-Requirements:
1. Plans and specifications for proposed use.
2. Letters of approval from utility companies, sewage and water.
Evidence of road access and other information deemed necessary.
3. A vicinity map, drawn to scale, depicting the subject property,
location and use of buildings and structures on adjacent lots.
An impact statement on the proposed use where required by Sections
5.03-5.03.12 of the Zoning Regulations.
5 Notice, including the name of the applicant, description of the
subject lot, a description of the proposed special use and the time
and place for the hearing, shall be given in the newspaper of general
circulation in that portion of the County .at least fifteen (15) days
prior to such hearing time. The' applicant shall bear the cost of
advertising.
6 A copy of Assessor'smap showing property; and a listing of all
adjoining property owners of said property. Notice, containing
information described under paragraph 5, shall be nailed to all owners
of record of these lots, at least 5 days prior to the hearing, by
certified, return receipt mail, and the receipts shall be presented at
the hearing by the applicant.
7. A fee of x$00.00 shall be charged for each application and shall be
submitted with the application.
8. Attach copy of proof of ownership (deed; title insurance).
This Special Use permit is in compliance with Sections 5.03 and 9.03 of
the Garfield County Zoning Resolution and as per application.
List all adjoining property owners of said property
Attach map showing all improvements (present and proposed) and all
pertinent; distances and area. Show all improvements on your property and
adjoining property.

Gentlemen:
American Matrix Reserves, Inc. (AMR) requests permission to build an 8,200
foot gas pipeline in Garfield County, Colorado. The line will gather gas
from our Baldy Creek 3-28 Well and connect to Northwest Pipeline at our
Baldy Creek 2-20 Well. The line will be constructed on an existing private
dirt road that AMR has maintained. A right of way has recently been
surveyed and will be recorded from both the State Department of Wildlife
and the present owner of Ming Properties, Inc. (see enclosed map).
The line will be 2" of steel pipe buried a minimum of 3' deep. It is hoped
that gas will be purchased by Northwest from this well which was completed
in May of 1981.
No new roads will be necessary and vegetation, wildlife, and water will not
be disturbed. For a period of three to four weeks, the construction will
require continuous daytime human and vehicular activity. This line will be
constructed and maintained in accordance with Department of Transportation
Pipeline Safety Regulations, Natural Gas Parts 191 and 192.
We anticipate immediate need for construction because of the impending
weather in this high terrain.

Gentlemen,
Kindly be advised that "American Matrik Reserve is
hereby authorized by our firm to apply to Garfield County
for a special use permit for the proposed construction of
the subject pipeline.
Also, the right of way agreement with Matrik and all
other parties concerned has been negotiated and agreed upon.
Said formal agreement in writing is in the process of
being executed and recorded at the county.

Dear Sir:
In reference to your letter of February 4, 1985, the "two --inch natural
gas pipeline" should not be classified as a pipeline. The two-inch line
is the production flow line for our USA 1--33DF gas well that was
permitted by Garfield County Resolution No. 81-293. The USA 1-33DF was
completed in August, 1982 and has been shut in since then pending
approval from Internorth to connect this well to their pipeline.
The metering facilities for the USA 1-33DF were located by Internorth at
the well pad for Spears 1-28DF. The Spears 1-28DF was permitted by
Garfield County Resolution No. 80-287. The two-inch flow line connects
the USA 1-33DF wellhead to these metering facilities.
Since Garfield County granted approval to drill the USA 1-33DF,
additional permits should not be required for the flow line to produce
the well. Flow lines are a normal and necessary requirement for all
wells.

RE: A two inch natural gas pipeline in Section's 28 and 33, T7S, R99W
Dear Mr. Amen,
This office has received notice of your intent to construct the above
referenced pipeline in Garfield County. All pipeline permits issued for
Open Space (Federal) lands require a $500 fee and a Conditional Use
permit. Where the pipeline is to be costructed on private land a Special
Use permit is required with the same fee. Copies of application forms for
both types of permits are enclosed.
Please contact this office immediately in regards to your application for
the above pipelines. We would be glad to answer any questions or concerns
regarding the permitting process and look forward to working with you on
the project,
