The followine: items are required by Garfield County for a final Inspection:
1) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non -absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan and
it's discovered that required information has not been provided by the applicant, this will result in the
delay of the permit issuance and in proceeding with building construction. The owner or contractor shall
be required to provide this information before the plan review can proceed. Other plans that are in line
for review may be given attention before the new information may be reviewed after it has been
provided to the Building Department.
TWO (2) SETS OF CONSTRUCTION DRAWINGS & TWO (2) SITE PLANS MUST BE
SUBMITTEDFOR ALL APPLICATIONS.
Please review this document to determine if you have enough information to design your project
and provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project. Any project with more than ten (10) occupants requires the plans to be sealed by a
Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building, plumbing
and mechanical codes, applicants are requested to review the following checklist prior to and during
design. Applicants are required to indicate appropriately and to submit the completed checklist at
time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and drawn
to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks,
balcony, steps, hand rails and guard rails, windows and doors, including the fmish grade line and

original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind speed,
wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be in compliance with the 2009 IRC, and 2009 IECC.
1. Is a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each comer of the proposed structure(s) prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Properties with slopes of
30% or greater must be shown on the site plan. (NOTE Section: 106.2) Any site plan for the
placement of any portion of a structure within 50 ft. of a property line and not within a previously
surveyed building envelope on a subdivision final plat shall be prepared by a licensed surveyor
and have the surveyor's signature and professional stamp on the drawing. Any structure to be
built within a building envelope of a lot shown on a recorded subdivision plat shall include a
copy of the building envelope as it is shown on the final plat with the proposed structure located
within the envelope.

If any required information is missing delays in issuing the permit are to be expected. Work may not
proceed without the issuance of a permit. If it is determined by the Building Official that additional
information is necessary to review the application and plans to determine minimum compliance with the
adopted codes, the application may be placed behind more recent applications for building permits in the
review process and not reviewed until required information has been provided and the application rotates

Dear Ms. Latkiewicz:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study for design
of foundations at the subject site. The study was conducted in accordance with our
agreement for geotechnical engineering services to you dated October 7, 2010. The data
obtained and our recommendations based on the proposed construction and subsurface
conditions encountered are presented in this report.
Proposed Construction: The proposed residence will be one and two story wood frame
construction above a partial basement and partial crawlspace with an attached garage. The
residence will be located on the site as shown on Figure 1. Basement and garage floors
will be slab-on-grade. Cut depths are expected to range between about 3 to 8 feet.
Foundation loadings for this type of construction are assumed to be relatively light and
typical of the proposed type of construction.
If building conditions or foundation loadings are significantly different from those
described above, we should be notified to re-evaluate the recommendations presented in
this report.
Site Conditions: The site is vacant of structures and vegetated with grass and weeds .
The ground surface is relatively flat with a slight slope down to the northwest.
Subsidence Potential: Bedrock ofthe Pennsylvanian age Eagle Valley Evaporite
underlies the Aspen Glen Subdivision. These rocks are a sequence of gypsiferous shale,
fine-grained sandstone and siJtstone with some massive beds of gypsum and limestone.
There is a possibility that massive gypsum deposits associated with the Eagle Valley
Evaporite underlie portions of the lot. Dissolution of the gypsum under certain conditions
can cause sinkholes to develop and can produce areas of localized subsidence. During
previous work in the area, broad depression areas and several sinkholes were observed
scattered throughout the Aspen Glen Subdivision. The nearest sinkholes are 700 feet to
the southeast and 600 feet to the north and northeast of the subject Jot. These sinkholes
appear similar to others associated with the Eagle Valley Evaporite in areas of the
Roaring Fork River valley. The site is mapped as being located about 150 feet north of

the northern edge of a broad depression area possibly related to old ground subsidence
(Chen-Northern, Inc., 1991).
Sinkholes were not observed in the immediate area of the subject lot. No evidence of
cavities was encountered in the subsurface materials; however, the exploratory pits were
relatively shallow, for foundation design only. Based on our present knowledge of the
subsurface conditions at the site, it cannot be said for certain that sinkholes will not
develop. The risk of future ground subsidence on Lot E-39 throughout the service life of
the proposed residence, in our opinion, is low; however, the owner should be made aware
ofthe potential for sinkhole development. If further investigation of possible cavities in
the bedrock below the site is desired, we should be contacted.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
excavating two exploratory pits at the approximate locations shown on Figure l. The
logs of the pits are presented on Figure 2. The subsoils encountered, below about 6
inches oftopsoil, consist of silty sandy gravel with cobbles and small boulders. A 6-inch
thick clay layer was observed in Pit 1 above the gravel soil. Results of a gradation
analysis performed on a sample of sandy gravel with cobbles gravel (minus 5 inch
fraction) obtained from Pit 1 are presented on Figure 3. No free water was observed in
the pits at the time of excavation and the soils were slightly moist to moist.
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature of the proposed construction, we recommend spread
footings placed on the undisturbed natural granular soil designed for an allowable soil
bearing pressure of3,000 psffor support of the proposed residence. The sandy clay soils
tend to compress after wetting and should be removed from beneath proposed footings.
Footings should be a minimum width of16 inches for continuous walls and 2 feet for
columns. Loose or disturbed soils and sandy clay encountered at the foundation bearing
level within the excavation should be removed and the footing bearing level extended
down to the undisturbed natural granular soils. Exterior footings should be provided with
adequate cover above their bearing elevations for frost protection. Placement of footings
at least 36 inches below the exterior grade is typically used in this area. Continuous
foundation walls should be rc:inforced top and bottom to span local anomalies such as by
assuming an unsupported length of at least 10 feet. Foundation walls acting as retaining
structures should be designed to resist a lateral earth pressure based on an equivalent fluid
unit weight of at least 45 pcffor the on-site soil as backfill. A representative ofthe
geotechnical engineer should observe all footing excavations prior to concrete placement
to evaluate bearing conditions.
Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to suppo~ lightly
loaded slab-on-grade construction. To reduce the effects of some differential movement,
floor slabs should be separated from all bearing walls and columns with expansion joints
which allow unrestrained vertical movement. Floor slab control joints should be used to
reduce damage due to shrinkage cracking. The requirements for joint spacing and slab
reinforcement should be established by the designer based on experience and the intended
slab use. A minimum 4 inch layer of free-draining gravel should be placed beneath
basement level slabs to facilitate drainage. This material should consist of minus 2 inch

aggregate with less than 50% passing the No. 4 sieve and less than 2% passing the No .
200 sieve.
All fill materials fur support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site granular soils devoid of vegetation, topsoil and oversized rock.
Underdrain System: Although free water was not encountered during our exploration, it
bas been our experience in the area that local perched groundwater can develop during
times of heavy precipitation or seasonal runoff Frozen ground during spring runoff can
create a perched condition. We recommend below-grade construction, such as retaining
walls, crawlspace and basement areas, be protected from wetting and hydrostatic pressure
buildup by an underdrain system.
The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free-draining granular material. The drain should
be placed at each level of excavation and at least 1 foot below lowest adjacent finish
grade and sloped at a minimum 1% to a drywell or sump and pump. Free-draining
granular material used in the underdrain system should contain less than 2% passing the
No. 200 sieve, less than 50% passing the No. 4 sieve and have a maximum size of2
inches. The drain gravel backfill should be at least 1 ~ feet deep.
Surface Drainage: The fullowing drainage precautions should be observed during
construction and maintained at all times after the residence bas been completed:
1) Inundation ofthe fOundation excavations and underslab areas should be
avoided during construction.
2) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas. Free-draining wall backfill should be
capped with about 2 feet of the on-site, finer graded soils to reduce surface
water infiltration.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the fOundation in all directions. We
reconunend a minimum slope of 6 inches in the first 1 0 feet in unpaved
areas and a minimum slope of3 inches in the first I 0 feet in pavement and
walkway areas.
4) Roof downspouts and drains should discharge well beyond the limits of all
backfill
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either express or implied. The conclusions and recommendations submitted in
this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Figure I and to the depths shown on Figure 2, the proposed type of
construction, and our experience in the area. Our services do not include determining the
presence, prevention or possibility of mold or other biological contaminants (MOBC)

developing in the future. If the client is concerned about MOBC, then a professional in
this special field of practice should be consulted. Our fmdings include interpolation and
extrapolation of the subsurface conditions identified at the exploratory pits and variations
in the subsurface conditions may not become evident until excavation is performed. If
conditions encountered during construction appear different from those described in this
report, we should be notified at once so re-evaluation of the recommendations may be
made.
This report has been prepared for the exclusive use by our client for design purposes. We
are not responsible for teclmical interpretations by others of our information. As the
project evolves, we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations, and to
verify that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We recommend on-site observation of excavations and foundation
bearing strata and testing of structural fill by a representative of the geoteclmical
engmeer.

As requested by Dan Gruenefeldt, a representative of Hepworth-Pawlak Geotechnical, Inc .
observed the excavation at the subject site on March 4, 20 I I to evaluate the soi Is exposed
for foundation support. The findings of our observations and recommendations for the
foundation design are presented in this report. We previously conducted a subsoil study for
design of foundations at the site and presented our findings in a report dated November I 1,
20 J 0, Job No. II 0 358A. Our services were performed in addition to our agreement for
geotechnical services letter to you dated October 7. 20 I 0.
At the time of our visit to the site, the foundation excavation had been cut in two levels from
2 to 13 feet below the adjacent ground surface. The crawlspace area on the east side ofthc
building area had been compacted and was covered with insulating blankets. The e xcavation
for the basement level was nearly completed at the time of our site visit. The soils exposed
in the bottom of the excavation consisted of dense, sandy gravel with cobbles and boulders
up to 3 feet in size. No free water was encountered in the excavation and the sods were
slightly moist.
The soil conditions exposed in the excavation are consistent with those previously
encountered on the site and suitable for support of' spread footings designed for the
recommended allowable bearing pressure of 3,000 psf. Loose and disturbed soils should be
removed in the footing areas to expose the undisturbed natural gravel with cobble soils . The
bearing soils should be protected from frost and concrete should not be placed on frozen
soils. Other recommendations presented in our previous repon which arc applicable should
also be observed.
The recommendations submitted in th is letter are based on om ohservat ion of th e soi ls
exposed within the foundation excavat io n and the previous subsu rface exploration at the
