1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road; ·
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. A complete bathroom, with washbowl, tub or shower, toilet stool, hot and cold rutming water,
non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3} steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all 1997 UBC requirements;
6. Outside grading do.ne to where water will detour away from the building;
7. Exceptions to the outside steps, d~cks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, where applicable.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE

In order to understand the scope of the work intended under a permit application and expedite
the issuance of a pennit it is important that complete information be provided. Adequate and
complete infonnation will prevent delays in the plan review process. Reviewing a plan and the
discovery that required infonnation has not been provided by the applicant may result in the
delay of the permit issuance and in proceeding with building construction. The owner or
contractor may be required to provide this infonnation before the plan review may proceed. This
causes delays because other plans that are in line for review may be given attention before the
new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. If you do not, it may
be helpful to obtain a book titled "Dwelling Construction Under the Uniform Building
Code". This book is available to you through this department at our cost. Also, please
consider using a design professional for assistance in your design and a construction
professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following checklist prior
to and during design.

Plans to be included for a Building Permit must be on draft paper at least 18"x 24"" and
drawn to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with
decks, balcony steps, hand rails and guard rails, windows and doors, including the finish grade
line. A section showing in detail, from the bottom of the footing to the top of the roof, including
re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material.
A window schedule. A door schedule.
A floor framing plan, a roofing framing plan, roof must be designed to withstand a 40 pound per
square foot. up to 7,000 feet in elevation, an 80 M.P.H. windshear, wind exposure B, windload of
15 pounds per square foot and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 1997 UBC, UMC and 1997 UPC.
Applicants are required to indicate appropriately and to submit completed checklist at
time of application for a permit:
1. Is a site plan included that identifies the location of the proposed structure, additions or
other buildings, setback easements, and utility easements showing distances to the
property lines from each comer of the proposed structure prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Slopes of 30%
or more on properties must be show on site plan. NOTE: 106.3.1 (7.) Any site plan for
the placement of any portion of a structure within 50 ft. of a property line and not within
a previously surveyed building envelope on a subdivision final plat shall be prepared by a
licensed surveyor and have the surveyors signature and professional stamp on the
drawing. Any structure to be built within a building envelope of a lot shown on a
recorde~sub ivision plat, shall include a copy of the building envelope as it is shown on
the final p with the proposed structure located within the envelope.

1. This access shall be constructed 20 -30 feet wide w ith turning radii to accommodate the
minimum turning radius of the largest vehicle or 20 foot, whichever is greater. Thla design
shall be In conformance with section 4 of the State Highway Acce5S Code, 2CCR 601-1.
2. The access shall be surfaced In accordance with Section 4.7 of the Access Code immediately
upon completion of earthwork construction and prior to use . This access shall be hard
surfacad in accordante wnn s8ction 4 . 7 of the Access Code a minimum distance of 20 feat
from the traveled way. Where the hard surface is to abut existing pavement , the exlsUng
pavement shaH be saw cut and removed a minimum of one foot back from the exlatlng edge
for bituminous. or until an acceptable existing cross slope is achieved. Surfacing shall meet
the Oepar1ment's speclficatlonl wtth minimum surfacing to be equal to or great.r than existing
~hwaX.,CW"'Itlons.

The culvert shall be kept free of blockage to maintain proper flow and drainage.
4. This permitted acceM is only for the use and purpose stated in the Application and Permit.
This Permit is Issued In accordance with the State Highway Access Code (2 CCR 601-1), and
is based In part upon the Information submitted by the Permittee. Any subsequent relocation,
reconstruction, or modifications to the access or change5 In the traffic volume or traffic nature
using the acc;ess shall be requested for by means of a new application. Any changes causing
non-compNanc:e with the Aaoess Code may render this permit void, requiring a new permit
5. The Permittee Is responsible for obtaining any necessary additional Federal, State and/or
City/County permits or clearances required for construction of the access. Approval of this
access permit does not constitute verification of this action by the Permittee. Permittee Is also
responsible for obtaining an necessary utility permibl In addition to this access permit.
8 . A Notice to Proceed. COOT Form 1265,1s required before beginning construction on the
access or any adlvlty within the highway Right-of-Way. To receive the Notice to Proceed the
applicant need& to submit a complete packet to COOT with the following items:
(a) A cover letter requesting a Notice to Proceed, and the intended date to begin constructk>n .
(b) Construction Plans in full compliance with the State Highway Acceas Code.
(c) Certificate of Insurance Liability as per Section 2.3(11 Xi) of the State Highway Access
Code.
(d) A certified Traffic Control Plan In accordance with Section 2.4(6) of the AcceSI Code.
7. No drainage from this site shall enter onto the State Highway travel lanes. The Permittee Is
required to maintain all drainage in excess of historical flows and time of concentration on site.
All existing drainage structures shall be extended ~ modified or upgraded. as applicable, to
accommodate all new construction and safety standards, In accordance with the Department's
standard specifications.
8. Nothing in this permit shall prohibit the chief engineer from exercising the right granted in CRS
43-3-1021ncluding but not limited to restricting left hand turns by construction of physical
medial separations.
9 . All workers within lhe State Highway right of way shall comply with their employe(s safety and

Administration (OSHA) regulations -including, but not limited to the applicable sections of 29
CFR Part 1910-Occupational Safety and Health Standards and 29 CFR Part 1926 -Safety
and Health Regulations for Construction. Personal protective equipment (e.g. head protection,
footwear, high vlsibHity apparel, safety glassaa, hearing protection, respirators, glove&, etc.)
shall be worn as appropriate for the work being performed. and as specified in regulation .
10. The pennittee is required to comply with the Americans Wittl Disabilities Act Accnslblllty
Guldetines (ADAAG) that have been adopted by the U.S. Architectural and Transportation
Barriers Compliance Board (Ac:ce&$ Board), and Incorporated by the U.S. Attorney Generales
a federal standard . These guidelines are defining traversable slope requirements end
prescribing the use. of a deftned pattern of truncated domes as detectable warnings at street
cmsstnga. The new Standards Plans and can be found on the DUign and Construction

11. When it is necessary to remove any highway right-of-way fence, the posts on either side of lha
access entrance shall be securely braced with approved end posts and In conformance with
the Oepartmenrs M-607-1 standard, before the fence Is cut, to prevent slacking of the
remaining fenc.e . All materials removed shall be returned to the Department.
12. Any damage to present highway facilities including traffic control devices shall be repaired
Immediately at no cost to the qepartment and prior to continuing other work. Any mud or other
material tracked or othefWise deposited on the roadway shall be removed dally or as ordered
by the Department lnspedor. Restoration shall meet the Department's standard specifications
for topsoil. fertilization, mulching. and re-seeding.
13. A fully executed complete copy of this pennit must be on lhe job site with the contractor at all
time5 during the construction. Failure to comply with this or any other construction
requirement may result In the Immediate suspension of work by order of the Department
inspector or the issuing authority.
14. Prior to any use as allowed by this permit, the applicant shall contact the Region 3 Access
Manager In writing to request a final Inspection. This request shall Include certlftcatlon that all
materials and construction have been completed In accordance with ell applicable Department
Standards and Specifications; end that the access is constructed In conformance wtth the
State Highway Acceaa Code, 2 CCR 601-1,1ncluding thla permit. The engineer of record as
Indicated on the constructton plans, shall be present for this inspection. The acceaa serviced
by this permit may not be opened to traffic until written approval has been given from the
COOT Access Manager.

A routine review of our open building permits indicates that we have not received a request for an inspection

Please be advised that building permits expire. per Uniform Building Code, section 106.4.4 and International
Code Section 105.5. "Every permit issued by the building official under the provisions of this code shall
expire by limitation and become null and void if the building or work authorized by such permit is not
commenced within 180 days from the date of such permit, or if the building or work authorized by such
permit is suspended or abandoned at any time after the work has commenced for a period of 180 days.
Before such work can be recommenced, a new permit shall first be obtained to do so, and the fee therefore
shall be one half the amount required for a new permit for such work, provided no changes have been made
or will be made in the original plans and specifications for such work, and provided further that such
suspension or abandonment has not exceeded one year. In order to renew action on a permit after expiration,
the permittee shall pay a new full permit fee ."
Please contact this office at your very earliest convenience, in writing, to advise us about the status of your
penni tted work.
If you have not responded within thirty days from the receipt of this notice the permit will be closed
and a permanent notation will be placed in the property records at the County Clerk and Recorders
office that indicates that the work permitted per Colorado Revised Statutes 30-28-205 & 210, was not
inspected, is not approved and that the building or area is illegal to occupy and is subject to legal
abatement action.

Building Permit, number o/~Stf , was due to expire, per section 106.4.4
& 5 of Uniform Building Code, and that you had been given an additional thirty days to
call for the required inspection or to request other action from the building department.
The above permit has expired and been closed. The work authorized by the permit has
not been inspected or approved. It is a violation of Colorado Revised Statutes 30-28-205
& 210, Uniform Building Code section 109, to use or occupy the building or area unless
final inspections have been completed and a Certificate of Occupancy has been issued .
Records of this failure to comply are included with property records at the Garfield
County Clerk and Recorders office. You are entitled to protest this action to the Garfield
County Board of Commissioners

Inspection of shop fabrication or field work at job site: Field
Report: Visually inspected the field welding at the mezzanine level bar
joists to steel beams. Verified joist bridging was installed between bar
joists. Visually inspected the high strength bolting at the main frame
column to rafter connections tightened using the tum of the nut method
with match marking.
The welding & bolting inspected above appeared to be as shown on
the drawings, the bolting appeared to be tightened using the tum of the
nut method as call out for on the drawings, and the welding met the
quality requirements of the A WS D 1.1 Structural Welding Code.

Inspection of shop fabrication or field work at job site: Field
Report: Visually inspected the field welding at the East end wall rafter
to colunm connection and light gauge metal framing at approximately
grid D/4. Visually inspected the high strength bolt tightening in process
at the mezzanine level steel connections using the turn of the nut method
with match marking. Tested some 5/8", 3/4", and 1" diameter sample
bolts that had been used in the main frame, end wall frames, and
mezzanine framing in a Skidmore-Wilhelm bolt tension testing device.
The steel erectors appeared to be properly tightening the bolts using
the tum of the nut method as call out for on the drawings, and the
welding inspected above appeared to be meet the quality requirements of
the A WS D 1.1 Structural Welding Code.

to column connection. Visually inspected the remainder of the high
strength bolting that had not been inspected at an earlier date at the main
frame rafter connections and end wall frame connections.
To the best of my knowledge this completes the special inspections
(provided the mezzanine level decking is not attached by welding) of the
structural field welding & high strength bolting as outlined in reports
lthru 3. To the best of my knowledge the structural field welding & high
strength bolting inspected as outlined in reports I thru 3 met the quality
requirements of the 2003 me, A WS D 1.1 Structural Welding Code, and
the AISC Specification for Structural Joints Using ASTM A325 or A490
Bolts.
