Please Note: The following submittal requirements apply to any proposed amended plat that does not
have an approved Preliminary Plan to verify the consistency with the proposed amended plat OR that
results in the relocation of property lines between more than two (2) adjacent properties.
SUBMITTAL REQUIREMENTS:
1) Copy of the deed showing ownership in the applicant, or a letter from the property owner(s), if
other than the applicant;
2) Names and addresses of owners of record of land immediately adjoining and within two
hundred feet (200') of the proposed amended plat, mineral owners and lessees of mineral
owners of record of the property to be a part of the amended plat, and tenants of any structure
proposed for conversion;
3) Evidence of the soil types and characteristics of each type;
4) Proof of legal and adequate source of domestic water for each lot created, method of sewage
disposal, and letter of approval of fire protection plan from appropriate fire district;
5) If connection to a community or municipal water or sewer system is proposed, a letter from the
governing body stating a willingness to serve;
6) Narrative explaining why amended plat is proposed.
The consideration of this amended plat, if applicable to the submittal requirements stated above, will
require at least one (1) public hearing for which public notice must be provided. The Planning
Department will mail you information concerning this hearing(s), approximately 45 days prior to the
scheduled hearing. You will then be required to notify, by certified return receipt mail, all adjacent
landowners and publish the notice provided by the Planning Department, in a newspaper of general
circulation. Both of these notices must be mailed/ published at least 30 days prior to the public hearing.

1.) Copy of deed attached.
2.) Names and addresses of property owners within 200 feet attached. There is no
separate ownership of mineral rights on the property. There are no structures on the
property, and no tenants.
3.) Copy of relevant soil survey map attached, along with description of Atencio-Azeltine
soil complex.
4.) Town of Carbondale Water Tap Authorization is attached. Sewage disposal will be by
ISDS, and Garfield County ISDS Permit has been applied for. Letter of approval from
Carbondale & Rural Fire Protection District is attached.
5.) Town of Carbondale Water Tap Authorization is attached.
6.) The purpose of the amended plat is to combine three existing lots, which are 25 feet
wide and were platted in the 1880's, into one building lot for a single family residence.

to remove the dividing lines between lots 10-11-12 Block 4 Town site of Cooperton.

This is a residential neighborhood It is not a commercial zoning. I wish to keep
it that way.
We live on a private road and do not want the commercial traffic that having a
business in the neighborhood would create. If this person wants to run a business then let
her run it out of an office in an office building that is for that purpose and have more
consideration for the people who live in this neighborhood.

Soil Survey Area: 655 ASPEN -GYPSUM AREA, COLORADO, PARTS OF
Map unit: 13 Atencio-Azeltine complex, 3 to 6 percent slopes
Description Category: SOI
This map unit is on alluvial fans and terraces. This unit is 60 percent Atencio sandy
loam and 30 percent Azeltine gravelly sandy loam.
The Atencio soil is deep and well -drained. It formed in alluvium derived dominantly
from sandstone and shale. The surface layer is sandy loam 10 inches thick. The
subsoil is sandy clay loam 10 inches thick over gravelly sandy loam 4 inches thick. The
u
pper 6 inches of the substratum is gravelly sandy loam. The lower part to a depth of 60
inches is very gravelly sand. In some areas the surface layer is gravelly or cobbly.
Permeability is moderate to a depth of 30 inches and rapid below this depth. Av
ailable water capacity is low. Effective rooting depth is 60 inches or more. Runoff is
slow, and the hazard of water erosion is slight to moderate.
The Azeltine soil is deep and well -drained. It formed in alluvium derived dominantly
from sandstone and shale. Permeability is moderate in upper part, and rapid below a
depth of 16 inches. Available water capacity is very low. Effective rooting depth i
s 60 inches or more. Runoff is slow, and the hazard of water erosion is slight to
moderate.

This authorization, along with the associated fees and worksheet, shall serve as the
necessary approval for connection to or increased use on the Town of Carbondale Water
and/or Wastewater system.
Conditions of Approval:
The undersigned certifies that the information contained on the application worksheet
constitutes to their best knowledge a complete description of the facilities and intended
uses for the property described on the application worksheet. The applicant agrees to
conform to all provisions of the Carbondale Municipal Code Standards and
Specifications, as adopted, pertaining to the consumption of water and the discharge of
any material into the Carbondale sewer as may be amended from time to time. It is
understood that the Town of Carbondale reserves the right to inspect the premises for
which this water and/or sewer tap is intended and to adjust the water and/or sewer tap fee
for any discrepancy between the actual use or structure served and the information
contained in the application. The undersigned further agrees to contact and advise the

Dear Mark:
The Nancy Smith property at Mesa Avenue in Carbondale is located inside the service area of
the Carbondale & Rural Fire Protection District. The District services the property for both fire
and emergency medical services.
It is my understanding that the existing water main that serves the area will be extended to serve
the property. An additional fire hydrant will be installed on the extended water main. This will
provide an adequate supply of water for fire protection to the property.
