GARFIELD COUNTY BUILDING AND PLANNING DEPARTMENT
AGREEMENT FOR PAYMENT FORM
(Shall be submitted with application)
GARFIELD COUNTY (hereinafter COUNTY) and
(hereinafter APPLICANT) agree as follows:
1. APPLICANT has submitted to COUNTY an application for
(hereinafter, THE PROJECT).
2. APPLICANT understands and agrees that Garfield County Resolution No. 98-09, as amended,
establishes a fee schedule for each type of subdivision or land use review applications, and the guidelines for the
administration of the fee structure.
3. APPLICANT and COUNTY agree that because of the size, nature or scope of the proposed
project, it is not possible at this time to ascertain the full extent of the costs involved in processing the
application. APPLICANT agrees to make payment of the Base Fee, established for the PROJECT, and to
thereafter permit additional costs to be billed to APPLICANT. APPLICANT agrees to make additional
payments upon notification by the COUNTY when they are necessary as costs are incurred.
4. The Base Fee shall be in addition to and exclusive of any cost for publication or cost of
consulting service determined necessary by the Board of County Commissioners for the consideration of an
application or additional COUNTY staff time or expense not covered by the Base Fee. If actual recorded costs
exceed the initial Base Fee, APPLICANT shall pay additional billings to COUNTY to reimburse the COUNTY
for the processing of the PROJECT mentioned above. APPLICANT acknowledges that all billing shall be paid
prior to the final consideration by the COUNTY of any land use permit, zoning amendment, or subdivision plan.

Hear is the letter for the approval for the request for the preliminary plan with the conditions met.

2) I have paid the School Impact Fees and the final plat.
3) We have had an unincorporated Homeowners Association written up by Barbara Burwell.
She also
Included the governance of the shared well for Lot 18. And also the governance of the
management plan for the ISDS. She has sent the paper work to Garfield County.
4) I have paid the Traffic Impact Fee as well as the fees for the Carbondale and Rural Fire
Protection
District.
5) Steve Anthony has come to the property and inspected for any noxious weeds. Per e-mail to

We have reviewed additional information concerning the above referenced proposal to
subdivide 6.81 acres into two residential lots of 4.28 and 2.61 acres. Each lot is proposed to have
one single-family dwelling. The water supply for the lots is to be provided by two wells. Water use
estimates and well permit numbers were not provided. Sewage disposal is to be through individual
septic systems
The additional information consisted of a letter from Samuelson Pump Co. dated August
25, 2003 concerning the test of the House Well, which states that: most of the drawdown occurred
in the first 15 minutes of pumping (from 256' — 61/4" to 320'-7'/2"); the water level was stable for the
remainder of pumping; recovery was to 300'-3" in the first 15 minutes after pumping ceased and;
recovery continued at a slow rate. Based on this information it appears that with adequate storage
capacity this well should provide an adequate supply for the proposed use.
Pursuant to Section 30-28-136(1)(h)(I), C.R.S., it is our opinion that the proposed water
supply will not cause injury to decreed water rights and is adequate. If you or the applicant has
any questions concerning this matter, please contact Craig Lis of this office for assistance.
