This Application contains the following. 1. The County s Application form for Vacating Public Roads and Rights -of -Way, which has been completed and which has been executed by Michael McCoy, President of CRMS. 2. A recorded Statement of Authority indicating that Mr. McCoy nas the authority to act on behalf of CRMS. 3. An executed Payment Agreement form. 4. Your summary of our pre -application conference which was held on August 27. 2013 5. A statement entitled Request to Vacate a Portion of County Road J 06 which provides the reason for the request and addresses the matters required by the Garfield County Land Use and Development Code including Section 4-108.C. 6. A 24 by 36 inch exhibit map ("Full Size Exhibit Map") consisting of an aerial photograph of the area in question that depicts the section of County Road 106 requested for vacation; the location of adjacent properties and structures within 200 feet of the proposed vacation and the names and addresses of the owners of such property; the land uses for all adjacent property; and existing roads within a 200 foot radius of the proposed vacation. A map identifying all utility lines existing within the portion of the road proposed for vacation. 10, Letter of Authorization from CRMS. 11. Copies of letters from me to Xcel Energy, CenturyLmk, Source Gas, and the Town of Carbondale, as all of the utility providers having utilities within the roadway. In this letter I indicated that CRMS will grant to such utility providers permanent, non-exclusive easements for their utility lines, and requested that I be provided with statements of the utility companies' position on the proposed vacation. 12 A letter from the Carbondale & Rural Fire Protection District indicating its support of the requested vacation. 13. A letter of recommendation from the Association of Colorado Independent Schools. 14. Description of current condition of the road and traffic analysis. 15.
