Authority. This application for a Building Pennit must be signed by the Owner of the property, described above, or an authorized agent. If the signature below is not tl1at of the Owner, a separate
letter of authority, signed by the Owner, must be provided with this Application.
Legal Access. A Building Permit cannot be issued without proof of legal and adequate access to the property for purposes of inspections by the Building Department.
Other Permits. Multiple separate permits may be required: (I) State Electrical Permit, (2) County lSDS Permit, (3) another pe1mit required for use on the property identified above, e.g. State or
County Highway/ Road Access or a State Wastewater Discharge Permit.
Void Permit. A Building Permit becomes null and void if the work authorized is not conuuenccd within 180 days of the date of issuance and if work is suspended or abandoned for a period of 180
days after commencement.

and specifications and other data submitted by me or on my behalf (submittals), based upon my certification as to accuracy.
Assuming completeness of the submittals and approval of this Application, a Building Permit will be issued granting permission to me, as Owner, to construct the structure(s) and facilities detailed on
the submittals reviewed by the Building Department.
In consideration of the issuance of the Building Permit, I agree that I and my agents wiU comply with provisions of any federal, state or local law regulating the work and the Garfield County Building
Code, ISDS regulations and applicable land use regulations (County Regulation(s)). I acknowledge that the Building Permit may be suspended or revoked, upon notice from the County, if the location,
construction or usc of the stn~eture(s) and faci!ity(ies), described above, are not in compliance with County Rcgulation(s) or any other applicable law.

The following items are required by Garfield County for a final Inspection:
I) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30 high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN

MINIMUM APPLICATION REQUIREMENTS
For
MANUFACTURED HOMES
In order to understand the scope of the work intended under a permit application and expedite the issuance of a
permit it is important that complete information be provided. When reviewing a plan and it's discovered that
required information has not been provided by the applicant, this will result in the delay of the permit issuance
and in proceeding with building construction. The owner or contractor shall be required to provide this
information before the plan review can proceed. Other plans that are in line for review may be given attention
before the new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design yonr project and
provide adequate information to facilitate a plan review. Also, please consider using a design professional
for assistance in your design and a construction professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the building, plumbing and
mechanical codes, applicants are requested to review the following checklist prior to and during design.
Applicants are required to indicate appropriately and to submit the completed checklist at time of application for a
permit.
All plans submitted must be incompliance with the 2003 IRC for Residences and 2003 IBC for Commercial
Offices and Storage.

I. Is a site plan included that identifies the location of the proposed structure or addition and distances to the
property lines from each comer of the proposed structure prepared by a licensed surveyor and has the
surveyors signature and professional stamp on the drawing? Properties with slopes of 30% or more must
be shown on site plan. (NOTE Section 106.2) Any site plan for the placement of any portion of a
structure within 50 ft. of a property line and not within a previously surveyed building envelope on a
subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's signature and
professional stamp on the drawing. Any structure to be built within a building envelope of a lot shown on
a recorded subdivision plat shall include a copy of the building envelope as it is shown on the final plat
with the proposed structure located within the envelope.

2. Does the site plan when applicable include the location of the I.S.D.S. (Individual Sewage Disposal System)
and the distances to the property lines, wells (on subject property and adjacent properties), streams or
water courses? This information must be certified by a licensed surveyor with their signature and
professional stamp on the design.

A RESOLUTION CONCERNED WITH THE APPROVAL OF A SPECIAL USE
PERMIT REQUEST FOR "TEMPORARY EMPLOYEE HOUSING" FACILITIES ON A
WELL PAD SKR 698:.04-AV, OPERATED BY CHEVRON USA, INC. AND LOCATED
WITHIN A,PROPERTY OWNED BY CHEVRON USA, INC. GARFIELI>COUNTY;

Recitals
A. The Board of County Commissioners of Garfield County, Colorado, received a Special
Use Pennit request for a "Temporary Employee Housing" facility located north of the Town of
DeBeque and within a property owned by Chevron USA, Inc., Garfield County.
B. The Temporary Employee Housing facility is to be located on Chevron USA, Inc. SKR
698-04-AV (Section 4, Township 6 South, Range 96 West) within a 54,000-acre property owned by
Chevron USA, Inc. and located on CR 211 north ofDeBeque, CO.
C. The subject property is zoned Resource Lands (RL) where such use is permitted as a
Special Use.
D. The Temporary Employee Housing facilities are to be completely contained within an
approved Colorado Oil and Gas Conservation Commission (COGCC) approved natural gas well pad.
The Temporary Employee Housing facility is to accommodate a maximum of24 Individuals at any

one given time.
E. The Temporary Employee Housing facility is to be on location for no more than a
cumulative of one (1) year each.
F. The Board of County Commissioners opened a public hearing on the 20th day of October,
2008 upon the question of whether the above-described SUP should be granted or denied, during
which hearing the public and interested persons were given the opportunity to express their opinions
regarding the issuance of said SUP.
G. The Board of County Commissioners closed the public hearing on the 20th day of October,
2008 to make a final decision.
H. The Board on the basis of substantial ccmpetent evidence produced at the aforementioned
hearing, has made the following determinations of fact:
1. Proper posting and public notice were provided as required for the meeting before the
Board of County Commissioners.
2. The meeting before the Board of County Commissioners was extensive and
complete, that all pertinent facts, matters and issues were submitted and that all interested
parties were heard at that meeting.
3. The above stated and other reasons, the proposed special use permit has been
determined to be in the best interest of the health, safety, morals, convenience, order,
prosperity and welfare of the citizens of Garfield County.
4. The application has met the requirements of Special Use (Sections 5:03 and 5.0221)
the Garfield County Zoning Resolution of 1978, as amended.
RESOLUTION
NOW TIIEREFORE, BE IT RESOLVED by the Board of County Commissioners of Garfield
County, Coloredo, that:
A. The forgoing Recitals are incorporated by this reference as part of the resolution.
B. A Special Use Permit for a "Temporary Employee Housing" facility located north of the
Town ofDeBeque and north of the terminus ofCR 211 within a property owned by Chevron
USA, ln., Garfield County is hereby approved subject to compliance with the following
conditions: ·

1. All representatio11S of the Applicant either within the application or stated at the hearing
before the Board of County Commissioners, shall be considered conditions of approval
unless explicitly altered by the Board. The operation of the facility be done in accordance
with all applicable federal, state, and local regulations governing the operation of this
type of facility. .
2. Emissions of smoke and particulate matter: every use shall be operated so as to comply
with all Federal, State and County air quality Jaws, regulations and standards.
3. Emission of heat, glare, racliation and fumes: every use shall be so operated that it does
not emit heat, glare, radiation or fumes which substantially interfere wi!h the existing use
of adjoining property or which constitotes a public nuisance or hazard .. Flaring of gases,
aircraft warning signals, reflective painting of storage tanks, or oilier such operations
which may be required by law as safety or air pollution control measures shall be
exempted from this provision.
4. Water pollution: in a case in which potential hazards exist, it shall be necessary to install
safeguards designed to comply with the Regulations of the Envirorunental Protection
Agency before operation of the facility may begin. All percolation tests or ground water
resource tests as may be required by local or State Health Officers must be met before
operation of the facility may begin.
5. All Special Use Permits for Temporary Employee Housing are subject to all applicable
building code, state and federal permit requirements, fire protection district requirements
and fire code requirements.
6. Water and wastewater systems proposed to service Temporary Employee Housing must
comply with all applicable state and local laws and regulations.
7. Applicants must keep appropriate records, to be provided to the County upon request to
demonstrate that water supplied to a site is from an approved source and that wastewater
is disposed at an approved facility. For facility serving twenty-four (24) people or less,
the operator must conduct monthly tests (or quarterly if an on-site disiofection system is
installed) and maintain records of stored potable water samples specific for coli form.
Any tests indicating coli form contamioation must be disclosed to !he Garfield County
Board of Health or designee.
8. in no case shall unsafu water be used for drinking nor shall raw sewage or contaminated
water be discharged on the ground surface.
9. The maximum allowable time length of !he Special Use Permit for Temporary Employee
Housing is one (1) year; however, no single Temporary Employee Housing facility

allowed under this Special Use Permit shall be onsite for more than a cumulative of one
year. For good cause shown, the permit may be renewed annually in a public meeting
with notice by agenda only. Annual renewal review shall be based on the standards
herein as well as all conditions of the permit. A permit may be revoked anytime through a
public hearing called up by staff or the Board of County Conunissioners.
10. Inhabitants of the temporary housing shall be applicant's employees, contractor's and/or
subcontractors, working on the related construction or mineral extraction operation, and
not dependents of employees, guests or other family members.
11. Temporary employee housing sites shall be maintained in a clean, safe and sanitary
condition, free of weeds and refuse. Any hazardous or noxious materials that must be
stored on site for operational or security reasons must be managed in accordance with all
applicable federal, state and local laws and regulations.
12. Fire Protection General Requirements: Provisions shall be made for giving alarm in case
of fire. It shall be the responsibility of the duly authorized attendant or caretaker to
inform all employees about means for sununoning frre apparatus, sheriff's office and
resident employees. All frres are subject to §307 of the 2003 International Fire Code
(IFC) including but not limited to permits, attendance, open fires, coal grills, frre bans
and bon fires. One (I) or more approved extinguishers of a type suitable for flammable
liquid or electrical fires (Class A, Class Band Class C), carbon dioxide or dry chemical,
shall be located in an open station so that it will not be necessary to travel more than one
hundred (100) feet to reach the nearest extinguisher.
13. Outdoor food storage is prohibited unless facilities that prevent the attraction of animals
to the temporary employee housing site are provided.
14. The applicant shall provide a detailed map and GPS coordinates to the Garfield County
Sheriff's Office and the relevant Fire Protection District which is sufficient for
emergency response purposes, including location of the temporary employee housing
site; private and public roadways accessing the site, marked as open, gated and/or locked;
and detalled directions to the site from a major public right-of-way. The map is subject to
approval by the Garfield County Sheriff's Office and relevant Fire Protection District.
15. The applicant shall notify the County when site development begins for each facility. The
applicant shall verify in writing, by site plan and through photo documentation that the
site, water system, and sewage disposal system were designed, instailed and inspected in
accordance with the said special use permit and comply with all applicable regulations,
permits, and conditions. All written documentation and site plans verifYing compliance
must be stamped by a certified Colorado Engineer. The County also reserves the right to
inspect a site, without notice, to assess compliance with the Special Use Permit for
Temporary Employee Housing. A determination of noncompliance with any Special Use

Permit for Temporary Employee Housing, or condition approval thereof, is grounds for
revocation or suspension of said permi~ in accordance with Section §9.01.06.
16. No animals shall be allowed at temporary employee housing sites.
17. The maximum number of occupants permitted under this Special Use Permit for
Temporary Employee Housing is twenty-four (24),
18. . Sewage systems shall to be installed and maintained in accordance with the Garfield
County ISDS regulations with all pipes and connections water tight and lids kept securely
in place at all times except during normal cleaning operations.
19. Wildlife-proof refuse containers must be provided for trash. At least one thirty (30)
gallon ( 4 cubic yard) containeohall be provided for each unit or the equivalent in a
