November 15, 2011 Our office is the structural engineer of record for the foundation improvements for the noted residence, and has provided structural foundation construction documents for the residence. Our office issued construction documents on October 04, 2011. The foundation improvements include a series of helical piers and helical piers installed along the lower level, walk -out side (northeast side) of the basement. Eight (8) helical piers and three (3) helical tiebacks were installed along the northeast side of the residence. An additional row of five (5) helical piers were installed parallel to the northeast wall to provide support for a future lower level patio and main level deck. The contractor for the project is Nathan Moffatt of Hill Builders, Inc. I reviewed Hill Builders, Inc. proposal for the work prior to commencement. I performed a site inspection on October 17, 2011 to observe on-going construction. I reviewed the boring log of pier and tieback installation as provided by Hill Builders, Inc., and compared the boring log to the exploratory boring log from the original site specific soils report (Report No,. 101-196-1 by Hepworth-Pawlak, Geotechnical, Inc. (04/13/2006)). Based upon that review, it is apparent that the helical piers all penetrated through the upper, fine grained soils, and are embedded in the dense gravels. It is my professional opinion that the foundation improvements have been completed as specified on our construction documents. Kurtz and Associates, Inc. Brian Kurtz, P.E,
