The reports of the services requested and performed for the project are attached. We previously conducted a subsoil study for design of foundations at the site and presented our findings in a report dated March 30, 2015, Job No. 1 13 4711. The foundation bearing soils within the building excavation were observed and tested for compaction on May 27 and June 5, 2015 and are suitable for the recommended bearing pressure of 1,500 psf. Perimeter grade beam reinforcement steel was observed on June 11 and the fresh concrete placed for the grade beams was sampled and tested on June 11, 2015. The post -tensioned slab tendons were observed on June 22, the fresh concrete was sampled on June 23 and tensioning of the tendons was observed on July 6, 2015 according to the project specifications. The results of the tendon tensioning were submitted to Fuhrmann & Graf for their approval. Tendon 12 concrete pocket failed and the tendon pull after repair of the pocket was observed on July 20, 2015. We did not observe placement of the rigid foam insulation along the building perimeter, but since the perimeter grade beam has a minimum depth of 36 inches, it provides adequate frost protection rather than by insulation. The cable was left at a reduced tension of 3600 psi gauge pressure (22 kips force). Hayden Horsford with Silich was informed. We understand from discussions that the concrete at the anchored end will be repaired and the Engineer will be contacted for direction regarding re -tensioning of the cable.
