Dave, please find the materials attached for your review and approval. This project was approved via the Garfield County Board of
Commissioners resolution number 2012-60 on July 9, 2012. As discussed during our pre-application meeting on June 12, 2012, this
building permit application is for the screw-pile installation only. The geotechnical engineer will inspect these installations and will
provide Colorado P.E. sealed reports to me that I will submit to the building department for your files. We plan to apply for the
