Subject: As -Built Evaluation of On -Site Wastewater Treatment System, Proposed
Cabin and Existing Garage, Jolley Ranch, 1080 CR 335, Garfield County,
Colorado
Dear Mr. Jolley:
As requested, Hepworth-Pawlak Geotechnical (H -P Geotech) observed the installation of
the onsite wastewater treatment system (OWTS) for the proposed cabin and existing
garage at the subject property. Our observations and recommendations are presented in
this report. The services are supplemental to our agreement for professional services to
the Richard and Mary Jolley Family LLLP dated March 25, 2015.
We previously designed the OWTS for the project and presented our recommendations in
a report dated September 29, 2015 under the above job number. The system was
designed for four bedrooms for the cabin and the garage.
The OWTS components observed included: sewer pipe, sewer pipe encasement, effluent
transport pipe, distribution box, Infiltrator chamber soil treatment area, cleanouts,
inspection ports and the locations of these components. The installer was Haywood
Construction.
We performed our As -Built site evaluation on October 29, November 20 and December
7, 2015. The installation and locations of system components were verified as shown on
Figure 1. A Tu€ Tite distribution box was used instead of the Fralo distribution box
specified in our design. This will not alter the effectiveness of the OWTS operation.
Photographs of the OWTS installation are provided in the attached Photograph Log.
It is our opinion that the OWTS was installed in general conformance with our design
dated September 29, 2015. The soils observed at the base of the STA trenches were
consistent with the sandy clay with gravel soils previously observed on the site and no
sizing modifications to the design were recommended.

observations and our experience in the area. We make no warranty either express or
implied. Alterations to the system or improper backfill and site grading could affect the
operation and effectiveness of the OWTS.
