Roth systems passed there finial functional tests. the sprinkler contractor (H FI Fire Protection) needs to install signs on the sprinkler system control valves and put sign on exterior of door to sprinkler room. The contractor is in the process of getting a Knox box to install and put the building keys in. I have also ask the contractor to follow up on having a locking device installed 011 the sprinkler control room door to secure the sprinkler system controls from the general public. After inquiry from Mr Batten and Keith Gastfield with Excell Fire Protection. I conducted further research and made contact with other state fire agencies as well as the Fire Engineering Department with the University of Oklahoma. For your reference please find enclosed photocopies of Chapter 5, (Design Approaches), of NFPA 13 and information from the Uniform Fire Code applications manual which provides answers regarding Fire Flow requirements. The Glenwood Springs Fire District has adopted the 1994 Uniform Fire Code within Garfield County. It is the intent of this code to require a minimum of 1500 G.P.M. of water for manual fire fighting for all buildings regardless of floor area and includes sprinklercd occupancies. There is a provision within Table III -A however which allows the Fire Chief to modify the fire flow downward for isolated buildings in rural areas where development of full fire flow is impractical. This became the basis for our requirements for the Church suppression system as mentioned in my previous letter of September 8`h, 2001. Based upon what we have already established and taking into consideration that you have an assembly occupancy which is remote, containing an entire spectrum of population, and potential wildland urban interfact 1 make the following clarifications to the requirements. Water Supply NFPA 13 states that the minimum water supply requirement for a hydraulically designed occupancy is determined by adding the hose stcarn demand to the water supply for the sprinklers. During our discussion it was mentioned that an 8000 gallon tank is proposed. This appears to meet the potential amount required for a Tight hazard occupancy based upon a quick look at the area density curve in chapter 5, and the hose allowance duration stipulated within the document. However there are other considerations of design which may effect the amount of water supply 806 COOPER AVENUE GLENWOOD SPRINGS. COLORADO 81601 9701945-4942 FAX 970/945-6040 based upon the designers approach and any potential special design situation with the project. Pump Design The hose allowance and the sprinkler demand are coming from the same source for the system and the pump needs to be designed for both. Exception # 3 from article 5-2.3.1.1 (NFPA 13- 1996) is not applicable in this case. Paraphrasing from the book, Fire Protection Hydraulics and Water Supply Analysis (p243, Pat Brock -Author), ... The hose stream demand plus the sprinkler demand must be available at the residual pressure required by the sprinklers... This provides an extra safety factor in design and redundancy beyond the ability of the fire department pumper to also add pressure to the system. The standard makes no provision to recognize the arrival of fire- fighters with fire department pumping equipment. Hydrant Location NFPA 24 Private Fire Service Mains and their Appurtances, requires hydrants to be placed no closer than 40 feet from the buildings protected. This is about safety, giving the fire departemnt room to work especially when developing secondary water through tender shuttle operations at the scene free from the effects of radiant heat, collapse zones etc. All of the respondents 1 spoke with state that a wall hydrant is a last resort option and will not be accepted for this project. Hose Connection A 2 '''/" hose connection with National Standard Thread needs to be installed in a location of greatest advantage. In my dialogue with Mr. Batten on the proposed building layout, by placing this appliance on the North end of the building near the proposed F.D.C. location, it can be utilized toward the critical wildland exposure areas and still be able to reach most of the inside of the building and outside parking facility for manual firefighting. This connection provides us with added safety and system redundancy should damage to the hydrant occur or quick fire attack is needed. Colorado 81602 Dear Dana, This letter incorporates general requirements for the fire suppression provision of the Uniform Fire Code as discussed at our meeting of 9-06-0I as it regards New Creation Church. In that meeting with Chief Piper and Dave Batten, we discussed the concept for compliance is to sprinkler the building, meeting the standards of NFPA 13 with an onsite water supply and tank system and, if applicable. a fire pump based on standards that arc stipulated within that same document. The desig. of thc sprinkler system must incorporate the following features. The water flow and tamper switches are required to be monitored through a central receiving station. This allows for early notification of Fire Department response to more effectively mitigate a fire event. • The system must be supplied with a Fire Department Connection, (FDC), as close to the building front as possible. The location of the FDC may he impacted by the actual location of the tank. The FDC allows us to connect to the building to boost water volume and pressure through the sprinkler system in order to suppress a fire of significant intensity. • A dry hydrant suction or wet hydrant dependant on design is required off of the tank. The nozzle end of the hydrant should have a 4 '/2" standard steamer thread connect. The hydrant location needs to be within 150 feet of the Fire Department Connection and no closer than 40 feet to the building. The hydrant enables the Fire Department to take water from thc tank and put the water through the fire department connection. • The system will require outside and inside hose allowance per the occupancy of greatest demand as determined in the standard. • Consideration for freezing must be taken into account with the design. Should the system incorporate domestic water with the required water supply for the sprinkler system then the system shall be designed such that the required fire supply will be available at all times. One of the advantages to tying the domestic into the system is the potential of circulating the water to offset the effects of corrosion through the system. The submitted plans shall bear the stamp ofa professional engineer who is licensed through the State of Colorado. Refer to the Glenwood Springs Fire Department Construction and Development Packet for plan submittal requirements and other related items. With these design features in place the department should be able to effectively mitigate a fire event 806 COOPER AVENUE GLENWOOD SPRINGS. Other applicable provisions of the fire code will or may apply based upon the construction type and occupancy classifications as determined by the building official of Garfield County during the plan review process. Box 1996 Glenwood Springs, Colorado 81602 Dear Brian; As I stated in our recent phone conversation of Nov. 30th, I have contacted Garfield County Chief Building Official Don Owens in response to your request for a Christmas Party and Tom Kelly's proposal of an auto -body detail shop taking place at the old Boat Marina facility near Canyon Creek. After conversing with Mr.Owens it's apparent that the County needs to be brought into the communication loop with any of these types of proposals. It appears zoning allows for these uses but appropriate building code provisions and occupancy type should be evaluated. Due to the number and variety of requests that the Church has for the use of this building and property, the best approach is that the County be contacted first through the applicable permit process, then with the above evaluations done, the Fire Department can more effectively apply fire code requirements. As previously discussed, one of our major concerns with this complex, is water supply for fire fighting. At a minimum, for these buildings to have any kind of occupancy, the original water supply as required by the Fire Department for the Boat Marina needs to be reinstated and maintained. The suggestion to use an existing cistern is a possibility. Documentation as to cistern size and adaptations complying to our needs, shall be submitted to the fire department. This system will need to pass final testing before approval. In regards to the proposed Church Project it is my understanding that the County has approved it's Zoning. The "Change in Use" provision of the Uniform Building Code, and applicable Fire Code articles require the building to be sprinklered. SE Job No. 98010.01 Dear: Mark: Attached herewith is a report of our findings and includes a design for the subject site improvements. The design is based on engineering evaluations pertaining to the phased redevelopment and new development associated with this project. We have utilized a report from Hepworth-Pawlak Geotechnical, Inc. (HP ) date November 23, 1999 Job No. 198 152 — 1, a report from Zancanella and Associates dated July 24, 1998, and the proposed site plan from Ernest C. Delto, Architect which includes survey information from Richard L. Holsan. Our attached proposed Site plan, (Exhibit A) water and ISDS design incorporates pertinent features from the above referenced sources. Site Location The Site is located in Section 35, T 5 S., R 90 W. of the 6`h P.M. at 44761 U.S. Highway 6, Garfield County, Colorado. The subject site is located approximately 500 feet north of the Colorado River and approximately % mile west of Canyon Creek. The site is bordered on the south by U.S. Highway 6 and the northwest by County Road 335. Latitude is 39 deg 34' 19" and Longitude 107 deg. 27' 32" Project Overview The project consists of two phases in the development of the New Creation Church. Phase I includes the construction of a main church facility near the existing westernly building A to be demolished. An ISDS system will be constructed for the peak usage of this facility only. A new Transient, non -community water system will be installed to serve the peak requirements of the Phase I development as well as phase II. Phase II includes the remodel construction of a school/daycare facility incorporating the existing building B. A separate ISDS system will be designed and installed to accommodate this facility. A water treatment and distribution system will be constructed to provide potable water to the phase I facility. An expanded water system distribution system will be constructed to serve the needs of the phase II facility. This water system will then be classified as a non -transient, non -community water system. Alternatives Considered The conceptual design for water and sewer improvements have been ongoing for three years. The initial conceptual plan involved connecting water and sewer services to the Canyon Creek Estates system located approximately one mile to the east adjacent to Canyon Creek. In order to utilize the water system the church would be required to share the cost to provide for an upgrade to accommodate the subdivisions ultimate build out as well as accommodating the required additional storage for fire flow and in house needs at the church. The increased cost of providing treatment, distribution and storage for approximately 102,000 gallons is cost prohibitive. The distribution line would run approximately 5000 feet and include a watercourse crossing. The total additional cost between constructing an onsite system versus utilizing the off site system is approximately $ 160,000. The current average daily wastewater flows for the Canyon Creek subdivision is 16,000 gallons per day with a maximum plant capacity of 21,000 gallons per day. It has been determined that the wastewater treatment plant at Canyon Creek has the capacity to treat the additional wastewater from the church, however the plant needs some maintenance work. Zancanella and Associates estimated the cost to the church at $12,000. The increased cost of providing treatment plant maintenance, distribution and lift stations is cost prohibitive. The Sewer line would run approximately 5000 feet, include a water course crossing and lift station. The total additional cost between constructing an onsite ISDS system versus utilizing the off site wastewater treatment plant system is approximately $175,000. The total additional cost between constructing onsite water and ISDS system versus utilizing the off site systems is approximately $335,000. Due to economic constraints scheduling and legal agreements a decision has been made to forego connection to the off site water and sewer systems and construct the proposed water and ISDS systems presented in this report. However the church will retain the option to connect to a future regional water and sewer facilities if the engineering and economic realities are favorable. ISDS System Wastewater from Building A will discharge to a series of septic tanks. Effluent will discharge out of the secondary tank by use of a metered dosing siphon. The dosing assembly will gravity discharge effluent to a hydro splitter device for discharge through 6 individual lines which will distribute the effluent to the absorption fields located along the southern property boundary on the site. Wastewater from Building A will flow to a 2500 gallon septic tank and one 1000 gallon septic tankidosing tank connected in series. We have designed the system to allow for dosing which would discharge evenly over two separate fields. Based upon soils reports and Garfield county requirements we have calculated the absorption field areas for Building A to be 1108 square feet each. The total calculated volume of required septic tank volume is 2375 gallons and the peak dosing volume is 500 gallons. We recommend a dosing siphon to control the dosing volume of approximately 500 gallons to insure adequate saturation of the fields during a dosing cycle and allow for a rest period. Pastor Mark Bintliff SE Job No. 98010.01 November 8, 200I Page 3 Existing Site Conditions: The 5.8 -acre site was previously occupied by a boat and recreational vehicle storage and maintenance facility. A one story pre -fabricated steel structure and a one story wood frame over a walkout basement/garage exist near the southwest and southeast corners of the site respectively. A shallow pond is located near the east central portion of the site. The ground surface is relatively flat in the southern portion of the site and continues with a moderate grade up to the northern most point on the site. The overall slope is slight to moderate with decreasing grade from north to south. Approximately 35 to 40 feet of elevation difference exist across the site. Vegetation typically consists of grass and weeds. The southern portion of the site is covered with gravel and road base along with two existing structures and large concrete slabs. The southern portion of the site where the proposed absorption fields will be located consist of disturbed pasture grasses that have been irrigated in the past with slopes Less than 10%. The existing sub surface materials at the proposed ISDS location consist of native materials. Proposed Site Condition: The existing Building A will be demolished and a new church facility building will be constructed with associated parking and driveways. The project phase 1 intends to serve a maximum of 475 people at the 450 seat church facilities for Sunday service. Water will be supplied by an existing well in the south portion of the property and is shown on the attached site plan. We have based our design on a peak day, design flow of 5 gallons per day per person to accommodate a maximum of 475 persons on the peak day once a week. The attached Water Usage Demand Requirements table (Exhibit B) delineates peak day and average water requirements for Phase 1 and 2 incorporated into the design. A 20% allowable reduction has been applied to the design flow for water conserving fixtures. All plumbing fixtures in the proposed facility will utilize permanently installed water conserving devices. Subsurface Conditions: Attached herewith are the results of the subsurface investigation and percolation tests conducted by Hepworth Pawlak Geotechnical, Inc. (RP) dated October 22, 1997 Job No. 197 490 (See Exhibit C). The subsoil encountered at the site consists of 0 - 5 feet of fill overlying sand and silt. Silty sandy gravels containing cobbles and possible boulders was encountered at depths between 5 and 1.6 feet. The ground water was not encountered at the time of the investigation and is not expected to be encountered at a depth, which may influence the design of the proposed absorption system. The absorption rate of the soils on site was measured to be 28 to 45 minutes per inch with an average of 34 minutes per inch. We recommend using 34 minutes per inch for design purposes. The system will be installed in natural soils below any fill encountered. Pastor Mark Bintliff SE Job No. 98010.01 November 8, 2001 Page 4 System Design: A site plan is provided to detail the various components of the proposed sewage disposal system. Please note that the size of the proposed leach field system is based on Garfield County's peak flow per person per estimated institutional usage rates. Pursuant to a pre submittal conversation with Dyain Watson of the Colorado Department of Public Health and Environment (CDPHE) the peak day design flow of waste water has been established by utilizing the daily flow per person based on the number of seats in the in facility. The maximum number of people expected to utilize the facility on the peak day is 450. For conservative design 475 people is used in calculating the design flow. Dyain Waston has agreed that the 20% reduction for utilizing water conserving devices is appropriate for this application. The following panes detail the technical criteria utilized in design of the proposed system. We recommend using a conventional absorption bed system without an area reduction. Desjgn Criteria: The proposed system is designed to serve a church facility containing 450 seats or 475 people maximum per peak day once a week. The design flows are calculated as follows Institutional use — peak day per person usage = 475 persons. Daily Flow (Qda) = # of people X 5 gal/person/day Qda =475X5=2375gpd Design Flow (Qd) = Qda X (0.80) gallons/person/peak day Based on permanently installed water conservation devices, allowable 20% reduction. Qd = 2375 gpd X (0.80) = 1900 gpd Septic Tank Design: Volume (v) of tank = Design flow (Qd) * 1.25 (30 hour retention) V = 1900 X 1.25 = 2375 gallons A 2500 -gallon septic and a 1000 gallon double compartment septic/dosing tank will be installed. The tanks will be installed and operate in series. The first tank will hold the solids and insure adequate detention time of the effluent. The outlet of this tank will incorporate a maintainable effluent filter to collect any suspended solids that may be present in the effluent. The second double compartment tank will store the dosing volumes. The second double compartment septic/dosing tank will accommodate the dosing siphon system to be installed in the secondary chamber of the second tank. This system will gravity discharge primary treated effluent to a hydro -splitter device for discharge to the absorption fields. Pastor Mark Bintliff SE Job No. 98010.01 November 8, 2001 Page 5 Percolation Test Results As mentioned above, the soils on the site are adequate to provide effective percolation within normal ranges. However, additional sub surface soil investigations will be performed, by the engineer, in the final staked out Iocation of the absorption field to confirm assumption utilized in the design. Absorption Field Disposal System Design: Average percolation rate is 34 minutes per inch A (SF) = (Qd)/5 X (t)1/2: where A = area, Qd = (gaVpeak day) (t)tr = square root of time of absorption rate (min/inch) This design will give a recommended minimum absorption field area: A = 1900/5 X (34)1/' = 2215 sq. feet Distribute dosing over two fields Area each field = 2215 1 2 = 1 108 sq. feet Use two separate 18 x 69 foot fields (1152 SF) with equal distribution dosing approximately 4 times per day. The system will provide gravity dosing to the fields through individual distribution lines from a distribution manifold located in a Large distribution box. The effluent will gravity flow to separate headers connected to each pair of rows in the absorption fields. We recommend that an Orenco Systems Inc., 8" FT -series Biotube effluent filter be installed in the outlet of the first septic tank. Dosing Tank The second septic/dosing tank in series will be a double compartment tank that will store the closing volumes and accommodate the dosing siphon system. This system will incorporate a float switch connected to a dosing counter to monitor wastewater flows. We recommend that an Orenco Systems Inc., 3" dosing siphon be installed. Use a model OSI-3-I8BF. The system will be monitored for performance and incorporate a flow meter counter which will record dosing events. Distribution System We recommend installing an Orenco Systems Inc.; Hydrosplitter housed in a 24" distribution box riser vault. Use a model HSA 200-06-10 Hydrosplitter. Biotube pump vault system or equivalent in the secondary compartment of the septic/dosing tank. The systems have easy access and maintenance design and are well suited for this commercial application. Pastor Mark BintIiff SE Job No. 98010.01 November 8, 2001 Page 6 Site Improvement Layout: With respect to the current conditions, all septic tanks and sewage lines must be watertight. A peak dosing volume of 500 gallons will most likely be required due to the peak day design flow. The equal dosing to the separate fields will allow for better treatment and recovery within the absorption beds. The hydro splitter will ensure equal distribution to the field. The dosing/septic tank will be gravity fed from the first (2500 gallon) septic tank. The septic tanks must be watertight therefore, it is recommended to install one piece tank units or factory sealed two piece units to prevent seepage out of the tanks. If the tank pieces have to be joined in the field extra care shall be used to insure watertight seams. A pressure test is required to insure maximum resistance to infiltration. Technical specifications for the construction of the waste water pipes are contained in Exhibit D. General Notes: 1) All materials and installation practices shall conform to the Garfield County ISDS regulations. 2) All sewer lines in the system shall be schedule 40 or SDR 35 PVC unless specified otherwise on the plan. 3) The contractor shall ensure that the concrete septic tank and sewer lines are completely watertight. 4) The absorption fields shall be planted (sod or mulch seeding) with suitable vegetation cover. 5) Install a two-way clean out on the service line from the church facility. 6) The bed areas must be protected to prevent damage from vehicular or livestock traffic and must be crowned to divert drainage away from the beds. 7) The Engineer shall be notified to inspect the system during construction prior to the backfilling of any components of the system and perform a final as -built inspection of the entire system. Our design and recommendations are based upon data supplied to us by others. If subsurface or site conditions are found to different from these described in this report, we should be notified to evaluate the effect it may have on the proposed ISDS. If the County Environmental Health Department requests changes or modifications to this design, we should be contacted to evaluate the effect on the ISDS. Pastor Mark Bintliff SE Job No. 98010.01 November 8, 2001 Page 7 Water System The water system will incorporate installation of new distribution, storage and treatment systems. Technical specifications for the construction of the water mains are contained in Exhibit D. The existing well has sufficient capacity (24 gpm) to easily meet the needs of the church. See Exhibit E and F for physical and legal information regarding the existing well. On site storage will be required to meet fire flow requirements using a system separate from the potable water system. Water treatment will be required for in-house/potable water. Disinfection of potable water is required and we propose the use of chlorine with a linear contact chamber consisting of approximately 100 feet of I2" DIP. Standard 2" corporation stop taps will be installed on either end of the 12" pipeline. In addition a reverse osmosis system will be utilized to supply drinking water fixtures within the church facility at selected locations. Potable Water System The water will be supplied by an existing well located an the southern portion of the property east of the old dairy building. Water wilI be routed through a water treatment and control building adjacent to the well. The well water will be metered, treated and pressurized through pressure tanks for distribution through 100 L.F. of 12" DIP. Treatment will consist of sand separation and/or filtration with chemical disinfection. The chlorine contact time of 30 minutes is achieved by storage in the pressure tanks and the 12" DIP installed in the distribution main trench. A 2" class 200 polyethylene water main will be installed along the southern driveway on site from the treatment and control building to the church facility. The distribution system will supply water for domestic use in the church facility only. A separate water supply and distribution system will be installed for fire protection. An additional curb stop for a 2" water service and a stub out line will be installed to serve the Phase II "Building B". Additional 2" water main may be installed as required to extend service for Phase 11 useage. See Attachments 2 and 5 for physical and legal information regarding the proposed water system. Pressure Tank Storaee The potable water system will utilize (4) 25 -gallon pressure tanks to store pressurized water for in house use only. The tanks will be housed in the proposed water treatment and control building and will be installed in parallel with means for isolation. The tanks will serve to provide partial chlorine contact time detention storage. The tanks will be installed with isolation ball valves and quick connect fittings. A pressure switch will control the well pump. An appropriate pressure gage will be installed. The system will limit pressure to below I00 psi with an operating range between 35 and 60 psi. The tanks shall be installed in accordance with the State of Colorado, Design Criteria for Potable Water Systems and AWWA standards. Shop drawings and specifications shall be submitted to the engineer for approval. Other accessories may be required by the Water Control Division. Water Treatment and Control Building The Water Treatment and Control Building shall be constructed in accordance with all applicable State and Local building codes. All applicable building and treatment components shall be installed for compliance as a water treatment facility per State of Colorado, Design Criteria for Potable Water Systems. The system will include an above around water supply line inspection loop with a boiler air release valve, a 3/." water meter, pressure gage, and a sand separator/filter device. High and low pressure alarms shall be installed along with a low chorine alarm. All controls and components of the system to be housed in the insulated heated and vented building. The building to be wired for 120 volts, 40 amps single-phase electrical service for equipment. Disinfection The potable water system will utilize chlorine contact volume installed on the distribution system. Chlorine will be applied into the water supply inspection loop on the supply end prior to the sand separator device. The sand separator unit will provide a means for adequately mixing the chlorine. The system will include two liquid chlorine chemical storage containers, a chemical feed pump, disinfectant injection port, 3/." hose bib, controls, sampling ports, testing equipment and supplies. Other accessories such as chlorine level monitors and alarm should be incorporated into this system. The chlorine application system shall be designed to provide for a residual chlorine concentration of 2 mg/L. The volume required for minimum chlorine contact time of 30 minutes is 720 gallons. The required Volume is based on the well's sustained production yield of 24 gallons per minute. This volume is adequate to treat the water for the proposed in- house uses. It is noted that the water is aesthetically not intended for potable consumption with out additional treatment through Reverse osmosis systems in the building. Approximately 100 gallons of Chlorine contact volume is provided in the pressure tanks. The additional 620 gallons required is provided in approximately 100 feet of 12" ductile iron pipeline installed along the approximate 330 foot distribution run from the Treatment and control building to the church facility. Some additional volume exists in the 2" polyethylene water distribution main. The chlorinated water from the distribution piping in the treatment building inters the 12" diameter linear contact pipe chamber through a corporation stop service connection on the supply end. The detained water will exit the enlarged pipeline on the distribution end through a corporation stop service connection and continues through the 2" polyethylene water distribution main to the church facility. 100 Year Flood Plain The site does not lie in a flood plain. The approximate range in elevations in the Colorado River flood plain south of the site is 5608 to 5612 feet above MSL. The low end of the site is approximately 30 feet above the 100 -year flood elevation. See Attachment 4 for detailed flood plain information. Water Quality A water sample from the water supply source, the Dairy Basement well, was collected and analyzed in May of 1998. An additional sample was collected and analyzed in February of 2000. The lab results from the two sampling events present similar concentrations of constituents contained in the well's water. The parameters tested indicate a naturally soft, alkaline water with relatively high concentrations of sodium, chloride and bicarbonate which constitutes the majority of the total dissolved solids. Sodium, Chloride and TDS exceed the maximum containment levels (MCL) for drinking water however they are all secondary parameters that are not enforceable by the CDPHE. It is recommended that drinking water be treated for limited usage at selected locations only. All primary parameter test results indicated concentrations below drinking water standard MCL's. The concentration of fluoride is elevated but is below the limit. The Pastor Mark Bintliff SE Job No. 98010.01 November 8, 2001 Page 9 Bacteriological test indicated zero colonies of coliform. The turbidity at the time of sampling was slightly high, however we do not anticipate any adverse effects associated with the chemical or mechanical treatment of the water. It is noted that the sample was collected in conduction with a well aquifer pump test after an extended period of well in activity. We anticipate that any elevated levels of suspended solids will only be present during maximum pumping demands and will be filtered out by the sand separator device prior to distribution. An additional filtration cartridge system may be incorporated however is not recommended at this time. Other parameters tested are within acceptable ranges precluding the need for additional treatment. See Attachment 6 for detailed water analysis information. Water Treatment Water treatment shall consist of chlorination as mentioned above and reverse osmosis (RO) to treat and store limited amounts of drinking water only. The RO water would be distributed via separate supply lines to fixtures at select locations within the building. No other treatment processes are anticipated at this time. R.O. System The R.O. system will be a central unit that will include the RO Filters with automatic pre and post treatment controls, and associated plumbing. A 37 -gallon pressure tank will store treated water for distribution to the selected fixtures. The system is estimated to process approximately 125 gallons per day. A primary drinking water fixture will be installed next to the kitchen sink. If desirable additional small gooseneck fixtures may be installed in the primary public rest rooms for limited use drinking only. If more dedicated drinking water is desired or needed for peak day use, an additional pressure storage tank can be installed. The recommended system is a Culligan B-1 L plus or approved equal. Water Quality Monitoring The management of the system shall include a periodic water quality monitoring program. The program shall have a schedule to collect water samples from designated sampling points for testing of specific parameters as required for transient, non -community public water systems. The Water Control Division will establish the monitoring frequency during the compliance period and allow for waivers when test results indicate a waiver is appropriate. The parameters that must be monitored for this system include but are not limited to bacteriological, nitrate and nitrite. Phase two of the project, when the day care/school facility is constructed, will change the system to a Non transient, non -community water system, and will require additional parameters to be tested. When a treatment process will effect the corrositivity of the system's potable water, a lead and copper assessment must be completed. The independent R.O. distribution system shall utilize appropriate plastic pipes. Fire Protection Water System The fire protection storage tank (10,000 gallons) location will only provide approximately 8 -psi water pressure to Building A. A 4" DIP water pipeline will be installed from the tank will supply stored water to the fire suppression system. A fire booster pump will be installed as required to provide adequate pressure to the proposed building for sprinkler requirements. A separate 4" DIP water pipeline will be installed • 44 Pastor Mark Bintliff SE Job No. 98010.01 November 8, 2001 Page 10 from the tank to supply stored water to a standard fire hydrant located at least 40 feet from the proposed building. A minimum storage volume of 8,000 gallons is required for inside/outside hose allowance demand and volume required for the sprinkler system. This is based on the amount required for light hazard occupancy. Additional storage is required for the fire hydrant for a total storage volume of 10,000 gallons. The system will include a Fire Department Connection, (FDC), installed near the north end of the building. The FDC allows the Fire Department to connect to the building to boost water volume and pressure through the sprinkler system in order to suppress a fire of significant intensity. A 2 %_" hose connection with national standard threads will be installed near the FDC to be utilized for manual fire fighting inside and outside of the building. This connection will provide safety and system redundancy if damage to fire hydrant occurs or quick fire attack is needed. A wet hydrant will be installed with a 4 '/2" standard steamer thread connect. The hydrant may be used to take water from the tank and put the water through the fire department connection. All fire protection system shall be operated and installed in accordance with the 1994 Uniform Fire Code. Fire Water Supply The water supply source for Fire Protection water and possible irrigation needs will be provided by an existing well located just east of the site. The well is reported to be currently in service and provides similar water quantity and quality as the Dairy Basement Well proposed as the potable water supply well. And existing water supply line exists and runs through the site. The line will be located and evaluated. A portion of the line may be used if adequate or replaced. This well may also provide irrigation water supply. Water Storage Tank The water storage tank should be installed in the location shown on the plan at the highest elevation feasible on the site. The tank will be buried or partially buried and covered. The storage volume in the tank has been calculated to provide the required fire flow of 10,000 gallons. The tank shall be constructed per AWWA standards. Shop drawings and specifications shall be submitted to the engineer for approval. The tank shall be vented to prevent differential internal and external pressures. The tank shall be equipped with an inlet riser to deliver fresh well water near the top of the tank to promote mixing. The water supply line to the tank shall be equipped with a float valve or other type of automatic valve to allow the tank to fill on demand. Low and high water alarms shall be installed to notify responsible parties of system failures. Tank shall be installed in accordance with the manufacture specifications. Other accessories may be required with the tank installation by the Water Control Division. Storaze Tank Foundation The tank shall be supported and backfilled with structural compacted granular material. Site grading around the tank shall provide for positive drainage away from the tank. The well pump in the fire water supply well may be utilized to supply pressure for irrigation purposes or to fill a reservoir. A by pass valve, distribution pipes and yard hydrants may be installed at the potable supply well head and/or at the water treatment and control building to provide for irrigation needs independent of potable distribution system. The fire water storage tank water could be utilized for irrigation purposes if withdraw rates are lower than the re -supply rate. There are three points of entry allowing the Fire Department to get to all exterior points of the building easily. The fire hydrant is in a good location for easy access, is near the fire department connection and provides us with enough distance for firefighter safety. Water Tank for Fire Suppression A 10,000 gallon underground tank is being installed to supply the fire suppression system The church is considered to be a Tight hazard occupancy per NFPA 13. Dana has shown me documentation from various fire suppression system installers estimating 8000 to 9000 gallons as meeting design compliance for sprinkler and hose requirements within NFPA 13 for this project. Underground Piping used for Fire Protection Supply Lines In addition to the plan notes regarding the water main stipulated within the plans, the main needs to comply with the enclosed section of Code of Colorado Regulation 8 CCR 1507- I I. At this juncture I have not received plans for the sprinkler system itself which needs to meet the requirements mentioned in earlier correspondences provided to Mr Yerian and copied courtesy to you. 1 offer the following comments towards this review. Access Access as depicted on the site plan meets compliance to Section 902 of the Uniform Fire Code. There arc throe points of entry allowing the Fire Department to get to all exterior points of the building easily. The fire hydrant is in a good location for easy access, is near the fire department connection and provides us with enough distance for firefighter safety. Water Tank for Fire Suppression A 10,000 gallon underground tank is being installed to supply the fire suppression system. The church is considered to be a Tight hazard occupancy per NFPA 13. Dana has shown me documentation from various fire suppression system installers estimating 8000 to 9000 gallons as meeting design compliance for sprinkler and hose requirements within NFPA 13 for this project. Underground Piping used for Fire Protection Supply Lines In addition to the plan notes regarding the water main stipulated within the plans, the main needs to comply with the enclosed section of Code of Colorado Regulation 8 CCR 1507-11. Essentially the line needs to be installed, tested, flushed and inspected in accordance with NFPA Standard 24, Private Fire Service Mains and their Appurtenances. A completed copy of the Contractors Material and Test Certificate for Underground Piping needs to be provided to the Department prior to final hookup to the fire suppression system. Existing Building B The existing building footprint on the plans states that a proposed daycare is intended for phase 1I of the project. In my conversation with you and Dana we noted that this potentially constitutes a change of use and fire protection would need to be designed with this building when phase 1l occurs. We appreciate the efforts the Church, Mr Yerian and Sopris Engineering have conducted with this project so far. A copy of the 'Contractor's Material and Test Certificate for Underground . Piping' must be completed and provided the owner and certified fire suppression system inspector by the contractor. 5.8 Additional Conditions 5.8.1 Flow tests an water supply systems must be less than one year old, unless approved by the certified tire suppression system inspector having jurisdiction. 5.8.2 When calculating water euppty requirements, deed ten (10) • percent from the flow test. Kon Biggers 945.4942 4 S f. r.♦SPIc-troat F;$1 f4C7 INDUSTINAL SERVICES RECEIVED NOV 0 4 2002 GARFIELD COUNTY BUILDING & PLANNING Final Report for New Creation Church (Garfield co #8250) Upon our initial site visit on 9-9-2002, it was discovered that the high-strength bolts (A- 325) were found to be not tensioned to the plan required specification. Additionally it was noted that at seven locations a standard machine bolt grade five had been substituted for the required A-325 bolt in critical rafter to column connections. At several locations field fixes requinng cutting and welding were performed to attempt to repair factory fabrication errors. The workmanship exhibited in completion of these field fixes was not compliant with applicable codes. When we requested copies of engineer -approved drawings (RF1's) to direct the work required to correct misfabncated components, we were told that there were no drawings. A certified welder allegedly did these noncompliant field repairs under the verbal instructions of the responsible structural engineer. Clearly, the workmanship and techniques used to facilitate repairs were not indicative of work performed by a certified and experienced welder under an engineer's direction. Additional work completed that was not in compliance with applicable codes was found at the entrance canopies on both the Southly and Westly sides of the building. This deficiency was noted as bolts that had been left loose and welding that had unsatisfactorily performed. Pitkin Iron was notified and work to repair these deficiencies was undertaken by their staff. Work by Rocky Mountain Steel Structures to tension the high strength bolts using the turn of nut method as specified in approved plans began on 9-12-2002 and continued through the date of 10-14-02. Many of the locations where bolts needing tensioning existed had been framed over and sheetrock applied. At these locations the necessary amount of demolition work required to access these connections was performed so that all A-325 bolts could be tensioned. This work continued for several days. During the process of bolt tensioning some additional items that were noncompliant appeared. These can be characterized as holes that had not been drilled to accommodate bolts at required locations as shown on print, and also as other bolt holes that had been torched oversize. In both of these situations the appropriate measures were taken to correct findings in such a manner to be compliant with the approved plans and applicable codes. Rocky Mountain Steel Structures undertook this work in a demeanor best described as very willing to correct what was needed and do it completely. After a small delay to allow for welder certification testing, the staff of Rock Mountain Steel Structures acting under the direction of RF1's provided by the Responsible 1✓ngincer removed the defective work as noted in paragraph one and then completed the necessary re -fabrication and welding as necessary to complete the building. With this work completed by both Rocky Mountain Steel Structures and Pitkin Iron Co: Industrial Inspection Services certifies that to the best of our knowledge and ability, all the high strength bolts in this structure (A, B and C buildings) have been tensioned to meet the plan specification. Industrial Inspection Service also bears witness to the work necessary to repair mis-fabrication as noted on inspection records is now complete and in accord with applicable codes. Industrial Inspections also declares that the code required tension -testing of high strength fasteners was not preformed as we were not employed prior to erection of building. Additionally there were an insufficient number of bolt samples available to provide tension tests as required in the applicable code.
