As requested, structural steel special inspection was conducted per the IBC 2009 Edition - Chapter
17: Structural Tests and Special Inspection and Garfield County requirements. 1 arrived on site at 12:00
PM. 1 met with Mr. Marty Ruggles u ith Encana to review project requirements and high strength bolting
specifications. The pre -manufactured metal building fabricator for this project is Clear Creek Construction
in Oklahoma City, OK. The steel erector for this project is Clear Creek Construction in Oklahoma City,
OK. and also Elkhorn Construction in DeBeque. CO. Prior to my arrival I received and reviewed a copy of
an engineering clarification report dated 11-30-12 from Mr. Bill Rose P.E. with Group W Engineering. Inc
in Lakewood, CO referencing acceptance of the -tack' ri elds on the crane rail beams as referenced in my
10-26-12 report. Please refer to the attached report for project documentation.
Visual re -inspection was conducted at the Roof] raining Plan level on the repairs to the overhead
crane rail splice plates and bolts as referenced in my 10-26-12 report. Inspection was conducted at grid line
A - D & 6 - 9. Crane rail splice plates and bolts were observed to he properly installed.
