Dear Terry: As requested by Scott Green, Hepworth-Pawlak Geotechnical observed the installation of the On-site Wastewater Treatment System (OWTS) for the proposed residence and barn structure at the subject site. The services were supplemental to our agreement for professional services with you dated February 16, 2012. We previously designed the OWTS and presented our recommendations in a report dated July 16, 2012, Job No. 112 027B. The proposed construction is similar to that discussed in our design report and consists of a six bedroom residence and a barn structure with one bathroom. Our observations and recommendations of the As -Built construction are presented in this report. We performed our As -Built site evaluation on August 23, 2012. Scott Green Excavation performed the OWTS installation and we met with Scott Green during our site visit to discuss the system installation. The OWTS components we observed included: sewer lines, septic tank, cleanouts, effluent line, absorption trench excavations, Infiltrator Standard Quick4 chambers, serial distribution pipes, inspection ports, installed well location and the location of these components. Our As -Built OWTS site plan showing the locations of the components is provided as Figure 1, attached. The sewer pipes from the residence foundation and barn foundation to the septic tank inlet were backfilled prior to our arrival. It is important to not to heavily irrigate the areas around the absorption fields to prevent hydraulic overloading and possible premature failure of the absorption fields. The elevation change from the foundations down to the septic tank was approximately 5 to 10 feet. The cleanouts were installed adjacent to the building foundations and adjacent to the septic tank inlet. The 2,000 gallon, three -chamber Copeland Concrete septic tank was installed level at the location shown on Figure 1. The 4 inch non -perforated effluent pipe from the septic tank outlet to the first absorption trench was properly installed and sloped at 2% minimum down to the absorption trench. Our OWTS design report specified the absorption field area be excavated down approximately 3 feet below existing grade due to the near surface clay soils encountered during our subsoil exploration. Based on the subsoil conditions, the trench excavations were specified to be 3 feet from the established grade or approximately 6 feet in depth from the initial grade. The soils encountered below the predominantly shallow clay consisted of clayey sandy gravel. The absorption trenches were properly excavated down 5 to 6 feet from the natural ground surface grade and into the more gravelly soils. Our OWTS design specified an absorption field consisting of five trenches with 20 Infiltrator Quick4 standard chambers within each trench for a total of 100 chambers and 987 square feet of absorption area. The chambers were properly installed and level at the base of each absorption trench in the approximate location shown on the attached Figure 1. A slight change to the absorption field configuration was made by the contractor. Our design specified the trenches be connected with a manifold line. The trenches were connected using serial distribution pipes. The 4 inch non -perforated serial distribution pipes were properly installed and sloped at 2% minimum down to the adjacent trenches. The installed water well met the required setbacks to the OWTS components and was approximately 209 feet from the installed absorption field. It is our opinion that the OWTS meets the general intent of our July 16, 2012 design report. The change in the absorption area layout using serial distribution instead of the specified manifold pipe does not change the intent of our OWTS design and is acceptable.
