Persistent Violator - Failure to Monitor
Dear Water Purveyor:
The purpose of this letter is to inform you that your persistent
failure to monitor for bacteriological contaminants is a violation
of the Colorado Primary Drinking Water Regulations.
As you have been previously informed, Article 3.1.1(1) of the
Colorado Primary Drinking Water Regulations requires that each
community water system have a minimum number of samples analyzed
for bacteriological quality each month according to population.
Our records indicate you again did not submit the required monthly
sample(s) for the month of April, 1993. Previously, you failed to
submit proper samples for February, 1993; November, September &
July, 1992;.
If the required bacteriological analysis has been performed, you
are allowed a period of 10 days from receipt of this letter to
submit a PFOTOCOPY of analysis result.
NOTE: TNTC -Too Numerous to Count, CONFLUENT GROWTH, and OUTDATED
SAMPLES are NOT VALID samples and are never counted.
REPLACEMENTS ARE ALWAYS REQUIRED.
As provided in Article 12.1.2(1) of the Colorado Primary Drinking
Water Regulations, you are hereby required to issue a Public Notice
in order to notify those persons supplied by your water system of
your failure to monitor. This notice shall be published in a daily
newspaper, AND in writing in the next set of water bills, or if you
do not issue water bills another form of direct mail must be used.

THIS NOTICE HAS BEEN DISTRIBUTED TO COMPLY WITH THE PRIMARY
DRINIKING WATER REGULATIONS OF THE COLORADO STATE HEALTH
DEPARTMENT.
This notice must be made within 90 days of receipt of this letter
and no later than November 15, 1993 or we may make public notice
in a newspaper or general circulation in your area. As required by
Article 9.1.1(5), copies of all public notices issued must be
furnished to the Department within 10 days of completion.
Our primary concern is the protection of public health. You will
receive an official notification letter from this division each
time you fail to monitor. An enforcement order is being considered
at this time. We are in the process of investigating your system
for all previously known violations. Failure to comply with the
provisions of a Final Enforcement Order will lead to issuance of a
summons requiring your appearance in court. Ultimately, a civil
penalty could be assessed to $1,000 per day for each day your
system was in violation.

RE: Annual Inspection of Water Treatment Facilities, PWS -ID No. 123123,
for Asgard Subdivision, Garfield County.
Dear Homeowners:
Enclosed is your copy of the above report form for the inspection performed
on May 6th, 1993. During the inspections the following items were noted
and need to be brought to your attention:
1) This system was sent a follow up letter dated March 6th, 1992,
requesting that a particulate analysis be performed on the raw water
from the wells in use. The Denver Drinking Water Section will contact
you in regards to this test.
2) An operator, certified at the "D" level, is required for this system.
3) Access points into the pump house wet well need to be sealed. Any
vents must be screened (24 mesh).
4) The overflow and drain pipes coming from the water storage tank need to
be screened (1/4 inch hardware cloth).

2nd Failure To Monitor
Dear Water Purveyor:
The purpose of this letter is to inform you that a failure to monitor for
bacteriological contaminants is a violation of the Colorado Primary Drinking
Water Regulations.
Article 3.1.1 (1) of the Colorado Primary Drinking Water Regulations requires
that each community water system have a minimum number of samples analyzed for
bacteriological quality each month according to population.
Our records indicate you did not submit the required sample(s) for the month
of JULY 1991. In our letter dated August 6, 1991 we had notified you that the
required sampling had not been performed during the month of JUNE 1991. This
failure to monitor is the second violation during the past 12 months.
NOTE: TNTC -Too Numerous to Count, CONFLUENT GROWTH, and OUTDATED SAMPLES are
NOT VALID samples and are never counted. "SAFE" REPLACEMENTS ARE
ALWAYS REQUIRED.
As provided in Article 12.1.2 (1) of the Colorado Primary Drinking Water
Regulations, you are hereby required to issue a Public Notice in order to
notify those persons supplied by your water system of your failure to
monitor. This notice shall be published in a daily newspaper, AND in writing

If the required 'safe' bacteriological analysis has been performed, you are
allowed a period of 10 days from receipt of this letter to submit a photocopy
of the analysis result. Unless we receive the PHOTOCOPY, we must assume
proper samples have not been submitted and you must make public notification
as stated above.
