The following items are required by Garfield County for a final Inspection:
1} A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; aH
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors, walls finished , and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8} A fmal inspection sign off by the Garfield County Road & Bridge Department for driveway
instaHation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN

During the removal of fire damaged materials the roof framing was exposed. One of the trusses possibly
from the original construction had additional structural loads added to it in subsequent remodel work.
The truss at some point in time was altered to carry the additional loads. Since there were not any
records to verify if the truss had been altered structurally in a correct manner, the county inspector
requested that a structural engineer verify there was not an issue with the altered truss and that the
loads imposed were adequately supported.
Please call if you or your adjuster has any questions. Thank you for your cooperation to verify nothing
unsafe exists with the project.

Important that complete information be provided. Please review this document to determine if you have
adequate information to design your project and to facilitate a plan review.
SINGLE FAMILY OR DUPLEX BUILDING PERMIT APPUCATION CHECKLIST
This checkfist is to be used with all single famRy dwelling construction.
APPLICATION FORM: Please make sure the following information is provided on the application .
• Parcel number obtained from the County Assessor's Office.
• Job address (assigned physical address)
• Legal Description: lot, block, filing, subdivision or tract, section, township and range.
• Owners name, mailing address, phone, fax, or cell.
• Contactors name, mailing address, phone, fax, or cell.
• ArchitecVEngineer's name, mailing address , phone, fax or cell.
• Building size, height, number of stories, lot size.
• Use of building (residentiaQ
• Class of Work (new, addition, alteration, repair, demo, move, other)
• Description of work.
• Type of heating (natural gas , propane, electric, other).
• Sewer system (Community or ISDS), also see other items below .
• Garage (attached or detached).
• Driveway permit (please see other items below)
• Valuation (labor and materials) see attached worksheet.
OTHER ITEMS:
• If you anticipate obtaining a water-tap from the City of Rifle, please provide a letter indicating that
the City will provide water service. (Required to submit building permit application).
• ISDS requires a septic permit application to be submitted with the building permit application.
• A separate electrical permit must be obtained from the State of Colorado Electrical Board.
• Prior to submittal of a building permit , you are required to show proof of legal and adequate
access to the site. This may include proof of right to use a private easement, County Road &
Bridge permit, or Colorado Department of Highway permit, including a Notice to Proceed. The
County Road & Bridge Department can be reached at 970-625-8601.
• If you anticipate excavating or grading prior to issuance of a building permit, you will be required
to obtain a separate grading permit.
• If you belong to a Homeowners Association (HOA), it is your responsibility to obtain written
approval, if required.
• Provide copies of any Resolutions and/or Land Use Permits associated with this property.
• Additional Dwelling Units may require Administrative Review.
• Minimum dwelling dimension is 20 ft wide x 20 feet long.
• A Colorado State Licensed Electrician and Plumber must perform installation and hook-ups,
unless the homeowner is performing the work .
BUILDING PLANS : Two (2) sets of plans must be submitted. Plans must be 18"x24" minimum size,
complete, identical, legible, and to scale. All plans will be checked at the counter for completeness. If any
of the required information is missing , the building permit application cannot be accepted.
SITE PLAN: Please make sure the following information is provided on the site plan.
• · Property address/legal description.
• North Arrow and Scale on each page.
• Show all property lines, building envelopes , flood plain, and easements.
• Provide setback distances from boundaries, buildings, septic, well, and waterways.
• Location and direction of the county or private road and driveway accessing the property.
• Proposed and existing structures include sheds, barns, decks, patios, and any other buildings.
• Existing and proposed grade, positive drainage around structure (to be contained on site).
• Streams, rivers, creeks, springs, ponds and ditches.
• Existing and proposed wells, septic tanks, leach f~elds , and other systems. (if applicable)
• Retaining walls (engineer stamped detail required for walls over four (4) ft, measured from bottom
of footing to top of wall).
NOTE: Any site plan depicting the placement of any portion of the structure within fifty (50) feet of a
property line or not within an established building envelope shall be prepared, stamped , and signed by a
licensed surveyor. For structures to be built within a building envelope, provide a copy of the recorded
subdivision final plat with the proposed structure located in the building envelope .
ARCHITECTURAL PLANS: Please provide the following information on the architectural plans .
• Minimum four (4) elevations (N,S ,E,W) Indicate height of building measured from existing grade
to the midpoint between the ridge and eave of a gable or shed roof or to the top of a flat roof.
• Floor plans for each level including dimensions and scale noted.
• Clearly label each room or space (bedroom, bath, kitchen, closet, etc).
• Show the location of the mechanical equipment.
• Building cross sections with construction details .
• Window sizes, types, and operation noted on the floor plans or elevations.
• Specify roof slope/pitch, roof covering, and siding materials.
• Stair and guard details specifying rise, run , height and spacing.
• Attic, roof, and crawlspace ventilation details. Indicate size and location of ventilation openings.
• Minimum insulation shown for walls, floors, roofs/ceilings, slabs, basements and crawlspaces.
• Provide stove and fireplace make, model, EPA or Colorado Phase II Certification. Masonry
fireplaces shall be designed to the International Residential Code (IRC) requirements.
STRUCTURAL PLANS: Please provide the following information on the structural plans.
• Design specifications to include roof and deck snow load, wind and seismic design, frost depth.
• Foundation plan showing complete footing and foundation dimensions.
• Footing and foundation details specifying reinforcement and referenced on plans.
• Framing plans for each floor level and roof. All beams, columns, joists, rafters, and trusses
specified . Pre-manufactured structures requ ire the engineers stamp, signature, and date.
• Framing /connection details, braced wall lines, shear wall schedule, location and hold downs.
NOTE: If any required information is missing, delays in issuing the permit are to be expected. If
determined by the Building Official that additional information is necessary to review the application
and/or plans for compliance, the application may be placed on hold until the required information is
provided. Work may not proceed without the issuance of a permit.
The building department will collect a Plan Review fee at time of application submittal. The permit fee, as
well as any Septic or Road Impact fees will be collected when the permit is issued.
The permit application must be signed by the owner or by person having written authority from the owner
to act as their representative.
Building cannot be occupied until a Certificate of Occupancy (CO) is issued. Using the building without a
CO will be considered an illegal occupancy and may be grounds for vacating the premises. (Final/CO
inspection requirements will be attached to your approved field set of plans.)

EXECUTIVE SUMMARY
On January 6, 2011, Mr. Brandon Sinkbeil of OS Consulting, Inc. (DSC) performed a limited inspection and
asbestos bulk-sampling at 17 499 Old Highway 82 in Carbondale, Colorado, in order to identify potentially
hazardous friable and non-friable asbestos-containing materials (ACM) within portions of the above-
referenced, single family residence that was recently affected by a fire. The Colorado Department of Public
Health and Environment's (CDPHE) Regulation 8, Part B defines an asbestos-containing material {ACM) as
a material containing more than 1% asbestos.
Mr. Sinkbeil performed asbestos bulk-sampling of ceiling texture, drywall and joint compound (surfacing
materials) in a total of eleven (11) locations within the affected areas of the residence (see Section IV for
descriptions of sample locations). All eleven (11) bulk-samples were analyzed by Aerobiology & Associates,
Inc. laboratory in Lakewood, CO (NVLAP #200860) with all eleven (11) laboratory results reporting as
negative for asbestos (see Appendix 8 for laboratory results).

PROJECT OVERVIEW
1. Introduction
A limited inspection and bulk-sampling for ACM was conducted at 17499 Old Highway 82 in Carbondale,
Colorado, by Mr. Sinkbeil on January 6, 2011, at the request of Mr. Mike Hefner with Belfor Property
Restoration. Mr. Sinkbeil is a Colorado State Certified Building Inspector; having EPA Accreditation
#13442 (see Appendix A for certificate). The purpose of the inspection was to identify, sample and assess
potentially hazardous friable and non-friable ACM within the areas of the residence recently affected by a
fire.
II. Structural Design
The structure is a one-level ranch-style, wood-framed, single-family residence.
Ill. Sampling and Analytical Procedures
The inspection, assessment and sampling were conducted by an EPA and AHERA accredited Building
Inspector qualified by experience, education and training in the recognition of potential ACM and
approved bulk-sampling techniques. The asbestos bulk-sampling was conducted on suspect ACM with a
limited number of bulk-samples being collected within the residence.
The inspection, assessment and sampling were performed in accordance with Environmental Protection
Agency 1 AHERA recommended procedures. These procedures call for the visual inspection of the area of
concern and the collection and analysis of representative bulk-samples of suspect material.
Some minor destructive sampling was conducted. Walls, columns and perimeter pipe chases were not
broken into in order to locate and quantify suspect ACM. It should be noted that additional ACM might be
located in these and other inaccessible areas.
Random bulk-samples, representative of the suspect asbestos-containing building materials (ACBM) of
each homogeneous area (HA), were collected according to the guidelines published as Environmental
Protection Agency (EPA) Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15 USC, Sections
2641 through 2654 and in compliance with 40 CFR, Part 763. Representative sampling is based on the
following criteria:
1. The distribution of the suspect material throughout the HA.
2. The suspect material's physical characteristics and application.
3. Random sampling patterns determined for each HA.
Suspect materials sampled and analyzed should be considered to be representative of materials in each
HA if they exhibit similar physical characteristics and the application of the sampled material can be
correlated to the application of un-sampled material. Bulk-samples collected were analyzed utilizing the
EPA's Method for tbe Determination qfAsbestos jn Bulk Building Materials (EPA 600/R/116, July, 1993)
and the McCrone Research Institute's The Asbestos Particle Atlas as methods references. Analysis of the
bulk-samples was performed on the "date reported," as listed in the bulk-sample analysis report.

IV. Notes on Report Format
Suspect materials alike in appearance and application were sampled as HAs. Suspect materials were
divided into three classifications:
1. Surfacing Material: sprayed or troweled onto structural building member.
2. Thermal System Insulation {TSJ): any type of pipe, boiler, tank, or duct insulation.
3. Miscellaneous Material: other suspect materials, floor tile, sheet vinyl/linoleum, ceiling tiles,
insulation, and finishing materials.
Condition assessments were performed by the accredited inspector at the time of inspection. Condition
assessments are listed in the following section. Ratings of "good," "damaged," and "significantly
damaged" are meant to indicate the overall condition of the material.
A material in "good" condition has no visible damage or deterioration, or showing only very limited
damage or deterioration.
A material in "damaged" condition has the following characteristics: The surface is crumbling, blistered,
water·stained, gouged, marred or otherwise abraded over less than one·tenth of the surface if the
damage is evenly distributed (one·quarter if the damage is localized). Accumulation of powder, dust or
debris similar in appearance to the suspect material on surfaces beneath the material can be used as
confirmatory evidence. A material in "significantly damaged" condition has one or more of the following
characteristics:
• The surface is crumbling or blistered over at least one·tenth of the surface if the damage is evenly
distributed (one·quarter if the damage is localized).
• One-tenth (one·quarter, if localized) of the material is hanging from the surface, deteriorated, or
showing adhesive failure.
• Water stains, gouges, or mars are over at least one·tenth of the surface if the damage is evenly
distributed (one·quarter if the damage is localized). Accumulation of powder, dust or debris
similar in appearance to the suspect material on surfaces beneath the material can be used as
confirmatory evidence.
Response-action recommendations for asbestos·containing HAs are listed in the section VII.
Recommendations may be for more than one HA, if materials are alike. Recommendations are either
"general" or "immediate." An immediate recommendation indicates the presence of asbestos greater
than 1% within the bulk·sample, or a bulk·sample in the same HA, and should be addressed accordingly.
A general recommendation indicates asbestos does not exist greater than 1% within the bulk-sample, or
all bulk-samples in the same HA, and no further abatement activities are required for removal of the
material. Any sample reporting a "TRACE" amount of asbestos must be considered to be positive for
asbestos greater than 1% unless it is analyzed by the point-count method to be less than 1%.
V. Inspector Comments
No asbestos·containing material (ACM) was identified during the bulk-sampling conducted at 17499 Old
Highway 82 in Carbondale, Colorado. A total of eleven (11) bulk-samples were collected from ceiling
texture, drywall and joint compound in the above-referenced residence. All eleven (11) bulk-samples
tested negative for asbestos.

GENERAL:
The laboratory results of the potential ACM sampled at 17 499 Old Highway 82 in Carbondale, Colorado,
indicate that all eleven (11) bulk-samples collected from the main hallway, master bedroom/bathroom,
main hallway bathroom and dining room/hallway ceiling were reported as negative for asbestos.
Therefore, no professional abatement activities are recommended to remove the above-referenced

Bill did repairs and installation on walk in coolers, small commercial
coolers and freezers. He was responsible for the servicing and repair
of boilers pumps and all control systems . He was responsible for the
main punch list and start up monitoring of all HVAC system upon
opening of the Ritz Carlton . He also was responsible for the repair of
badly installed systems and getting contractors to warranty their work
as needed . Bill and the chief engineer John did set up a computer
monitoring system for all HVAC systems and a maintenance program
for every piece of equipment in the building .

• l Bill is the owner of Rocky Mt. HVAC LLC. Rocky Mt. just completed
the Elk Mt. Lodge project which was a 37,000 sq. ft. luxury home up
Castle Creek Rd . in Aspen Co. From 312007 until present day. Rocky
Mt. HVAC was responsible for installation of 12 separate forced air
systems with 12 commercial McQuay Units. These units are
equipped with hydronic heating coils and chilled water cooling coils.
This required more than 4000 linear feet of duct work . EML control
system: Individual touch screen logic panel systems in all 27 rooms
controlling separate heating, cooling, sound, commercial window
treatments, audio visual entertainment, and in-floor radiant heat
Rocky Mt. HVAC installed the master control system with April Aire
Controls and 27,000 feet of low voltage control wiring throughout the
lodge. The lodge systems are internet monitored by Rocky Mt
HVAC, thereby controlling its heat and cooling set points 24 hrs a day
7 days a week . Rocky Mt. also services all the Smiling Moose Deli's
in Colorado . Rocky Mt. services Hestia Restaurant in Carbondale,
Co . Just a few of our accounts. Rocky Mountain team of installation
and service technicians have all been with the company for almost 1 0
years with collective experience of more than 165 years . Each
employee has at least 15 years experience. Bill Wadsworth proudly

[1999-2000] Copeland Compressor-This school taught repair and
trouble shooting of all Copeland Compressors. We studied system diagnoses and replacement.
We also studied rebuilding of semihormatic compressors. This school was a hands on school
where we built and diagnosed Compressors troubles on test bench set up.
[May 2000) Scottsman Ice Machines-This seminar covered installation
and repairs of there new computer control unit. We were showed how to program these units as
well as retrieve problem codes and complete repairs; also reset units once serviced.
[2000-2003) Local Pipe Fitters Union 208-trade schools I completed 86
hrs. of refrigeration and boiler training (hands on classes) Please see certifiCate of completion
enclosed.
[March 2010] Daikin AC University-I completed the sky air installation,
commissioning and service school. This school covered trouble shooting, installation of sky air
VRV units. The school also went into piping and line set installation . They also covered wiring of
sky air and VRV units, start up and diagnosis.
[Aug. 2010] April Aire-This seminar was 3 days and they covered
installation, troubleshooting, of April Aire controls and system design. I also installed a 50
thermostat system at Elk Mt. Lodge in Aspen Co.

On July 5 , 2011 I visited the above referenced site to observe the structural framing affected by a
recent fire The interior drywal l had been removed from the ceiling and walls on most of the
s1rudure, and the exposed framing had been painted at the time of the visiL
J was told that the fire had started 1n the bedroom , so I obseNed the walls and roof trusses in this
area , but I did not find any signs of structural damage. I obsetved the remaining exposed waU
studs and roof trusses, and there was no sign of structural damage Some of the roof sheathing
had apparenlty been completely replaced in certain locations, but I was told that these areas were
reinforced due to previous moisture damage
In summary, I observed the exposed general structural fram•ng , but I did not find any evidence of
damage that would compromise the strudural1ntegnty.
This report is based on a visual observation without removing any fintsh materials, and the scope
of our &eMCe did not extend beyond the items discussed . Please do not hesitate to contact us
