Howard Subject: Chevron Central Processing Facility-Stage 2 Garfield County Foundation Permit No. 11208 Switchgear Building GEG Job No. 3,241 Document Number: PBSR-59836AVF-CIV-RPT-GEG-03241-00002 Dear Mr. Howard, As requested, a Geotechnical Engineering Group (GEG) representative performed observation and testing services on a full time basis during construction of the structure foundations listed above by Garfield County Building Permit No. The GEG representative tested driven pile foundation installation, tested concrete properties, observed reinforcing steel placement and documented structure to foundation anchorage for each of the structures. The purpose of this letter is to present our review of observation and testing data and our conclusions regarding final foundation construction of the Switchgear Building. In summary, the Switchgear Building structures (Permit #11208) was constructed in conformance with the referenced engineering design documents. Reference Documents: Foundation design sheets by Projects Associates, Incorporated 2033-201-CL-1101-01 dated 12/27/07 and revised 11/13/08 (all permits) 2033-201-20-AP-0002-02 dated 5/14/08 and revised 5/18/09 (all permits) 2033-240-30-SP-0004-01, Rev D dated June 21, 2008 (all permits) 2033-201-30-CI-1101-04, Rev 1 dated 6/19/08 (#11208) 2033-201-30-CI-1136-02, Rev 1 dated 6/19/08 (#11208) Chevron Central Processing Facility-Stage 2 Page 2 of 2 Garfield County Foundation Permit Letter Piles, 7-1-09 GEG Job No. 3241 Conclusions Review of our records indicates proper driven pile installation, proper concrete strength, proper reinforcing steel placement and proper structure to foundation anchorage. Based on this review, we conclude the Switchgear Bulding foundation was constructed in conformance with the referenced engineering design documents.
