Permit No. 5363 issued on November 16, 1994. Dear Mr. Guarini: As of this date the project referred to above has not had a final inspection and approval by this department, I was notified that Mountain Builders will no longer be the general contractor on the project and I will need a new signature on the application prior to resuming construction. The • last recorded inspection on this project was an insulation inspection on December 20. 1994. He would like to do this when he returns from Florida in August of 1995, I told Mr. Guarini that I will be willing to issue a certificate of occupancy for the use of the building as originally permitted after receiving verification of approval by the State Electrical Inspector, and upon his continuance in applying for and receiving a Special Use Permit and a Building Permit for the building in it's present configuration. 1 have drafted the attached agreement that I believe to be consistant with our conversations on Thursday, March 30, 1995.
