In accordance to your request, on July 7, 2011 Oddo Engineering's personnel observed
the placement of the footing reinforcement at the Barnes Cabin site. Oddo Engineering's
scope of service was to verify the correct size of rebar, rebar placement, and footing size
only; overall building dimensions and material testing were not verified. Our observation
waslimited to areas with easy access and not all wall areas were verified. Please see
below for field observation notes:
1. Rebar appears to be clean with minimal rust.
2. Oddo Engineering stated to S2M that verification of the #5 vertical dowels prior to
concrete placement shall be centered in the footing and no stabbing or moving of
the dowels/bars is permitted once the concrete has been consolidated.
3. The exterior spread footing dimensions appeared to 24" wide as required along with
the additional 4'-0" square footing at grid "3 & A.5" was installed per the plans.
4. #5, #6, #7 bars were used for footing reinforcement and appeared to have the
required minimum clearances by the way of concrete block risers.
5. #5, #6, #7 dowels were used and appeared to have the required minimum lap splice
of 40 bar diameters.
6. Proper concrete consolidation with a vibrator appeared to be used by S2M.
7. Oddo Engineering was not present for the full duration of concrete placement.
In general, the rebar placement in the footing was satisfactory and appears to be per Oddo
Engineering's drawings dated "Construction 7-7-11." Let me know if you have any

In accordance to your request, on July 14, 2011 Oddo Engineering's personnel observed
the placement of the exterior wall reinforcement and interior pad footing reinforcement at
the Barnes Cabin site. Oddo Engineering's scope of service was to verify the correct size
of rebar, rebar placement, wall thickness and footing size only; overall building dimensions,
pad locations, and material testing were not verified. Our observation was limited to areas
with easy access and not all wall areas were verified. Please see below for field
observation notes:
1. S2M Concrete was not present at time of inspection.
2. Rebar appears to be clean with minimal rust.
3. Appears that #5 & #6 bars were used for the interior pad footing reinforcement and
appeared to have the required noted spacing and the minimum clearances.
4. Appears that #5 & #6 vertical bars (mixed together) appeared to be used and
appeared to have the required minimum lap splice of 40 bar diameters with the
dowels.
5. The corner bar lap appeared to be 24".
6. Verification of the wall reinforcement and pad footing reinforcement shall be
performed by S2M. prior to concrete placement
7. The vertical bars appeared to be centered in the wall with the required 4" clearance
as stated on Oddo Engineering's drawings.
8. The wall dimension appeared to 8" wide as required.
9. A 4'-0" square prefabricated steel window frame was installed along grid "A" and
appeared to have #5 reinforcement each side and top and bottom which is
acceptable.
10. The interior pad footings appeared to be sized according to the latest Oddo
Engineering's drawings.
11. It is assumed that proper concrete consolidation with a vibrator will used by S2M.
12.Oddo Engineering was not present at time of concrete placement.

RE: Final Structural Approval of Barnes Cabin, Garfield County, Colorado
Dear Bill and Barbara:

We understand that Garfield County requested a structural observation of the final existing
framing per the Oddo Engineering drawings dated 7-18-11. Glenwood Structural and Civil Inc.
performed an onsite observation on August 22, 2013 and verified compliance with the
construction documents with some deviation.
The deviations were described in an email to you from Adolfo Gora P.E. dated August 23, 2013.
Oddo Engineering responded to that email on September 11, 2013 with red mark comments to
each of the five deviations. A final field report was submitted by Adolfo Gora on September 11,
2013.
Since then we have been in contact with Adolfo Gora, Mark Stoelzel and you on acceptable fixes
for the deviations which are consistent with our email dated September 11, 2013. To date we
understand that all fixes to the deviations have been completed.
In summary, the existing framing has been verified and complies with the Oddo Engineering
construction documents dated 7-18-11 and the deviations discovered on August 22, 2013. The
construction of the building is acceptable from the general observation as describe above.
