Sketch map at a minimum scale of 1'=200r showing the legaldescription of the property, dimension and area of all lots orseparate interests to be created, access to a public
right-of-wayr and any proposed easements for drainage,irrigationr access or utilities;Vicinity map at a minimum-scale of L'-2000' showing the general
topographic and geographic relation of t,he proposed exemption tothe surrounding area within two (2) miles, for which a copy oF
U.S.G.S. quadrangle map may be used.
Copy of the deed showing orrnership by the applicant, or a letter
from the property owner(s) if other than the applicant; and
Names and addresses of owners of record of land immediatelyadjoining and within 2OO feet of the proposed exemption, mineral
olrners and lessees of mineral ohrners of record of the property tobe exempted, and tenants of any structure proposed for
conversion; and
Evidence of the soil types and characteristics.of each type; and
Proof of lega1 and adequate source of domestic water for each lotcreated, method of sewage disposalr dnd letter of approval offire protection plan from appropriate fire district; andIf connection to a community or municipal water or sewer systemis proposedT d letter from the governing body staEing avlillingness to serve; andNarrative explaining why exemption is being requested; andIt shal1 be demonstrated that the parcel existed as described
rlanuary 1, L973 or the parcel as it exists presently is onenot more than three parcels created from a larger parcel asexisted on January l, 1973.
A $100.00 fee must be submitted with the application.

Based on the information received by this office and past experience
with sewage disposal facilities in the immediate area of the proposed
exemption, tfrts'office has determined and bel ieves on-site sewage d.rt-
posai facilities to be bothfeasibleand practical' cornmensurate with
botfr County and State regulations, provided proper permits are obtained
prior to construction.
It should be further noted that due to the known conditions, only the
following types of on-site sewage disposal systems would be acceptable'
provided unforseen problems are not encountered:
