Until further notice, the followrng conditions shall be attached to all approved Permits-to'Drill for all fvlesaverde Group or deeper wells in the Mamm Creek Field, Garfield County: 1. The Mamm Creek Field ,Area is defined as follows: Township 6 South, Flanges 91-93 West, 6rh F'.M. Township 7 $outh, Ranges 91-93 West, 6b P.M. Township 8 South, Ranges 91-93 West, 6rn P.M. Township I South, Range g1 West, 6'r'P.M. Z. Wells shall be required to be cenrented to five hundred (500) feet above the top ol gas. a- The cement coverage shall be verified with a csment bond log. b. The cernent compressive strength for all cemenl slunies used to cement the productlon casing shallbe sufficient to meet the requirements of Rule 317, i.and allow the carnent to be aasily identified wlth the use of a cement bond log (CBL). g. A drilling proonosls showing projected top of gas and formation tops shall be required with the Application for Permil-to-Drill Fonn 2. This intormalion shall be provided in the torm of a wellbore diagram showing the top ol gas, formation tops, and top of cement for each stage. Proceclure rnd Heporting Ghanges: 1. Upon completion of the primary cementlng operation, the annular fluid level around the pioduction caeing shall be monltored for a mlnimum of 4 hours prior to the installation of the casing slips. This equirernent may also be met by setling the casing slips immediately after cementing and maintaining the ability to rnonltor the annular fluid level and keep the hole full. The amount of mud that is used to keep the hole full shall be reconded. lf mud volumes in excess of ntrenty (20) banels are necessary to keep the hole full, the loss of fluld shall be repofted to the COGCC imrnediately. ltre record of the mud volume needed to keep the hole full shall be included on the Sundry Notice, Form 4 requesting approvalto cornplete the well. 2. The bradenhead pressure shall be measured at intervals of 6, 1 2, ?4,48 and 72 hours atter the production casing is cemented. lt bradenhead pressures greater than one hundred fifty t150) psig are olrserved. such oressures shall be immediately reported to the COGCC and a remediatlon procedure shall be prepared for COGCC approval. 3. Operators shall perfom a Formation lntegrily Test (FlT) at least fifty (50) feel below the sudace cssing shoe. The intermediate casing shall be cemented to a height of at least five hundred (500) feet above the depth of the intermediate caslng shoe if no kicks have been encountered in the Wasatch Formation. lf klcks have been encountered in the Wasatch Formation the top of cement shall be adjusted to comply wlth Rule 317.1. and to cover all gas bearing inlenrals. lf intermediate casing is neeessary, lhe requirement to obtain a temperature $urvey and cement bond log on the production casing within a 12 to 48 hour period following the cementing of the produciion casing shall not apply- The operator shall obtaln COGCC staf{ approvalto complete the well prior to conducting completion operations. e.
