The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. All bathrooms must be complete, with washbowl, tub or shower, toilet stool, hot and cold
running water, non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all IBC and IRC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan
and it's discovered that required information has not been provided by the applicant, this will result
in the delay of the permit issuance and in proceeding with building construction. The owner or
contractor shall be required to provide this information before the plan review can proceed. Other
plans that are in line for review may be given attention before the new information may be reviewed
after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. Also, please consider
using a design professional for assistance in your design and a construction professional for
construction of your project. Any project with more than ten (10) occupants requires the plans
to be sealed by a Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following checklist prior to
and during design. Applicants are required to indicate appropriately and to submit the
completed checklist at time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and
drawn to scale.

Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with
decks, balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line
and original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re -bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house -rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind
speed, wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
1. Is a site plan included that identifies the location of the proposed structure or addition and
distances to the property lines from each corner of the proposed structure(s) prepared by a
licensed surveyor and has the surveyors signature and professional stamp on the drawing?
Properties with slopes of 30% or greater must be shown on the site plan. (NOTE
Section: 106.2) Any site plan for the placement of any portion of a structure within 50 ft.
of a property line and not within a previously surveyed building envelope on a
subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's
signature and professional stamp on the drawing. Any structure to be built within a
building envelope of a lot shown on a recorded subdivision plat shall include a copy of
the building envelope as it is shown on the final plat with the proposed structure located
within the envelope.

Notes: If you have answered "No" on any of the questions, you may be required to provide this
information at the request of the Building Official prior to beginning the plan review process.
Delays in issuing the permit are to be expected. Work may not proceed without the issuance of a
permit. If it is determined by the Building Official that additional information is necessary to review
the application and plans to determine minimum compliance with the adopted codes, the application
may be placed behind more recent applications for building permits in the review process and not
reviewed until required information has been provided and the application rotates again to first

Garfield County, Colorado, is hereby given by the Elk Springs Architectural Review Committee
(ARC) with the following conditions and exceptions:
1) The general contractor must meet with the Committee to obtain Contractor
Excavation Approval.
2) The light source of exterior lighting must not be visible from off the property. Light
fixtures must be no more than 10' above the plane illuminated and no more than 60
watts.
3) All disturbed areas must be re -vegetated. Permanently irrigated areas must not
exceed 3000 square feet.
4) The planting of 8' to 14' coniferous trees of varying height will be required for
screening. The specific number, locations and heights will be determined by the ARC
after construction and consultation with the owners.

9) Driveways must be surfaced with chip -seal, asphalt or concrete within one year from
date of occupancy.
10) Driveway lights must be 30' off the road and 20' apart, and no more than 24" high
and no more than 25 watts.
11) All wood burning fireplaces must be Colorado certified.
12) Complete engineering plans for ISDS need to be submitted prior to installation of
system along with engineers stamp and signature.
13) Variance request for dormer ridge run of approx. 3' above 24' height limit is
approved.
14) Any exterior lighting must be approved by the ARC prior to installation.
15) Staging and parking areas need to be defined.
16) Probably should consider a chimney instead of a wood stove flue. The black painted
flue could stick out of the roof approx. 8-9 feet and would be unstable without
installing an even more unsightly brace.
NOTE: This certificate of Architectural Approval is automatically revoked one year from the date
of issue if construction has not commenced. Extension must be sought 30 days prior to the

expiration date. The review of plans by the ARC does not cover compliance with local building
Codes or other authorities. Approval for conformance with building codes must be sought directly
from the local authorities.
This certificate of Architectural Approval is not effective until: 1) all conditions and exceptions are
acknowledged by the owner's signature; 2) the executed original is returned to the ARC and 3)
Contractor Excavation Approval is granted. A Certificate of Occupancy and a Certificate of
Compliance must be obtained within 18 months of the Effective Date written below.

Subject: Subsoil Study for Foundation Design, Proposed Residence, Lot 17. Los
Amigos, Garfield County, Colorado.
Dear Mr. Paausta:
As requested, Hepworth-Pawlik Geotechnical, Inc. performed a subsoil study and
percolation test for design of foundation and septic disposal system designs al the
subject site. The study was conducted in accordance with our agreement for
geotechnical engineering services Ill you dated February 7, 1995. The data obtained
and our recommendations based on the proposed construction and subsurface conditions
encountered ate presented in Ibis report.
Proposed Construction: The proposed residence will be a 2 story wood frame
structure above a walkout baseutent level and located ott dtc site as shown on Fig. 1.
Ground floor will be slab -on -grade. Cul depths are expected to range between about 3
to 8 feet. Foundation loadings for this type of construction art assumed to be relatively
light.
If (wilding conditions or foundation loadings are significantly different from those
described above, we should be notified to reevaluate the revv tendations presented in
this report.
Site Conditions: The site was vacant and covered with patdty snow at the lime of our
1ield work. The ground surface in the building area is moderately sloping down to the
southwest with about 10 fat of elevation difference across the building footprint. To
the west of the building area is a small ravine. Vegetation consists of a pima and
juniper forest with undcrslory of grass, weeds and sage brush.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
excavating 2 exploratory pits at the approximate locations shown on 19g. 1. The logs of
the pits are presented on Fig. 2. The subsoils encountered, below about 6 to 10 inches
of topsoil. consist of basalt gravel cobbles and boulders in a sandy silt matrix to a
depth of 7 feet in the building area and gravelly sandy clay to tate pit depth of 4 feet in
the driveway area. At Pit 1 lava now ruck appeared to be encountered at 7 feet where
Were was practical refusal to backhoe digging. Results of swell -consolidation testing
perforated on a relatively undisturbed sample of the sandy clay from Pit 2, pressed on
Fig. 3, indicate low compressibility under existing moisture conditiotls and light loading
and a low expansion potential when wetted. itesults of plasticity testing performed on
the clay front Pit 2 showed a low to medium plasticity. No free water was observed in
the pits al the time of excavation and the soils were slightly moist

Foundation Reoousmendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature of the proposed construction, we reconuncnd spread
footings placal on the undisturbed natural soil designed for an allowable soil bearing
pressure of 2,000 psf for support of the proposal resideuc . The bearing soils will vary
and there could be some post construction settlement If the bearing soils become wetted.
Footings should be a minimus width of 16 inches for continuous walls and 2 feet for
columns. The excavation bottom will likely he ragged due to the basalt luck. Loose
aunt disturbed soils and encountered al the foundation bearing level within the
excavation should be moistened and compacted or removal and the fooling bearing
kvel extended down to Uto undisturbed natural soils. Exterior footings should be
provided with adequate cover above their bearing elevations for frost protection.
Placement of footings at least 36 inches below the exterior grade is typically used in this
arca. Continuous foundation walls should be reinforced top and bottom to span local
anomalies such as by assuming an unsupported length of al least 12 feet. Foundation
walls acting as retaining structures should be designed to resist a lateral earth pressure
based on an equivalent lluld unit weight of at least 50 pcf for the on-site soil as backfill
excluding oversized rock.
Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support
tightly to loaded slab -on -grade construction. To reduce the effects of some differential
movement, floor slabs should be separated from all bearing walls and columns with
expansion joints which allow unrestrained vertical movement. Flour slab control johns
should be used to reduce damage due to shrinkage cracking. The requirements fur joint
spacing and slab reinforcement should be established by the designer basal on
experience and the attended slab use. A minimum 4 -inch layer of free -draining gravel
should be placed beneath basement level slabs to facilitate drainage. lids material
should consist of minus 2 -inch aggregate with less than 50% passing the No. 4 sieve
and less than 2% passing the No. 200 sieve.
All fill materials for support of floor slabs should be compacted to at least 95 % of
maximum standard Proctor density at a moisture content near optimum. Required 1111
can consist of the on-site soils devoid of vegetation, topsoil and oversized rock.
Underdrain System: Although free water was not encountered during our exploration,
it has been our experience in mountainous areas that local perched groundwater may
develop during limes of heavy prceipilatkur or seasonal runoff. Proven ground during
spring runoff an create a perched condition. We recommend below grade
construction, such as retaining walls and basement areas, be protected from wetting and
hydrostatic pressure buildup by an underdrain system.

The drabs should consist of drainpipe placed in the bottom of the wall backflt
surrounded above tate invert level with free -draining granular material. The drain
should be placed at each level of excavation and at least 1 foot below lowest adjacent
finish grade and sloped at a minimum 1% to a suitable gravity outlet. Free -draining
granular material used in the uudcrdrain system should contain less than 2% passing the
No. 200 sieve. less flan 50% passing the No. 4 sieve and have a maximum sine of
2 inches. The drain gravel backlit should he at least 1 1R feet deep.
Surface Drainage: The following drainage precautions sltoutd be observed during
construction and maintained at all times after lite residence has been completed:
1) Inundation of the foundation excavations and underslab areas should be avoided
during construction.
2) Exterior backlit! should be adjusted to near optimum moisture and compacted to
at least 95% of the maximum standard Proctor density in pavement and slab
areas and to at least 90% of the maximum standard Proctor density in landscape
areas. Free -draining wall backfill should be capped with at least 2 feet of the
on-site, finer graded soils to reduce surface water infiltration.
3) The ground surface surrounding the exterior of the building should be sloped to
drain away frau the foundation in all directions. We reconuncnd a utinit®n+
slope of 12 inches in Ile first 10 feet in unpaved areas and a minimum slope of
3 inches lit the first 10 feet itt pavement and walkway areas. A swale will be
needed uphill to direct surface runoff around the residence.
4) Roof downspouts and drains should discharge well beyond the limits of all
bacitfihl.
Percolation Testing: Percolation tests were conducted on February 10. 1995 to
evaluate the feasibility of an htfillratitm septic disposal system at the site. One Inutile
pit and 3 percolation holes wctc dug at locations as shown on Pig. 1. The test butes
were hand dug at the button of shallow backhoe pits and were soaked with water. The
soils exposed la the percolation holes arc similar to those exposed in the Profile Pit
shown on fig. 2 and consisted of basalt rock fragments in a sandy silt matrix. The
percolation lest rates varied from 7 to 30 minutes per inch. The site appears suitable
for a conventional infiltration septic disposal system. 'the selected disposal arca airuuld
be investigated for adequate soil depth below the disposal level at the time of
construction. A civil engineer should design the infeluatiun septic disposal system.

the project evolves, we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations, and to
verify that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We reconunend un -site observation of excavations and foundation
bearing strata and testing of structural 1111 by a representative of the soil engineer.
