Box 149 Silt, Colorado 81652 buttonranch@gmail.com Office Locations: Parker, Glenwood Springs, and Siiverthome, Colorado Project No. 16-7-200 Subject: As -Built Evaluation of Onsite Wastewater Treatment System, Proposed Residence, Parcel 11, Section 21, 178 Broken Yoke Drive, South of Silt, Garfield County, Colorado Dear Doug: As requested, H-P/Kumar observed the installation of the Onsite Wastewater Treatment System (OWTS) for the proposed residence at the subject site as required by the Garfield County Building Department. Our observations and recommendations are presented in this report. The services are supplemental to Hepworth-Pawlak Geotechnical's (now HP/Kumar) agreement for professional services to you dated December 31, 2014. We previously designed the OWTS and presented the recommendations in a report dated May 31, 2016, Job No. 114 553A. We performed the As -Built evaluation on July 12, August 15 and September 7, 2016. The OWTS components observed included: sewer pipe, cleanout, sewage lift station, sewage lift station pump operation, septic tank, effluent transport pipe, distribution box, soil treatment area and the locations of these components. Several modifications to our design were implemented by the installer as outlined below. The modifications were verbally approved by us prior to installation and will not change the overall performance of the design. It is our opinion that these modifications will not alter the effectiveness of the OWTS. • A 1,050 gallon Precast Concepts septic tank and concrete distribution box were installed instead of the components specified in the design. These components are considered suitable equivalents. • The septic tank, distribution box and STA were rotated and relocated further to the east as shown in Figure 1. The soils at the STA were similar to those encountered in the profile pits and consisted of Loam. This is acceptable for our design. • A Liberty Pumps sewage pump and sewage lift station basin were used instead of the system recommended in the design. The Liberty Pumps LE41 pump meets the pump performance design requirements. The Liberty Pumps 41 -gallon Pro 380 sewage lift station basin is smaller than the basin specified in the initial design, but is suitable for the OWTS with careful monitoring by the homeowner. The sewage lift station alarm was verified by us. The installation and locations of system components were verified as shown on Figure 1. Photographs of the OWTS installation are provided in the attached Photograph Log. H -P KUMAR Project No. 16-7-200 Doug Button September 15, 2016 Page 2 It is our opinion that the OWTS was installed in general conformance with the May 31, 2016 design with the exceptions listed above. The observations and recommendations presented above are based on our site observations and our experience in the area. We make no warranty either express or implied. Alterations to the system or improper backfill and site grading could affect the operation and effectiveness of the OWTS.
