This tetter constitutes a cost estimate to go along with the
Subdivision Improvements Agreement and is part of "Exhibit B" as
referred to in that document. The other part of "Exhibit B" is the
plan titled Utility and Drainage Plan, Eastbank Commercial. Center,
by High Country Engineering, Inc., latest revision date December 5,
1991. The estimated cr>nstructi-on costs would be as follows!
1. .Install ultraviolet disinfection system in accordance with the
plans and specifications submitted to the Colorado nepartment
of Health for the Eastbank CQmmerCial Center Water System,
October 1991. ($1000.00)
2. Perform roadway and entrance improvements as shown on the
Utility and Drainage Plan by High Country Engineering, Inc.,
revised 13/27/92. ($1100.00)
3. Construct additional storm water detention volume as described
on the Utility and Drainage Pian by High Country Engineering,
Inc., revised 11/27/91. ($500.00)
Install water meters on each water service as required by
Garfield County. ($200.00)

John J. Stanford (herein called "Applicant"), hereby requests
permission and authority from the Board of County Commissioners to construct a
driveway approach(es) on the ricaht--o1'-w; y of Garfield County Road Number- 154
adjacent to Applicant's property located on the. S'ly side of the road, a
distance of 1400 feet from the CMC intersection for the purpose of
obtaining access to Lot 1, Eastbank Subdivision and the. Roy Rakish property.
Applicant submits .herewith for the consideration and approval
of the Board of County c;ommissioners, a sketch of the proposed Instal--
)ation showing all necessary specification detail including (1) front-
age of lot along road, (2) distance from centerline of road to
property line, (3) number 0f ari.veways requested, (4) width of pro-
posed driveway(s) and angle of approach„ (5) distance frorn driveway
to road intersection, if any, (6) size and shape: of area separating
driveways if more than one approach, and (7) setback distance of
buildina(s) and other structures or improvements.
GENERAL. PROVISIONS
FIRST: The Applicant represents all parties in interest, and affirm;
that the driveway approach(es) is to be constructed by him for the
bona fide purpose of securing access to hip property and not for the
purpose of doing business or servicing vehicles on the road right -of' -way.
SECOND: The Applicant shall furnish all labor and materials, perform
ell work, and pay all costs in connection with the construction of the
driveway(s) and its appurtenances on the right-of-way. 411 work shall
be completed within 30 days of the Permit date.
THIRD: The type of construction shall be as designated and/or approved
by the Board of County Commissioners or their representative, and all
material; used shall be of satisfactory quality and subject to inspec-
tion and approval, of the Board of County Commissioners or their
representative..
FOURTH: The traveling public shall be protected during the instal-
lation with proper warning signs and signals and the Board of County
Commissioners and their duly appointed agents and employees shall be
held harmless against any action for personal injury or property
damage sustained by reason of the exercise of the Permit.

We have reviewed the above referenced proposal for comments on a sketch plan submittal for
the commercial center. Currently there are two existing buildings and one proposed building. The
proposed water supply for the three buildings is an existing well, Permit Number 37778-F.
This well is permitted for fire protection, commercial purposes and irrigation of not more than
0.08 acres (3,200 square feet) of lawns. If the applicant proposes these uses of the well for the
complex, we can recommend approval of this plan.

SPECIFICATIONS
ULTRAVIOLET TREATMENT SYSTEM
The ultraviolet water sterilization system shall be an Aquafine
Ultraviolet Sterilizer Model DW -8 with a maximum flow rate of 20
gpm and a maximum operating pressure of 120 psi. All installation
and maintenance shall be in accordance with the manufacturer's
specifications and manuals. The date of installation and the rated
life of the UV lamp shall be posted in the well control vault to
alert the operator to replacement schedule.

1. Install ultraviolet disinfection system in accordance with the
plans and specifications submitted to the Colorado Department
of Health for the Eastbank Commercial Center Water System,

2. Perform roadway and entrance improvements as shown on the
Utility and Drainage Plan by High Country Engineering, Inc.,
