NEW CONSTRUCTION
ADDITIONS
ALTERATIONS
and
MOVED BUILDINGS
In order to understand the scope of the work intended under a permit application and expedite
the issuance of a permit it is important that complete information be provided. Adequate and
complete information will prevent delays in the plan review process. Reviewing a plan and the
discovery that required information has not been provided by the applicant may result in the
delay of the permit issuance and in proceeding with building construction. The owner or
contractor may be required to provide this information before the plan review may proceed. This
causes delays because other plans that are in line for review may be given attention before the
new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. If you do not, it may
be helpful to obtain a book titled "Dwelling Construction Under the Uniform Building
Code". This book is available to you through this department at our cost. Also, please
consider using a design professional for assistance in your design and a construction
professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following checklist prior
to and during design.

Plans to be included for a Building Permit must be on draft paper at least 18"x 24"" and
drawn to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with
decks, balcony steps, hand rails and guard rails, windows and doors, including the finish grade
line. A section showing in detail, from the bottom of the footing to the top of the roof, including
re -bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house -rap, (which is required), siding or any approved building material.
A window schedule. A door schedule.
A floor framing plan, a roofing framing plan, roof must be designed to withstand a 40 pound per
square foot. up to 7,000 feet in elevation, an 80 M.P.H. windshear, wind exposure B, windload of
15 pounds per square foot and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 1997 UBC, UMC and 1997 UPC.
Applicants are required to indicate appropriately and to submit completed checklist at
time of application for a permit:
1. Is a site plan included that identifies the location of the proposed structure, additions or
other buildings, setback easements, and utility easements showing distances to the
property lines from each corner of the proposed structure prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Slopes of 30%
or more on properties must be show on site plan. NOTE: 106.3.1 (7.) Any site plan for
the placement of any portion of a structure within 50 ft. of a property line and not within
a previously surveyed building envelope on a subdivision final plat shall be prepared by a
licensed surveyor and have the surveyors signature and professional stamp on the
drawing. Any structure to be built within a building envelope of a lot shown on a
recorded subdivision plat, shall include a copy of the building envelope as it is shown on
the finlat with the proposed structure located within the envelope.

Dear Ms. Drussel:
Thank you for your submittal of the site plans last Friday. I have continued with the review of the
building permits. Upon review of the application for the 4-plex at 63 Angelica, several areas will need
to addressed. These areas are as follows:
1. My copies of the plans are not sealed by a licensed architect. The structural drawings are
stamped but the architectural drawings are not. Sealed plans are required.
2. As per Sec. 1103.1.9.3 of the code, multi -unit buildings containing four or more dwellings must
be built to the accessibility requirements for a Type B dwelling. This allows for the building to
be properly sized and adaptable at construction without having to provide all the accessible
features required of a Type A building. This requirement is not included with the submitted
plans.
3. I have spoken with Brian Sims regarding the 1 hr. door requirement and the required change to
the foundation. Both these requirements are redlined on the plans submitted. The required
change to the foundation has been made. The door requirement can be checked in the field.
4. Also redlined on the plans, are the requirements for the engineer of record to inspect the
subgrade and foundation prior to the concrete being poured. A letter from the engineer will be
required post inspection.
5. As per plat notes associated with the PUD, footer drains and radon testing is required. Both
these requirements are noted on the plans.
Many of these requirements may apply to the 6-plex pending review. If so, additional submittals will
be required for it. With all the projects submitted utilizing the shallow frost protected foundations, an
additional insulation inspection will be required prior to backfill to verify the rigid insulation is
installed properly as per Sec R403.3 of the 2003 IRC. This inspection can be done by the county
inspectors or the engineer of record for the project.

I/We hereby acknowledge and understand that our failure to complete Home Construction, Improvement
Projects or Landscaping Construction within the time constraints noted in Conditions 2. & 3. On Page
6 of this Application will result in a Notice and Opportunity for a Hearing before the BMSA Board of
Directors. If I/We are found in violation of these Conditions at the Hearing it will result in a fine to the
Owner of not less than $500.00 per month for each full month in excess of the allotted completion time
frame.
I/We hereby acknowledge that Home Construction or Improvement Projects or Landscape Construction
features which are built in non-conformance with this full application, including the Conditions of
Approval noted on Page 6 of 6 herein, I/We will be cited by the BMSA/AC in written citation form at the
time of discovery. In extenuating circumstances a Variance may be requested from the BMSA/AC,
provided such Variance request item is within the purview and approval jurisdiction of the BMSAJAC.
If a Variance is not granted by the BMSA/AC and if the Non -Conforming Items are not corrected within
60 days after the date of written citation, I/We will be served Notice of a Hearing before the BMSA Board
of Directors, and if I/We are found in violation of these Non -Conforming Items at the Hearing I/We will
be fined a minimum of 5500.00 per Non -Conforming Item. Notwithstanding the foregoing, IfWe
acknowledge that the BMSA Board of Directors and/or Architectural Committee may pursue any legal
remedy available in the event of a non-compliance.
This portion of the Application Form is to be completed by the
Architectural Committee (not the Applicant).
CONDITIONS OF APPROVAL or DISAPPROVAL
1. No sitework or building construction may proceed on any Lot (including no preliminary site clearing,
grubbing, grade alteration, storing of material, foundation excavation, extension ofunderground utilities,
etc.) until the Project has received a Project approval from the Architectural Committee and has been
issued a Building Permit (as required by Garfield County for new home or Renovation projects).
2. All general building and site improvements construction for any Project shall be completed and shall have
issued a Certificate of Occupancy from the Garfield County Building Department no later than one (1)
year after the issuance date of this Architectural Committee Approval.
3. The complete landscaping and revegetation ofthe improved Lot, to specifically include the front, side and
rear yard areas, shall be completed no later than six (6) months from the date of the Certificate of
Occupancy.
4. If the selection of colors and /or landscaping plans are not included with this application they must be
submitted for approval before any action is taken to install the missing items.

Battlement Mesa, Garfield County, Colorado
Dear Mr. Carei:
This letter presents Yeh and Associates, Inc. proposed scope of work and estimated project costs
for preparation of a geotechnical study for the subject project.
Project Description: The project includes subdivision of a parcel of land for residential housing,
construction of utilities and roadways. We were provided with a geotechnical report by HP
Geotech for the infrastructure and a site -grading plan by High Country Engineers. The client
requires a geotechnical study for the proposed structures to be constructed on the property.
Scope of Work: Based on the information provided to us, additional results of field investigation
to be performed, and our understanding of the project, we propose the following scope of work for
the completion of a geotechnical study:
1. Contract drilling or excavation services for the investigation. This estimate assumes that
the client will provide excavation equipment for the test pits.
2. Provide laboratory testing services.
3. Excavate and sample between 12 and 16 borings or test pits at the proposed site. The total
number of borings will be determined in the field. Analyze the field data and laboratory
test results from the investigation and all pertinent data obtained. Based on this data we
will provide a report with recommendations and geotechnical considerations for the
following:
• An assessment of the groundwater conditions and recommendations for surface and
subsurface drainage considerations.
• Recommended foundation types and design criteria based on the site and
subsurface conditions.
Lateral earth pressures for foundation walls.
Corrosive potential, if any, of soils on concrete.
Backfill soil type and degree of compaction and suitability of on-site soils for use
on the project.
Foundation construction criteria.
A slope stability evaluation and recommendations for mitigation of the potential
slope hazard.

4. We will observe and document site-specific foundation conditions at the time of excavation
for each of the 11 planned structures. Our estimate is based on 3 hours per site for the 11 sites.
Contact drilling or excavation services (Task #1) if needed, will be charged at cost plus 15%. The
client will be invoiced in accordance with the attached Standard Fee Schedule and General
Conditions for all other work. We estimate the following approximate costs for the project:
