As requested, MPA Consulting Engineers conducted a site inspection of the rebar placement for the floor slab
for the Middle Fork filter/manifold building on November 26, 2013. The inspection was limited to the floor
slab and interior spread footings prior to placement of concrete and included measurements taken on site to
verify dimensional compliance with the design drawings and visual inspection of the formwork.
The floor slab of the filter/manifold building is approximately 66 feet long by 45 feet wide, and the entire floor
slab is below grade approximately 3 feet to form a water containment system below the piping within the
building.
Dimensions and rebar placement were verified for the floor slab and interior footings to the extent possible.
Please refer to Appendix A for photos taken during the site inspection.
Dimensions were in compliance with the design drawings, and formwork appeared to be properly installed.
Inspection of the interior spread footing rebar and floor slab rebar verified the formwork and rebar placement
is in accordance to the design drawings and is acceptable.
