Pursuant to our conversation of September 13, 1999 enclosed is a copy of Ursula Cowgill's
Boundary Line Affidavit. We had previously provided a copy of the plat detailing this BLA which
had been approved and signed by Sam Phelps the Garfield County Surveyor. Please advise if we
can provide any additional information to expedite the process. Thank you for your help.

AFFIDAVIT REGARDING BOUNDARY LINE ADJUSTMENT
STATE OF COLORADO

COUNTY OF GARFIELD
The undersigned affiant, being first duly sworn, states as follows:
1. I am the owner of real property in the unincorporated area of Garfield County, which
property is described in Exhibit A attached hereto and incorporated herein by reference.
2. I am desirous of adjusting the boundary lines of the two lots described on Exhibit A
in the manner depicted on Exhibit B, and sign this Affidavit in accordance with Section 1:64 of the
Garfield County Subdivision Regulations of 1984.
3. I hereby represent that no new lots will be created and therefore, that Garfield County
will not be required to issue any building permits, other than what it would be required to issue for
the already existing lots.
4. I hereby represent that none of the parcels of property involved in this boundary line
adjustment is part of a previously platted subdivision of record.
5. I hereby represent that the boundary line adjustment made reference to herein will not
cause the loss of access by road or to utilities to any parcel of property involved.
6. I hereby authorize Garfield County to record a copy of this Affidavit with the Garfield
County Clerk and Recorder, and to furnish this Affidavit to the Garfield County Assessor.
