The onsite wastewater treatment system of reference was inspected on February 5, 2015. The
system was found to conform with the plans and specifications set forth in the engineered
sewage disposal system design for the aforementioned property prepared on January 28, 2015.
Swimming pools, ponds, or other water features were not included in the submitted or approved
design. Copies of the engineered plan are on file at Garfield County or may be obtained from the
undersigned design engineer.
