. Two{hirds pitch windings eliminate third-order
harmonic content of the AC voltage waveform and provide the standardization desired for paralleling of generator
sets. The standard excitation system is a self (shunt) excited system with the voltag^e regulator powered directly
from the generator set output. fhe standard aliemator is a singjle-phase 4-lead 125oC rise. Optional altemators

GARFIELD COUNTY BUILDING AND PLAI\NING DEPARTMENT
FEE SCIIEDTILE
Garfield County, pursuant to Board of County Commissioners ("Board") Resolution No. 98-09, has established
a fee structure ("Base Fee") for the processing of each type of subdivision and land use applications.
The Base Fee is an estimate of the average number of hours of staff time devoted to an application, multiplied
by an hourly rate for the personnel involved. The Board recognized that the subdivision and land use
application processing time will vary and that an applicant should pay for the total cost of the review which
may require additional billing. Hourly rates based on the hourly salary, and fringe benefits costs of the
respective positions combined with an hourly overhead cost for the office will be used to establish the actual
cost of County staff time devoted to the review of a particular project.
Actual staff time spent will be charged against the Base Fee. After the Base Fee has been expended, the
applicant will be billed based on actual staff hours accrued. Any billing shall be paid in full prior to final
consideration of any land use permit, zoning amendment or subdivision plan. If an applicant has previously
failed to pay application fees as required, no new or additional applications will be accepted for processing until
the outstanding fees are paid.
Checks, including the appropriate Base Fee set forth below, must be submitted with each land use application,
and made payable to the Garfield County Treasurer. Applications will not be accepted without the required
application fee. Base Fees are non-refundable in full, unless a wriffen request for withdraw from the applicant
is submitted prior the initial review of the application materials.
Applications must include an Agreement for Payment Form ("Agreement") set forth below. The Agreement
establishes the applicant as being responsible for payment of all costs associated with processing the
application. The Agreement must be signed by the party responsible for payment and submitted with the
application in order for it to be accepted.

2. APPLICANT understands and agrees that Garfield County Resolution No. 98-09, as amended,
establishes a fee schedule for each type of subdivision or land use review applications, and the guidelines for
the administration of the fee structure.
3. APPLICANT and COLINTY agree that because of the size, nature or scope of the proposed
project, it is not possible at this time to ascertain the full extent of the costs involved in processing the
application. APPLICANT agrees to make payment of the Base Fee, established for the PROJECT, and to
thereafter permit additional costs to be billed to APPLICANT. APPLICANT agrees to make additional
payments upon notifrcation by the COIINTY when they are necessary as costs are incurred.
4. The Base Fee shall be in addition to and exclusive of any cost for publication or cost of
consulting service detennined necessary by the Board of County Commissioners for the consideration of an
application or additional COLTNTY staff tirne or expense not covered by the Base Fee. If actual recorded costs
exceed the initial Base Fee, APPLICANT shall pay additional billings to COUNTY to reirnburse the COTINTY
for the processing of the PROJECT mentioned above. APPLICANT acknowledges that all billing shall be paid
prior to the final consideration by the COLINTY of any land use permit, zoning amendment, or subdivision
plan.
