The following items are required by Garfield County for a final Inspection:
1) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; ail
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors , walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O .) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN
ILLEGAL OCCUPANCY AND MAY BE GROUNDS FOR VACATING PREMISES

anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation, sheeting, house-rap,
(which is required), siding or any approved building material. Engineered foundations may be required.

A window schedule. A door schedule. A floor framing plan, a roofing framing plan, roof must be designed to
withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. windspeed, wind exposure B
or C, and a 36 inch frost depth.
All sheets need to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be incompliance with the 2003 IBC, !PC, IMC, IFGC, and IFC.
Applicants are required to indicate appropriately and to submit completed checklist at time of
application for a permit:
I. Is a site plan included that identifies the location of the proposed structure, additions or other buildings,
setback easements, and utility easements showing distances to the property lines from each comer of
the proposed structure prepared by a licensed surveyor and has the surveyors signature and
professional stamp on the drawing? Slopes of 30% or more on properties must be show on site plan.
(NOTE: Section 106.2) Any site plan for the placement of any portion of a structure within 50 ft. of a
property line and not within a previously surveyed building envelope on a subdivision final plat shall
be prepared by a licensed surveyor and have the surveyors signature and professional stamp on the
drawing. Any structure to be built within a building envelope of a lot shown on a recorded
subdivision plat, shall include a copy of the building envelope as it is shown on the final plat with the
proposed structure located within the envelope.

JOINTS NOT SUBJECT TO TENSION LOADS
JOINTS NOT SUBJECT TO TENSION LOADS need only be lightened
to snug tight condition. This is defined as the
tightness attained by a few impacts of an impact wrench or
the full effort of a man using an ordinary spud wrench.
JOINTS SUBJECT TO TENSION LOADS
Two tightening procedures are specified for A325 Bolts in
joints subject to tension loads, Turn-of-the-Nut Method or·
Direct Tension Indicator.
Turn-of-the-Nut-Method -First bring bolts to a 'snug tight"
condition to ensure that the parts of the joints are brought
into a good contact with each other. Next, place bolts in
all remaining bolt holes and bring to • snug tight'. Then
additionally lighten all bolts by tlie table above -
progressing from the bolts nearest the web, until the bolts
have been tightened to applicable nut rotation. During this
operation there shall be no rotation of the part not turned
by the wrench.
Consult the latest edition of the AISC Manual of Steel
Construction for more complete instructions for installing
high strength bolts.

Sheet walls from left to right.
Begin sheeting walls by centering the middle of the major rib
of the first panel on steel line at the left side of the wall.
Plumb the first sheet and fasten in place. Mark 3' -0" from
the end of the first sheet to where the edge of the next sheet
should terminate. When installing the next sheet, stretch or
compress the panel to hold the edge of the sheet on the mark you
have just made to keep the major ribs on one foot increments.

3. Anchor b olts shall be ac curately set to a tolerance of + /-1 /8" in both elev ation and location.
4. Column bose plates ore designed not to exceed o bearing pressure of 1050 pounds per square inch.
5. All walk doors ore to be field located with drilled in wedge anchors .
6. Total anchor bolt length is embedment + projection. Bend length is not included in total length.
7 . All bent anchor bolts to be ASTM A36 threaded round stock (rolled or cut thread) with plain
finish, unless noted .
8 . When anchor rods ore located close to the free edge of the concrete, the tensile and/or shear
strength of the anchor rods ca nnot be fully developed. In these cases, steel reinforcement in
the concrete must be sized and developed (not by Sentinel) for the required tensile or shear
forces in the anchor rods .
9 . For bu ildings classified os closed (Closed/Open = C), Sentinel Build ing Sys tem s does not
inve s tigate t he influence of the design wind load on the metal building sys tem with d oors and
windows left open. The end customer must assure that during times of high wind s oil o verhead
doors, sliding doors, Iorge doors (such as bi-fold & hydraulic), walk doors and windows will be
closed & latched .

Report: Visually inspected the high strength bolting at the main frame & end
wall frame column & rafter connections.
Tested some random samples of the high strength bolts used in the main
frame column & rafter connections using a Skidmore Wilhelm bolt tension
testing device to determine the testing torque required for the high strength
bolts. After testing torque was determined a minimum of two bolts per
connection were tested for proper tightening at the main frame column &
rafter connections.
The high strength bolting inspected above appeared to be in conformance
with the drawings for bolt size & grade, and appeared to be sufficiently
tightened. Flange & cable bracing was also installed.
To the best of my knowledge this completes the special inspections of the
high strength bolting & steel frame details as outlined above.

The following commentary refers to only to the hair pins designed by Engineering
and Technical Services, and to be built by TallyHo Construction.
The continuous hair pins may be replaced by a three piece unit consisting of two
standard threaded rods and one form saver of the same or larger diameter as the original
hair pins. The standard threaded rods shall be long enough to develop the full strength of
the 60 grade steel reinforcing to which they will be lap spliced to. Also the total length of
each leg of the new three piece unit shall be the same or greater than that originllllength
of the original hair pin.

2.0 DESCRIPTION
2.1 LENTON Couplers:
2.1.1 Gener•l: The LENTON system, consi,ting of the
LENTON couplers and rebar. is a lapered threaded splice
designed for use in relnlorced concrete conslructiQn. Devices
are &vailable in seven bask: styles: standard (A2),
TRANSITION (A2), FORM SAVER (SA or FS), WELOEO
HALF COUPLER(C3J),INTERLOK(LK), POSITION (P9)and
TERMINATOR (08). LEN.TON couplers are designed ttJ
mechanically butt-spliCe No. 4 (13) through No. 18 (57)
relnfoi'Qng steel (rebar) conforming Ia ASTM A 615 Grade 60
and ASTM A 706 specmcallons. LENTON Standard dellices
also are designed Ia mechanically butt-splice No. 9 (29)
through No. 18(57)rebarsconformlngtoASTMA615,Grade
75. Rebarmaybegalvani>:ed in accordance with ASTMA 767
or epoxy-coaled in accordance with ASTM A 775 or ASTM A
934 when utilized with the LENTON Standard (A2).

(06), WELDED HALF COUPLER (C3J), and POSITION (P).
All stylee have lnterlOr-tapered lhreads for joining the
reinforcing bars.
The coupl""' are manufaclured from steels listed in Figure
1.
The threads an the rebar are right-handed and tapered to
match the accompanying coupler. Threading is done at
locaUons approved by ERICO, Inc. Quality control checks are
performed at the threading location using gages supplied by
ERJCO, Inc. The bar-end gage controla the end diameter,
which automatically controls the thread length. The profile
gage determines the correct proftle and depth. Prior to quality
control checks, all threads are viaually Inspected for
cleanllnase and damage. Damaged threads ere removed by
saw cutting, and the bar Is then remachined using the
LENTON bar threader. Prior to shipping from the rebar
fabrication shop, the threads are protected to ensure
damage-free bar ends allhe jobs«e.
2.1.2 Design: The LENTON systems comply as tension or
compression splices for deformed reinforeing bars as
specified in Section 1912.14.3.4 of the 1997 Uniform Building
Code'" (UBC) and Sectlon21.2.8.1 of ACI318-99 [ACI 318·
99 Is referenced in 2000 International Building Cod .. (IBC)
Section 1901 and 2000 International RvsirlenOal Code• (IRC)
Section R812). The splice devices also comply with the Type
1 and Type 2 mechanical spRee requirements of Section
1921.2.6.1 ofthe UBC and Section 21.2.6.1 of ACI 318-99
(IBC and IRC). Tlle Terminator is designed as anchorage In
accotdance with Secllon 1913 of the IBC.
2.1.3 Installation:
2. 1.3.1 General: The LENTON couplers must be lnstalle~ in
accordance with the applicable code. this evaluation report,
and the ERICO Installation lnslruetions. The splice locations
must be detailed on the plans approved by the building
official. All required distances, spaCings and coverages
described In Secllons 1907.6 and 1907.7 and Table 7-A of
the UBC or Sections 7.6 and 7.7 of ACI 318-99 and Table
719.11(1) of the IBC must be measured from the oul$ide of
the sleeves. When bars act in compression only, the
connecting device may project 1/2 Inch (12.7 mm) lnlo the
required cover. However, In no case may lhe cover be less
than '1, inch (19.1 mm). As Type 2 splices, the couplers are
permitted in any location within a member for all seismic
zones or seismic design categories.
2.1.32 LENTON Standard Coupler (A2): The LENTON
Standard coupler Is used to join straight bars Where at leasl
ono rebar end is free Ia rotate. For field installation of lhe
Standard coupler, the thread protector is removed lrom the
threaded bar end, which is Inspected for cleanliness and
damage.ln some cases, the coupler is fastened Ia the bar al
the fabrication facility, to protect the threads. A Wire brush Is
used to remove rust and adhered conc"'te. Tlle coupler is
lhen 8Crewed onttJ the threaded end of a bar to be spliced,
and is Oghtened by hand. The second bar is inserte~ into I he
coupler and is rotated until hand-tight. A LENTON inspection
wrench secures the connecuon to the required seHing lisled
In Figure 2: a "click" in lhe handle can be le~. as wull as
heard, When the desired torque has been attained.
2.1.3.3 LENTON TRANSITION Couplers (A2): LENTON
TRANSITION couplers ate similar to the Standard coupler,

setting listed in Figure 2.
2.1.3.4 LENTON POSITION Couplers (P9): LENTON
POSITION couplers ar,. used to join curved or bent bars that
must be held in a predetermined position during the joining
process. The couplers ""n also be used where neither bar is
free to rol&te. All POSITION couplers are manufactured to
allow only the wupler lo rotate. Installation difrerlo frvm that of
all other LENTON wupler~~. Figure 5 shows installation
procedures.
2.1.3.5 LI!NTON TERMINATOR (08): LENTON
TERMINATORS are embedment-style coupler devices used
as an alternative to bent sections of reinfordng sleet in
reinforced concrete construction. The deVIce Is m&chined with
a LENTON tapered thread. The bearing cross-sectional area
is at least four times the nominal area of the reinforcement
bars. Installation of the device ID the rebar is done using a
LENTON Inspection wr~~nch. The connection is tightened lo
the approp~ate setting listed in Figure 3.
2.1.3.6 LENTON FORM SAVER (SA or FS): The LENTON
FORM SAVER SA or FS has a factory-installed mOIJnting
plate swaged to the LENTON coupler. Also, an internal thread
protector Ia Installed to protect the LENTON threads. The
FORM SAVER FS styleassembliesare produced by a friction
forging process, whereby the rvinfuncing steel Is attached to
the one end of the coupler by forcing the compOnents
together while the coupler Is revolving at a specifiC rate. The
other end of FORM SAVER FS accepts rebar with LENTON
tapered threadS. The friction weld quality is verified by
statisfical sampling of asoemblies. The FORM SAVER SA
style couplers accept rebar with LENTON tapered threads at
each end. To install the FORM SAVER (SA or FS) assembly,
the mounting plate rney be used to pO&ilion the coupler
against the form. Upon removal of the framework, the
protectors are removed from both the rebar and the coupler.
The threaded rebar is screwed into the exposed end of the
FORM SAVER SA. The connectlon is then tightened to the
appropriate setting listed In Figure 2.
2.1.3. 7 Welded HALF COUPLER (C3J): A LENTON HALF
COUPLER proVIdes a mechanical means of connecting .
reinforcing rebars to structural steel plates and ah$pes. One
end of the coupler Is machined to have LENTON standard
lntemal taper threads. Tha opposite side is prepored for
welding. Refer to the current AWS 01.1 Welding Code for
required welding procedures. The connOICiion Is then
tightened to the appropriate setting listed in F'I!Jure 3.
2.1.3.8 LENTON tNTERLOK (LK): The LENTON
INTERLOK system is designed to splice No. 61hrough No. 14
rebans. The sleaves are cast frvm ductlle iron complying with
ASTM A 536. The sleaves are streight cylinders wtth one end
threaded to receive threaded reber and the opposite end
having internal annular ridges spaced approximately 1 inch
(25.4 mm) on center. A typical splicinp systi!RI is shown in
Figure 4. The LENTON tNTERlOK spl1clng system must be
Installed In accordance With the applicable code, the
evaluafton report. and the ERICO Installation instructions. The
system consists of a splice sleeve and ERICO HY10L grout.
which is a non-sMnk, cementitlous mixture. The threaded
end of the sleeve hes LENTON tepered threads. The coupler
Is screwed onto the threaded end of a rvbar. and is tightened
by hand. A wrench is used to complete the LENTON threaded
connection. The connection is then Ughtened to the
appropriate setung listed In Figure 3.
The opposilu end of the sleeve is open to receive the
reinforcing steel of the adjoining precast structural member or
projecting dowel from the foundation. The connection Is
subsequenUy completed by pouring or pumping HY10L grout
into the Interior of the sleeve. Batches of ERICO HY10L grout

are miXed in accordance With the ERICO installaUcn
instructions. The grout is packaged in 50-pound (23 kg) bags.
Each 50. pound (23 kg) bag of dry grout is slowly blended with
approximately 90 ounces (2. 7 L) of potable water and
thoroughly stirred with a mechanical mixer. The correct
amount of water to be added to the grout Is predetermined by
field-testing the now of trial batches of grout mixtures with a
2~nch-dlameter (51 mm), 4-lnch-tall ( 102 mm) cylinder and a
LENTON INTERLOK flow template. The grout must now to e
diameter of no greater than 7 Inches (178 mm) on the now
template. Grout mix cannot be retempered. The minimum
compressive strength of 2-inch (51 mm) cubes of the grout at
28 days. when tested In accordance with ASTM C 109.02.
must be 8,500 psi (58,600 kPa). Compression test specimens
must be cured in accordance wnh ASTM C 109-02. Mixed
ERICO HY10L grout can b11 poured or pumped Into the
sleeve. All spaces within the sleeve must be fully grouted.
2.2 Spaciallnopectlon:
Special inspection is required in accordance With UBC
Section 1701 or IBC SecUon 1704, as applicable. The
lnspOICior's duUes include aempling of grout compressive
strength specimens. and verifying grads and size of
reinforcement bars. coupler and sleeve identification, thread
quality of bars, position of couplers and sleeves. InstallatiOn
of couplers end sleeves to bars, and welding of the
WELDABLE HALF COUPLER.
2.3 tdentifieation:
All devices are packaged with labels bearing the
manufacturer's name (ERICO, lne.) and address. model, sl:.e,
and evaluation report number (ER-3987). Each LENTON
coupler Is permanently stamped with the catalog number,
alze. heat number, Type 2 designation, and ths name
'LENTON." A shipping tag is attached to each bundle, pallet
or skid of each LENTON FORM SAVER shipment, and bears
the manufacturer's name (ERICO, Inc.). the pt~rl number and
the name of the inspaclion agency. Each LENTON
INTERLOK sleeve is identified with the rebar sio:e, part
number. Type 2 designation. and manufacturer's name
(ERICO. Inc.). Cartons or sleeves are labeled wtth the·
manufacturer's name (ERICO, Inc.) and address. Each bag
of HY10L grout is Imprinted with the manufacturer's name
(ERICO, Inc.) and address. product designaUon, mixing
instructions, and evaluation report number (ER-3967).
