Subject: As-Built Evaluation of On-Site Wastewater Treatment System, Proposed
Cabin, l..ot 4, Sillivan Subdivision, 4726 County Road 335, Garfield
County, Colorado
Dear Beth:
As requested by Mark Miller, Hepworth-Pawlak Geotechnical (HP Geotech) observed the
installation of the on-site wastewater treatment system (OWTS) for the proposed cabin at
the subject site. The services are supplemental to our agreement for professional services
to you dated April 2, 2015.
We previously designed the OWTS for the subject cabin and presented our
recommendations in a report dated August 31, 2015, Job No. 115 l30B. The OWTS was
designed for two bedrooms.
The OWTS components observed included: sewer pipe, cleanout, septic tank, effluent
transport pipe, Infiltrator chambers, inspection ports, distribution box and the locations of
components. Our observations and recommendations are presented in this report.
We performed our As-Built site evaluation on December 2, 2015. The installation and
locations of system components were verified as shown on Figure 1. A Precasl Concepts
septic tank was installed instead of the Valley Precast tank specified in our design. We
discussed and approved the change with the contractor prior to installation. The contactor
also installed a Precasl Concepts concrete distribution box instead of the Fralo
distribution box specified in our design. The distribution box must be equipped with
risers and a lid visible from ground surface. This was communicated to the contractor
during our site visit. Photographs of the OWTS instaJlation are provided in the attached
Photograph Log.
It is our opinion that the OWTS was installed in genera] conformance with our August 31,
2015 design.

observations and our experience in the area. We make no warranty either express or
implied. Alterations to the system or improper backfill and site grading could affect the
operation and effectiveness of the OWTS.
