Ms. Shaffer and Garfield County Community Development Department:
The ISDS installion at the above residence in Rifle, Colorado was completed during the fall of2016. An
inspection was performed on the septic tank, distribution box and chambered leach field arranged in a
trench configuration. The system was installed in a slightly modified location from that shown on the
plans and all modifications were approved by myself prior to completion. The specifications were
adhered to and an as-built plan is attached for reference.
