We would like to request a special use permit for a house located
at 0294 117 Road, Glenwood Springs, Colorado 81601. The house is
on its own sewage and water and will maintain present utilities
and roads. The requested use will be for a residential care facility
for 8 developmentally disabled adults originally from our area. It
will be operatedunder the auspices of Upper Colorado River Community
Center Board (Happy Time School) Adult Program. The facility will
be run as a group home type arrangement with supervision and train-
ing provided by live-in residential counselors. The training provided
at the facility will be mainly independent living skills e.g. cooking,
budget management, personal hygiene, use of community resources, etc.
During the day the the adult residential clients will participate
in the day program of the U.C.R.C.C.B. Adult Program.
This group home has been made possible because of a grant awarded by
the Developmental Disabilities Council of the State of Colorado. The
facility will be licensed and certified by the State Department of
Health. It will be licensed as a Res dential Care Fa.cilily and Certified
as an Intermediate Care Facility for Mentally Retarded. This will
allow additional on-going funding through Federal Title 19.
We feel that this residential facility will provide a much needed
resource to mentally retarded adults originally from our community and
their families and friends. We therefore urge your granting us this
special use permit.
Attached are letters of support from generic agencies that will be
directly impacted by this project.

State of Colorado, to grant a special use in connection with the following
described property situated in the County of Garfield, State of Colorado;

Dear Mrs. Norris:
As a result of the survey that was conducted on the residence in Glenwood
Springs, Mr. Johnson and I have the following comments:
1. Provide the following in each of the bedrooms when they are to be occupied.
a) Comfortable bed. (Rollaway -type bed, cots or folding beds
are not acceptable.)
b Mattress and pillow.
c Cabinet or bedside table.
d) Incombustible wastebasket with an impervious liner.
e) Chair.
f) Storage facilities.
g) Towel racks for each room occupant,
2. Provide a toilet room with a hand sink on the second floor. It is rec-
ommended that a bathing unit be provided on the second floor also.

3. Provide grab bars at bathing units.
4. Make the bottoms of all tubs and showers non -slip.
5. Provide a fence on the sides of the property that drops off to pevent
accidents and injuries to the residents.
6, Provide a stair to the second floor that is 36 inches wide with a handrail
on at least one side. The stair must be enclosed to meet the Federal
requirements for an ICF/MR. The risers must not exceed eight inches and
the treads must not be less than nine inches wide. It would be appre-
ciated if you would send a drawing of what you propose for the stair and
the upstairs bathroom for review by this office before construction, so
we could review and comment on them. Also include the second stair from
the second floor.

7.' The upstairs rooms must have a minimum ceiling height at the east and
west walls of five feet. This can be accomplished by providing addi-
tional walls inside those presently provided.
cic`� ` 8. The space in front of the toilet stool on the first floor must be at
/ least 24 inches.
9. All rooms must be provided with heat. There was no evidence of heat
in the north bedroom on the second floor. Heat must also be provided
in the new toilet room on the second floor.
10. Provide a fire extinguisher in the following locations:
a) Kitchen.
b Resident area on the first floor.
c Resident area on the second floor,
11. Provide a second means of egress from the second floor. Since the pro-
posed second exit will be through the north bedroom, the door to this
room must not be provided with a lock.
12. Provide a fire alarm system in the residence, This would consist of a
manual pull station at each exit on the first floor, a manual station
on the second floor and a sounding device on each floor.
13. After occupancy, there must be fire drills held at least once a month
and the fire alarm system must be used to conduct these drills.
14. Provide a front screen door.
15, The side screen door needs structural repair and extension to reach the
threshold.
16. All openable windows require screens,
17. Towel bars must be provided for each resident,
18. The downstairs bathroom sink fixture needs repair or replacement.
19. The downstairs bathroom door has a hole, Needs good repair or replacement.
20, Need door to bathroom off corner (front) bedroom.
21, Junk and debris must be removed from the grounds, garage, etc, Any useful
material must be stacked neatly.

22. Provide sound outside refuse container(s) with tight fitting lid(s)
and adequate capacity must be provided.
23. Approval of water source must be made by local sanitarian and docu-
mentation submitted to this Department.
24. High grass and weeds must be kept cut.
25. The laundry (dryer and washing machine) must be separated from the
other equipment along the cabinet wall of the kitchen by a partial wall
built out into room from outside wall to a point a few inches past the
front of the laundry machines. Also, a policy must be written to the
effect that no laundry will be done while food is being prepared, served,
or'while dishes are being washed.
Enclosed is a section of the Federal survey form as promised by Mr. Wil
Johnson. These are for your use.
This concludes our comments that resulted from our survey. If you have any
questions on any of these, please let me know.

Our Department wholeheartedly supports the ongoing
efforts of Happy Time School, Adult Program, toward re-
integration and community placement of institutionalized
developmentally disabled clients from our area. It appears
to us that Happy Time School has high quality programs and
personnel with which to serve these clients, and that they
have now found a way to bring these people back to our area
to receive the benefits of those programs. Based upon our
past experience with this Community Centered Board, we an-
ticipate that the best possible efforts 14111 be forthcoming
to assure success of the reintegration program.

We at Rehabilitation have long supported the Community Center
Board and have been aware of the need for programs and services
for the developmentally disabled which are offered by the Board.
We further feel that the programs offered are of excellent quality

We are also aware of the severe space limitations and housing
shortage which exist in our area and feel that a residential
