NOTICE
Authority. This application for a Building Permit must be signed by the Owner of the property, described above, or an authorized agent. If the signature below is not that of the Owner, a separate
Jetter of authority, signed by the Owner, must be provided with this Application.
LeggJ Access. A Building Permit cannot be issued without proof oflegal and adequate access to the property for pmposes of inspections by the Building Department.
Otber Permits. Multiple separate permits may be required: (l) State Electrical Pennit, (2) County ISDS Pennit, (3) another permit required for use on the property identified above, e.g. State or
County Highway/ Road Access or a State Wastewater Discharge Permit.
Void Permit. A Building Permit becomes null and void if the work authorized is not commenced within 180 days of the date of issuance and if work is suspended or abandoned for a period of 180
days after commencement.

I hereby certii)· that I have read this Application and that the information contained above is true and correct. I understand that the Building Department accepts the Application, alon'g witll the plans
and specifications and other data submitted by me or on my behalf(submittals), based upon my certification as to accuracy.
Assuming completeness of the submittals and approval of this Application, a Building Permit will be issued granting permission to me, as Owner, to construct the structure(s) and facilities detailed on
the submittals reviewed by the Building Department.
In consideration of the issuance of the Building Pcnnit, I agree that I and my agents will comply with provisions of any federal, state or local law regulating the work and the Garfield County Building
Code, ISDS regulations and applicable land use regulations (County Regulation(s)). I acknowledge that the Building Pennit may be suspended or revoked, upon notice from the County, if the location,
construction or use of the structure(s) and facility(ies), described above, are not in compliance with County Regulation(s) or any other applicable law.
I hereby grant pennission to the Building Department to enter the property, described above, to inspect the work I further acknowledge that the issuance of the Building Permit does not prevent the
Building Official from: (1) requiring the correction of errors in the submittals, if any, discovered after issuance; or (2) stopping construction or use of the structure(s) or facility(ies) if such is in violation
of County Regulation(s) or any other applicable law.
Review of this Application, including submittals, and inspections of the work .bY the Building Department do not constitute an acceptance of responsibility or liability by the County of errors, omissions
or discrepancies. As the Owner, I acknowledge that responsibility for compliance with federal, state and local laws and County Regulations rest with me and my authorized agents, including without
limitation my architect designer, engineer and! or builder.

The following items are required by Garfield County for a final Inspection:
I) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot"& cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN
ILLEGAL OCCUPANCY AND MAY BE GROUNDS FOR VACATING PREMISES

On 11129/10 Martin requested extension of building permit and was granted by Andy.

Received phone call from neighbor Bob Anderson (366-0772) informing me that another
building has been built. Provost confirmed with drive by week of 1/10111. On 1/17/11 made
phone call to Martin requesting that he do no more construction other than house that is
permitted. He said he received free greenhouse from friend and he had set it up (This must be the
structure that Anderson called about).
Martin informed me that Orrin Moon (Burning Mtn Fire Marshal) had been out to inspect wood
burning stove. I confirmed with Orrin via email dated 1/21/11 (see e lectronic file-T:\Code
Enforcement\NOV letters\Carrillo). Orrin is still doing research on manufactures' installation
specifications.

Called him to inform that 6 month extension was to expire end of this month. Told him to call
Andy to discuss options but other than renew permit (fee needs to be paid) there was little we
could do. W/o new permit (again good for only 6 months) he would have to remove trailers on
property.
2011-2013
Building permit had been renewed and expired every 6 months. As of2010, Malone Ranch took
back ownership of property and all structures but this department had not been informed . Martin
CmTillo has moved off the property and removed his travel trailer. The building p ermit will be
transferred to Malone Ranch LLC & R & B Ranch LLC by Deed in Lieu ofF oreclosure on
11116/2010 and recorded on 11119/2010. Mr. Malone is to forward a letter requesting building
petmit be transferred to his name.

B. A certificate of taxes due listing each taxing jurisdiction shall be obtained from the county treasurer or the
county treasurer's authorized agent;
C. Information regarding special districts and the boundaries of such districts may be obtained from the board of
county commissioners, the county clerk and recorder, or the county assessor.
Note: Colorado Division of Insurance Regulations 3-5-1, Subparagraph (7) (E) requires that "Every title entity shall
be responsible for all matters which appear of record prior to the time of recording whenever the title entity conducts

unrecorded lines will include: disclosure of certain construction information; fmancial information as to
the seller, the builder and/or the contractor; payment of the appropriate premium; fully executed Indemnity
agreements satisfactory to the company; and, any additional requirements as may be necessary after an
examination of the aforesaid information by the Company.
No coverage will be given under any circumstances for labor or material for which the insured has contracted for or
agreed to pay.

DISCLOSURE
The title company, Stewart Title of Colorado, Inc.-Glenwood Springs Division in its capacity as escrow
agent, has been authorized to receive funds and disburse them when all funds received are either: (a)
available for inunediate withdrawal as a matter of right from the fmancial institution in which the funds are
deposited, or (b) are available for immediate withdrawal as a consequence of an agreement of a fmancial
institution in which the funds are to be deposited or a fmancial institution upon which the funds are to be
drawn.
The title company is disclosing to you that the fmancial institution may provide the title company with
computer accounting or auditing services, or other bank services, either directly or through a separate entity
which may or may not be affiliated with the title company. This separate entity may charge the fmancial
institution reasonable and proper compensation for these services and retain any profits there from.
The title company may also receive benefits from the financial institution in the form of advantageous
interest rates on loan, sometimes referred to as preferred rate loan programs, relating to loans the title
company has with the fmancial institution. The title company shall not be liable for any interest or other
charges on the earnest money and shall be under no duty to invest or reinvest funds held by it at any time.
In the event that the parties to this transaction have agreed to have interest on earnest money deposit
transferred to a fund established for the purpose of providing affordable housing to Colorado residents, then
the earnest money shall remain in an account designated for such purpose, and the interest money shall be
delivered to the title company at closing.
CONDITIONS
I. The term mortgage, when used herein, shall include deed of trust, trust deed, or other security
instrument.
2. If the proposed Insured has or acquired actual knowledge of any defect, lien, encumbrance, adverse
claim or other matter affecting the estate or interest or mortgage thereon covered by this
Commitment other than those shown in Schedule B hereof, and shall fail to disclose such
knowledge to the Company in writing, the Company shall be relieved from liability for any loss or
damage resulting from any act of reliance hereon to the extent the Company is prejudiced by failure
to so disclose such knowledge. If the proposed Insured shall disclose such knowledge to the
Company, or if the Company otherwise acquires actual knowledge of any such defect, lien,
encumbrance, adverse claim or other matter, the Company at its option may amend Schedule B of
this Commitment accordingly, but such amendment shall not relieve the Company from liability
previously incurred pursuant to paragraph 3 of these Conditions and Stipulations.
3. Liability of the Company under this Commitment shall be only to the named proposed Insured and
such parties included under the definition of Insured in the form of policy or policies committed for
and only for actual loss incurred in reliance hereon in undertaking in good faith (a) to comply with
the requirements hereof, or (b) to eliminate exceptions shown in Schedule B, or (c) to acquire or
create the estate or interest or mortgage thereon covered by this Commitment. In no event shall such
liability exceed the amount stated in Schedule A for the policy or policies committed for and such
liability is subject to the insuring provisions and Conditions and Stipulations and the Exclusions
from Coverage of the form of policy or policies committed for in favor of the proposed Insured
which are hereby incorporated by reference and are made a part of this Commitment except as
expressly modified herein.
4. This Commitment is a contract to issue one or more title insurance policies and is not an abstract of
title or a report of the condition of title. Any action or actions or rights of action that the proposed
Insured may have or may bring against the Company arising out of the status of the title to the estate
or interest or the status of the mortgage thereon covered by this Commitment must be based on and
are subject to the provisions of this Commitment.
5. The policy to be issued contains an arbitration clause. All arbitrable matters when the Amount of
Insurance is $2,000,000 or less shall be arbitrated at the option of either the Company or the
Insured as the exclusive remedy of the parties. You may review a copy of the arbitration rules at<

All notices required to be given the Company and any statement in writing required to be furnished the Company shall be
