This Notice lo Operatore was developed consldering information thal was included in he March
10, 2006 reporl on the Phase I Hydrogeologic Characterization of the Mamm Creek Field Araa in
Garfield Coung as well as other pertinent information regarding the prudent drilling of oil and gas
wells in lhe Mamm Cresk Fietd Area.
Until further notice, the followrng conditions shall be attached to all approved Permits-to'Drill for all
fvlesaverde Group or deeper wells in the Mamm Creek Field, Garfield County:
1. The Mamm Creek Field ,Area is defined as follows:
Township 6 South, Flanges 91-93 West, 6rh F'.M.
Township 7 $outh, Ranges 91-93 West, 6b P.M.
Township 8 South, Ranges 91-93 West, 6rn P.M.
Township I South, Range g1 West, 6'r'P.M.
Z. Wells shall be required to be cenrented to five hundred (500) feet above the top ol gas.
a- The cement coverage shall be verified with a csment bond log.
b. The cernent compressive strength for all cemenl slunies used to cement the
productlon casing shallbe sufficient to meet the requirements of Rule 317, i.and
allow the carnent to be aasily identified wlth the use of a cement bond log (CBL).
g. A drilling proonosls showing projected top of gas and formation tops shall be required with
the Application for Permil-to-Drill Fonn 2. This intormalion shall be provided in the torm of a
wellbore diagram showing the top ol gas, formation tops, and top of cement for each stage.
Proceclure rnd Heporting Ghanges:
1. Upon completion of the primary cementlng operation, the annular fluid level around the
pioduction caeing shall be monltored for a mlnimum of 4 hours prior to the installation of the
casing slips. This equirernent may also be met by setling the casing slips immediately after
cementing and maintaining the ability to rnonltor the annular fluid level and keep the hole full.
The amount of mud that is used to keep the hole full shall be reconded. lf mud volumes in
excess of ntrenty (20) banels are necessary to keep the hole full, the loss of fluld shall be
repofted to the COGCC imrnediately. ltre record of the mud volume needed to keep the
hole full shall be included on the Sundry Notice, Form 4 requesting approvalto cornplete the
well.
2. The bradenhead pressure shall be measured at intervals of 6, 1 2, ?4,48 and 72 hours atter
the production casing is cemented. lt bradenhead pressures greater than one hundred fifty
t150) psig are olrserved. such oressures shall be immediately reported to the COGCC and a
remediatlon procedure shall be prepared for COGCC approval.
3. Following ihe cementing operation, a combination temperaturelcement bond tog shall be i'un
wlthin 12 to 48 hours to locate the actual cement top. lf the cement top does not meet ihe
requirements of this Notice to Operators. the COGCC shall be immediately notified and a

Operatcrs drilling within the East Mamm Creek Area are requlrect to follow the general
procedures descrlbed above and the special prccedures ciescribed in 4.a. through g. below:
a. An operator ma,y use only one (t ) rig at a lime to drill that operator's first five (5) wells
within the East Mamm Creek Area. lf the flrst five (5) wells are drilled wilhout incident,
thsn the operator may request approvat from the Direcior to use two (2) rigs at a time.
No more than two (2) rigs shallbe used by an operator in the East Mamm CrBek Area.
h. Surface casing shall be set at a depth equivalent to 15ozl, of the proposed totaldepth of
the well, or five hundred (500) feet below the depth oi any water well within a one (1) mile
:ladius, wftichever is greater, and shall be cemented to surface.
c. Operators shall perfom a Formation lntegrily Test (FlT) at least fifty (50) feel below
the sudace cssing shoe. The teet shall be performed to an equivalent rnud weighl of
13.0 pounds per gallon (ppg).A loss of more than '1 A",L ol the surface pressure applied to
perform the tes'i, in a 15 minute time period, shallconstitute failure. Faited FlTs shall be
reported to the COGCC staff immediately._COGCC staff shall be notified at least I hours
tn advance of the test to allow them the opportunity to witness it. The resuhs of the test
shallbe reported on the Form 4 Sundry Notice Heqr.resl to Complete and noted on the
wellbore schematic thai accompanles the Forrn 4. The FIT report shall inelude the depth

remediation Procedure shall be prepared for COGCC approval' Running the CBL rnay be

NOTE;
The option of defening the running of the CBL beyond 48 hours is not allowed ln the below-
delined East Mamm Crcet< Area. The CBL shall be run within 12 to 48 hours within the East
Mamm Creek Area.

d. lf the well bore does not test to the equivalent of mud weight of 13.0 ppg during the
above noted FlT, the operator shall be required to set intermediate casing at a depth at
least flfu (50) feei below the top of the Mesaverde Formation. The intermediate casing
shall be cemented to a height of at least five hundred (500) feet above the depth of the
intermediate caslng shoe if no kicks have been encountered in the Wasatch Formation. lf
klcks have been encountered in the Wasatch Formation the top of cement shall be
adjusted to comply wlth Rule 317.1. and to cover all gas bearing inlenrals. lf intermediate
casing is neeessary, lhe requirement to obtain a temperature $urvey and cement bond
log on the production casing within a 12 to 48 hour period following the cementing of the
produciion casing shall not apply- The operator shall obtaln COGCC staf{ approvalto
complete the well prior to conducting completion operations.
e. All drilling rlg employees shall have adequate understanding of and be able to operate
the blowout provenlion system. Well controltraining for blowout prevention shall be
required for at least one (1) person at th€ well slte during drilling operations.
f. Choke pra€sures during well control operations shall be restricted to levels that will
not cause the maximum wellbore lntegdly demonstrated by the FIT to bE exceeded. As a
precautionary measure, if the choke pressure exceeds the surface pressure used to
detErmine wellbore integrity during the FlT, il shall be immedlately reported to the
CCIGGC staff and lha operator shall submil a reporl of the well conlrolevent on a Sundry
Notice, Form 4 within 24 hours. The report shall lnclude the following information:o date and time of the event,. total depth of the well at the time of the event,. sildac€ easing depth, size and cernenting data,. type of kick (gas, water, or oll),. shut in drill pipe pressure, shtrt in casing pressure, or any other pressure
measurement or informatlon usod to determine the mud weight reguired to control
the kick,. initial mud weight at the time of the event,r pit gain volurne,r mud weight required to control the kick,. maximum choke pressure that occurred while circulating out the kick,. afiY indication of fluids migrating outside of ihe surface casing (surface expression,
etc.),. a narrative descriptlon of the well control evsnt and current condition of the well,
g. ln addltlon to the bradenhead pressure rnsasurernent requirements in the enlire
Mamm Creek Field Area, in the East Mamm Creek Area the bradenhead pressure of
each well on a pad shall be monrtored dally until 30 clays iollowing the cementing of the
prod.uotion casing ol the last wetl on the pad, Following that, the 6radenheacl pressures
shall be rnonitsned monthly for the following 12 rnonih perlod. lf bradenhead pressuresgreater than one hundred fifly (150) psig are observod, such pressures shall beimmediately reported to the COGCC and a rernodiation procedure shall be prepared lor
COGCC approval. These requhements shall also apply io rnonitoring intermediate casingprassure if intermediate casing is required.

5. Prior to completion of the well, the l:radenhead pressure rscord, cement bond log,
temparature survey log, and rsvised formation tops shall be provided lo the COGCC
Norlhwest Area Englneer along r,vith a Sundry lrlotice, Form 4 requestlng approval to
complete the well.
6 All information shall be subrnitted electronically via e-mail to the Northwest Area Engineer. A
separate e-mail with the required attachments, provtded in the required format, sha[ be sentfor each well.
The attachrnents shall include the follou,,ing:
a.) Hesults of the FIT and the finat muct weight.
b.) A CBL wlth ternperature survey in ..pdf or Schlumbgrgar format.
c.) A Sundry Notice, Form 4'reguesting approvalto complete the well stating the welthas been successfully cemented according io the approved plan.
d.) ,{ summary of lhe bradenhead pressure measurements.e.) A wellbore diagranr with the as-bullt cement tops, formation tcps, top of gas,
casing shoes etc.
f.) A temperature survev in a single page format.
