NOTICE
Authority, This application for a Building Pennit must be signed by the Owner of the property, described above, or an authorized agent. If the signature below is not that of the Owner, a separate
Jetter of authority, signed by the Owner, must be provided with this Application,
Legal Access. A Building Permit cannot be issued without proof of legal and adequate access to the property for purposes of inspections by the Building Department.
Other Permits. Multiple separate pennits may be required: (I) State Electrical Permit, (2) County ISDS Permit, (3) another permit required for usc on the property identified above, e,g. State or
County Highway/ Road Access or a State Wastewater Discharge Permit.
Void PermiL A Building Perrnlt becomes null and void if the work authorized is not conunenced within 180 days of the date of issuance and if work is suspended or abandoned for a period of 180
days after commencement.

and specifications and other data submitted by me or on my behalf (submittals), based upon my certification as to accuracy.
Assuming completeness of the submittals and approval of this Application, a Building Permit will be issued granting permission to me, as Owner, to construct the structure(s) and facilities detailed on
the submittals reviewed by the Building Department.
In consideration of the issuance of the Building Permit, 1 agree that I and my agents will comply with provisions of any federal, state or local law regulating the work and the Garfield County Building
Code, ISDS regulations and applicable land use regulations (County Regulation(s)). I acknowledge that the Building Permit may be suspended or revoked, upon notice from the County, if the location,
construction or use of the structure(s) and facility(ies), described above, are not in compliance with County Regulation(s) or any other applicable law.
I hereby grant permission to the Building Department to enter the property, described above, to inspect the work. I further acknowledge that the issuance of the Building Permit does not prevent the
Building Official from: (1) requiring the correction of errors in the submittals, if any, discovered after issuance; or (2) stopping construction or use of the structure(s) or facility(ies) if such is in violation
of County Regulation(s) or any other applicable Jaw.
Review of this Application, including submittals, and inspections of the work by the Building Department do not constitute an acceptance of responsibility or liability by the County of errors, omissions
or discrepancies. As the Owner, I acknowledge that responsibility for compliance with federal, state and local Jaws and County Regulations rest with me and my authorized agents, including without

The following items are required by Garfield County for a final Inspection:
1( 1) A final Electrical Inspection from the Colorado State Electrical Inspector.
~~pY2) Permanent address assigned by Garfield County Building Department and posted at the
v-· structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non~absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non~absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to alllBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan and
it's discovered that required information has not been provided by the applicant, this will result in the
delay of the permit issuance and in proceeding with building construction. The owner or contractor shall
be required to provide this information before the plan review can proceed. Other plans that are in line
for review may be given attention before the new information may be reviewed after it has been
provided to the Building Department.
TWO (2) SETS OF CONSTRUCTION DRAWINGS & TWO (2) SITE PLANS MUST BE
SUBMITTED FOR ALL APPLICATIONS.
Please review tbis document to determine if you have enough information to design your project
and provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project. Any project with more than ten (10) occupants requires the plans to be sealed by a
Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building, plumbing
and mechanical codes, applicants are requested to review the following checklist prior to and during
design. Applicants are required to indicate appropriately and to submit the completed checklist at
time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and drawri
to scale. ·

Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks,
balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line and
original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind speed,
wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
1. Is a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each corner of the proposed structure(s) prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Properties with slopes of
30% or greater must be shown on the site plan. (NOTE Section: 106.2) Any site plan for the
placement of any portion of a structure within 50 ft. of a property line and not within a previously
surveyed building envelope on a subdivision final plat shall be prepared by a licensed surveyor
and have the surveyor's signature and professional stamp on the drawing. Any structure to be
built within a building envelope of a lot shown on a recorded subdivision plat shall include a
copy of the building envelope as it is shown on the fmal plat with the proposed structure located
within~ envelope.

If any required information is missing delays in issuing the permit are to be expected. Work may not
proceed without the issuance of a permit. If it is determined by the Building Official that additional
information is necessary to review the application and plans to determine minimum compliance with the
adopted codes, the application may be placed behind more recent applications for building pei:mits in the
review process and not reviewed until required information has been provided and the application rotates

determined that you do not need to get a variance as the addition doers not appear to encroach into the

*Meeting was called to order at 7:15p.m.

1. President's Report-Patti Payne
a. A meeting was held by the Board of Directors and the residents of Apache
to discuss the results of¢e road survey of Apache. After numerous
discussions about how the road as it is currently ilsed is not in its proper
location, it was concluded that no one is going to do anything about the
results unless we have an issue. ·
b. High Rise Tree Service trimmed the trees along the roads. The Board felt
that more work could have been done .. We did get reimbursed 50% of the
cost by the· state. A vote by show of hands of the homeowners present on .
whether to do more tree trimming in the year 2004 resulted in NO.·
c. After discUssing the Boards attempts at getting lighting for our two
entrance areas, Eileen Koch made a motion to put it to a vote as to whether
we should continue to pursue this subject any further. Susan Dardine

fact that the HOA will NOT put lighting at the two entrances to our
subdivision.
d. There has been an increase in vandalism throughout our neighborhood.

2. · Vice President Report-Linda Carson,
a. After a discussion on weeds. and the possibility of having noxious weeds
sprayed along our roads, a vote was taken by those present with the result
being: NO spraying of any weeds will take place in our subdivision.
b. Wilbur Fix wanted to express his concern that when there is no longer
water in Eric Williams' ditch property owners that border this ditch may
be affected by nmoff.
3. FINANCE
a. The financial book containing Income Statements and Balance Sheets for
the year 2003 was passed around the room for the homeowners to view.
4. WATER&ROADSREPORT-FCDobbs
a. FC sununarized the HOA's dilemma in getting the roads adequately
worked on by Frontier Paving. In general, the work performed was sub-
standard and we have insisted they come back to redo some sections of the
roads. A few homeowners complained about the lack of compaction on
Apache resulting in soft dirt
b. We currently have one bid for snowplowing this winter and another bid is
in the works.
c. Some homeowners expressed their concern about the money spent on
maintaining the "fire road" or the extension of Apache down to the Elk
Run road. The recent work done there required a fair amount of extra dirt,
thus there was the added cost of time and labor. A vote was taken to
continue blading the extension.
d. Instead of cinders or sand to be placed in oilr road barrels, we are looking
into a natural substance called Ice Slicer. It costs $8.50 for 50 pounds and
it doesn't take much to have an effect on icy surfaces. ·
5. NEW BUSINESS
a. Eileen Koch wished to revisit the subject of Board member compensation.
According to her calculations it is costs the subdivision $13,000 annually
for administration. This cost includes monetary payment to the directors
as well. as the waiving of the monthly billings. Some homeowners feel the
billings should not be waived. After a lengthy deliberation involving job
descriptions and time involved, as well as the issue of how compensation
packages have increased homeowner involvement in the Association, it
was agreed that the newly elected Board of Directors would carefully
create a compensation schedule that reflected all the present homeowners
best interests. ·
b. By vote, the Board was granted permission to purchase a laptop 90mputer
for the purpose of conducting HOA business.
6. VOTING
a. A vote was taken to elect seven people to serve as the Board of Directors
to the Elk Creek Romeowners Association. The seven elected
homeowners were : Linda Carson, FC Dobbs, Sarah Pagni, Joyce Gaston,
Eileen Koch, Laurie O'Connor, and Sheryl Klima.
*Meeting was adjourned at 9:45p.m.
*The next Board of Directors meeting was scheduled for Monday November 24, 2003 at

Dear John and Elizabeth,
Please find attached hand drawn sketches of details for construction of the proposed
addition to your home at the above referenced address. We have prepared these
sketches based upon information supplied to us from yourself including the ILC prepared
by High Country Engineering and the hand drawn foundation plan, elevation and floor
plan prepared by yourself. Based upon this information as well as your description of
the construction of the existing home, please note that we have prepared these sketches
to aid in accomplishing the proposed construction. Please note the following:
1. The minimum joist necessary to carry the 15' span indicated is a 9 %" T Jl 210
spaced at 16" o.c. This joists can carry a 15'-9" clear span under a 40 psf live
load combined with a 20 psf dead load.
2. For the porch framing, given the proposal to install 3 porch posts, we would
recommend that each post support be provided by installing an isolated
pier/footing foundation to a depth of 36" below finish grade to bottom of footing.
The concrete pier will need to have a standard Simpson post base installed to
match your intended post size. We would recommend a minimum post size of
4x4. There will need to be a minimum of 6" from the top of concrete pier to finish
grade. The footing would be recommended to be 24" square X 10" deep with
2#5 bars each way located a minimum of 3" from the bottom of the footing. We
would recommend that the pier be a 1 0" diameter pier with 2#5 bars placed
vertically out of the footing into the pier. The height of the 2#5 bars would be
equal to the height of the pier.
3. This design has been prepared without the benefit of a soils report. However, the
design has been prepared to maintain consistency with adjacent observed
construction and minimum on site soil strength assumptions of 2,000 psf for
allowable bearing, 50 pcf for equivalent fluid weight of soil, 120 pcf for density of
compacted on site soils, footings to be placed on native undisturbed soil, footings
to not be placed on fill, all adjacent construction is that of stem wall/footing
construction and no ground water nor leach field influence is to be expected. If,
during excavation, it appears that any of the assumptions are awry, we will need

4. The design has considered a 40 psf live load for the roof as well as the 40 psf live
load for the floor.
5. As this is an addition, it is critical to note that there WILL be differential movement
between the existing building and the proposed construction. We have tried,
however, to minimize the effects of this movement by providing solid ties of
reinforcement between the existing and proposed foundations. Drywall cracking
at the common walls can be expected and will possibly require future repair
through possible re-taping, patching, caulking and/or painting.
