Onsite Wastewater Treatment System (OWTS) Installation Observations
5107 & 5109 CR 154
Garfield County, Colorado
Permit Number SEPT -3-15-3563
Craig,
ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on August 7 and 19, 2015 for the subject property. You, as the homeowner, installed the system.
The system was designed to accommodate 1 -bedroom in the existing cabin and 4 -bedrooms in the new
residence. The existing OWTS on the property was abandoned in place. An average daily wastewater
flow of 675 GPD was used.
The system installation included a 1500 -gallon, two-compartment concrete septic tank with an effluent
filter installed on the outlet tee. The soil treatment area (STA) consists of two beds, one with three rows
of 14 `Quick 4' Infiltrator® chambers connected with a level manifold (STA 1), and the other with four
rows of 10 `Quick 4' Infiltrator® chambers for a total area of 984 square feet of infiltrative area. The
infiltrative area may not exceed 4 -feet below native grade. Inspection ports were installed in all four
corners of each bed and may be cut to grade and covered with a sprinkler box for access. The STA
should be fenced or otherwise protected from traffic.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS:
Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.
Please call with questions.
Sincerely,
ALL SERVICE septic, LLC

Don't:
• Pour cooking grease or oil down the

• Rinse coffee grounds into the sink.
• Pour household chemicals down the

• Plant trees or shrubs too close to your
drainfield, roots can grow into your
system and clog it.

• Consult a septic service professional to
advise you of the proper distance for
planting trees and shrubs, depending on
your septic tank location.

• Concentrate your water use by using
your dishwasher, shower, washing
machine, and toilet at the same time.
All that extra water can really strain
your septic system.
Do:
• Stagger the use of water -generating
appliances. This can be helpful
especially if your system has not been
pumped in a long time.
• Become more water efficient by fixing
plumbing leaks and consider installing
bathroom and kitchen faucet aerators
and water -efficient products.
