Please review this document to determine if you have adequate information to design your project and to facilitate a plan review. SINGLE FAMILY OR DUPLEX BUILDING PERMIT APPLICATION CHECKLIST This checklist is to be used with all single family dwelling construction. APPLICATION FORM: Please make sure the following information is provided on the application. Parcel number obtained from the County Assessor's Office. Job address (assigned physical address) Legal Description: lot, block, filing, subdivision or tract, section, township and range. Owners name, mailing address, phone, fax, or cell. Contactors name, mailing address, phone, fax, or cell. Architect/Engineer's name, mailing address, phone, fax or cell. Building size, height, number of stories, lot size. Use of building (residential) Class of Work (new, addition, alteration, repair, demo, move, other) Description of work. Type of heating (natural gas, propane, electric, other). Sewer system (Community or ISDS), also see other items below. Garage (attached or detached). Driveway permit (please see other items below) Valuation (labor and materials) see attached worksheet. OTHER ITEMS: ® If you anticipate obtaining a water -tap from the City of Rifle, please provide a letter indicating that the City will provide water service. (Required to submit building permit application). IISDS requires a septic permit application to be submitted with the building permit application. A separate electrical permit must be obtained from the State of Colorado Electrical Board. Prior to submittal of a building permit, you are required to show proof of legal and adequate access to the site. This may include proof of right to use a private easement, County Road & Bridge permit, or Colorado Department of Highway permit, includinuc a Notice to Proceed. Minimum dwelling dimension is 20 ft wide x 20 feet long. A Colorado State Licensed Electrician and Plumber must perform installation and hook-ups, unless the homeowner is performing the work. BUILDING PLANS: Two (2) sets of plans must be submitted. Plans must be 18"x24" minimum size, complete, identical, legible, and to scale. All plans will be checked at the counter for completeness. If any of the required information is missing, the building permit application cannot be accepted. SITE PLAN: Please make sure the following information is provided on the site plan. Property address/legal description. North Arrow and Scale on each page. Show all property lines, building envelopes, flood plain, and easements. Provide setback distances from boundaries, buildings, septic, well, and waterways. Location and direction of the county or private road and driveway accessing the property. Proposed and existing structures include sheds, barns, decks, patios, and any other buildings. Existing and proposed grade, positive drainage around structure (to be contained on site). Streams, rivers, creeks, springs, ponds and ditches. Existing and proposed wells, septic tanks, leach fields, and other systems. (if applicable) Retaining walls (engineer stamped detail required for walls over four (4) ft, measured from bottom of footing to top of wall). NOTE: Any site plan depicting the placement of any portion of the structure within fifty (50) feet of a property line or not within an established building envelope shall be prepared, stamped, and signed by a licensed surveyor. For structures to be built within a building envelope, provide a copy of the recorded subdivision final plat with the proposed structure located in the building envelope. ARCHITECTURAL PLANS: Please provide the following information on the architectural plans. II Minimum four (4) elevations (N,S,E,W) Indicate height of building measured from existing grade to the midpoint between the ridge and eave of a gable or shed roof or to the top of a flat roof. Floor plans for each level including dimensions and scale noted. Clearly label each room or space (bedroom, bath, kitchen, closet, etc). Show the location of the mechanical equipment. Building cross sections with construction details. Window sizes, types, and operation noted on the floor plans or elevations. Specify roof slope/pitch, roof covering, and siding materials. Stair and guard details specifying rise, run, height and spacing. Attic, roof, and crawlspace ventilation details. Indicate size and location of ventilation openings. Minimum insulation shown for walls, floors, roofs/ceilings, slabs, basements and crawlspaces. Provide stove and fireplace make, model, EPA or Colorado Phase II Certification. Masonry fireplaces shall be designed to the International Residential Code (IRC) requirements. STRUCTURAL PLANS: Please provide the following information on the structural plans. IDesign specifications to include roof and deck snow Toad, wind and seismic design, frost depth. Foundation plan showing complete footing and foundation dimensions. Footing and foundation details specifying reinforcement and referenced on plans. Framing plans for each floor level and roof. All beams, columns, joists, rafters, and trusses specified. Pre -manufactured structures require the engineers stamp, signature, and date. II Framing /connection details, braced wall lines, shear wall schedule, location and hold downs. I NOTE: If any required information is missing, delays in issuing the permit are to be expected. If determined by the Building Official that additional information is necessary to review the application and/or plans for compliance, the application may be placed on hold until the required information is provided. Work may not proceed without the issuance of a permit. The building department will collect a Plan Review fee at time of application submittal. The permit fee, as well as any Septic or Road Impact fees will be collected when the permit is issued. The permit application must be signed by the owner or by person having written authority from the owner to act as their representative. Building cannot be occupied until a Certificate of Occupancy (CO) is issued. Using the building without a CO will be considered an illegal occupancy and may be grounds for vacating the premises. We appreciate your wonderful house design and very thorough drawings, the BCR DRC have granted final approval with a few minor conditions below. 1. As we discussed, regarding thinning the willow against the pond, the DRC recommends that you contact some neighbors who may be impacted, specifically Brian Flynn - lot 16 and Carol Ingham - lot 22, prior to construction, to develop the discussion early on. 2. Per section 6.3 of the guidelines, please coordinate a pre -construction meeting with John Blatz, BCR HOA (379-3709) and Brian Flynn, BCR Property Manager (309-2425). 3. Asphalt shingle must comply with guidelines, specifically section 4.5(C)(5) as we discussed. 4. The findings of our observations and recommendations for the foundation design are presented in this report. The services were performed in accordance with our agreement for professional engineering services to Norwegian Wood Construction, dated July 27, 2011. We recently provided a letter dated July 29, 2011, providing recommendations based on a site visit on July 28th, We previously conducted a subsoil study for design of foundations at the site and presented our findings in a report dated February 11, 2009, Job No. 109 015A. We performed density testing of compacted on-site fill place on the building interior for support of slab -on -grade floors on August 2nd. Up to about 3 feet of fill had been placed. The results of tests taken at the final grade indicated dry densities of 120.8 and 124.6 pcf at moisture contents of 9.5% and 9.7 %. Based on an assumed standard Proctor value of about 128 pcf, the relative compaction of the fill is on the order of 94% to 97 %, suitable for support of floor slabs. At the time of our August 5th visit to the site, we observed that the foundation excavation had been cut in two main levels from 2 to 4 feet below the adjacent ground surface. The soils exposed in the bottom of the excavation consisted of firm, silty sandy gravel with cobbles. No free water was encountered in the excavation and the soils were moist. Interior wall and column footings had been excavated about 10 inches below the top of the recently placed compacted fill. Recommendations for placing footings on fill compacted to at least 100 % of the maximum standard Proctor density were provided in our original subsoil study report. Since the fill was compacted to about 95%, we recommended that the interior footing grade be lowered to bear on the underlying natural gravels. As an alternative, the interior footings could be redesigned I '.II1.CI LIH Norwegian Wood Construction August 6, 2011 Page 2 for a maximum allowable soil bearing pressure of 1,500 psf to reduce potential settlement of the compacted fill. We understand that the structural engineer, Dale Kaup, will provide recommendations for widening the column pad tootings, where necessary, so that the interior footings will have no more than 1,500 psf allowable soil hearing pressure. The natural gravel soils exposed in the exterior footing excavations are consistent with those previously encountered on the site and suitable for support of spread footings designed for the recommended allowable bearing pressure of 2,500 psf. Loose and disturbed soils should be compacted or removed in the footing areas to expose the undisturbed natural soils. The compacted fill on the building interior is suitable for support of footings designed for an allowable hearing pressure of 1,500 psf with a risk of future settlement. The differential settlement between the exterior and interior footings is expected to be about r/z inch. Other recommendations presented in our previous report which are applicable should also be observed. The recommendations submitted in this letter are based on our observation of the soils exposed within the foundation excavation and the previous limited subsurface exploration at the site. Variations in the subsurface conditions below the excavation could increase the risk of foundation movement. We should be advised of any variations encountered in the excavation conditions for possible changes to recommendations contained in this letter. Our services do not include determining the presence, prevention or possibility of mold or other biological contaminants (MOBC) developing in the future.
